Book excerpt: Introduction : L'imagerie médicale est au coeur de la prise en charge des patients par sa contribution au diagnostic, au suivi thérapeutique et au traitement de nombreuses pathologies. Afin d'optimiser la pertinence des examens d'imagerie dans un souci constant d'une meilleure qualité de soin, la SFR a élaboré le GBU puis une 2ème version destinée aux médecins généralistes : l'ADERIM. L'objectif principal de notre étude est d'évaluer l'utilisation de ce nouvel outil l'ADERIM auprès des radiologues et de recueillir leur avis dans une perspective d'amélioration et de diffusion du guide. Méthodologie : Étude qualitative par focus groupes réalisée auprès de 23 radiologues afin d'évaluer un outil d'aide à la prescription des examens de radiologie. L'analyse thématique des données recueillies lors des entretiens a été réalisée par la technique de SWOT. Résultats : Les radiologues ont d'abord eu un avis positif du guide. Ils apprécient sa présentation et le trouvent facile et rapide d'utilisation. Cependant, ils soulignent des faiblesses comme des motifs de consultation manquants, une hétérogénéité dans la précision des items, une validité externe et une diffusion du guide insuffisantes. Certaines perspectives d'amélioration ont été proposées telles que l'optimisation de l'ergonomie, l'amélioration de la qualité de l'information des items, le renforcement de la validité externe, la création d'une application et surtout l'amélioration de sa promotion. Des facteurs menaçant son utilisation ont été soulignés, comme un temps de consultation des médecins généralistes limité ou encore l'hétérogénéité de l'accessibilité à l'imagerie selon les territoires de santé. Conclusion : Notre étude a permis de dégager de nombreuses perspectives d'amélioration de l'ADERIM facilitant son utilisation afin d'optimiser la pertinence des actes. S'inspirer d'autres SADM et créer une application intégrant un algorithme décisionnel pourrait être une innovation intéressante. Afin de généraliser nos résultats, une analyse quantitative est réalisée auprès d'un grand nombre de radiologues de France permettant d'évaluer leurs principales attentes.

Il n’existe cependant pas de support adapté pour leur apprendre à lire de façon interactive un examen d’imagerie. Pour répondre à ce problème, nous avons travaillé au développement d’un outil informatique simulant les fonctions de défilement d’une console de visualisation d’examens d’imagerie médicale. Cet outil, consultable sur Internet à l’adresse app.mnapp.tk, a été initialement élaboré pour des étudiants de deuxième cycle des études médicales. Il permet une analyse visuelle volumique à partir de coupes jointives d’examens présentés avec un court texte décrivant le contexte clinique. Il s’agit d’un outil interactif et didactique avec des QCM à remplir par les étudiants. Les réponses à ces questions sont illustrées et assorties de généralités sur la pathologie en question. Les dossiers concernent actuellement essentiellement des examens de médecine nucléaire. Ce logiciel a été testé avec succès lors d’une séance de 2 heures organisée pour des étudiants de 5ème ou 6ème années de médecine. Ceux-ci ont considéré que cet outil était nettement supérieur aux supports de cours existants et qu’il était aussi bien adapté à la préparation au futur concours de l’ECN sur tablette informatique.

Book excerpt: Introduction : les demandes d'examens d'imagerie constituent un moyen de communication entre les médecins généralistes et les radiologues. Dans un but d'amélioration de la qualité des échanges entre ces professionnels de santé, l'URPS des Hauts-de-France a créé une fiche standardisée de demande d'imagerie, à l'attention des médecins généralistes. Notre objectif est d'évaluer l'utilité de cette fiche auprès des médecins généralistes. Matériels et méthodes : il s'agit d'une étude descriptive transversale quantitative interrogeant les médecins généralistes de trois sous-bassins locaux de Toulouse. Cinq fiches de demande d'examen d'imagerie étaient distribuées à chaque médecin du secteur défini. Après utilisation de celles-ci, les médecins ont répondu à un questionnaire pour juger l'utilité et la faisabilité de cette fiche dans leur pratique quotidienne. Résultats : le taux de réponse au questionnaire était de 45,2% (42 réponses sur 93 questionnaires envoyés). 62% des médecins répondeurs ont utilisé la fiche (n = 26). La fiche de demande d'examen d'imagerie était jugée tout à fait utile ou plutôt utile pour 92 % des médecins ayant utilisé la fiche (n = 24). Elle permettait pour 87,5% d'entre eux (n = 21) une meilleure communication avec les radiologues, et un rappel des contre-indications lors de la prescription des IRM et TDM (75%, n = 18). Le manque de temps est ressorti comme le principal frein à l'utilisation de la fiche, cité spontanément par 50 % des répondants (n = 13). Un support informatisé de la fiche, intégré aux logiciels médicaux, est souhaité par la majorité des médecins, ayant utilisé la fiche (76,9%, n = 20) ou non (62,6%, n = 10). Conclusion : une fiche de demande d'examen d'imagerie est utile pour les médecins généralistes, permettant d'améliorer la communication entre les généralistes et les radiologues mais le support papier semble dépassé dans un monde où l'on s'oriente vers le "tout numérique".
