Book excerpt: Introduction : Les anticoagulants oraux directs (AOD) ont démontré leur non-infériorité par rapport aux antivitamines K (AVK) dans la prévention des accidents vasculaires cérébraux (AVC) chez les patients en fibrillation auriculaire (FA). Ils sont potentiellement moins contraignants que les AVK et devraient améliorer le vécu des patients. Aucune étude ne s'est intéressée à ce jour, en France, au vécu des patients anticoagulés pour une FA en ambulatoire. Méthodes : Étude épidémiologique observationnelle et descriptive "en vie réelle", ayant évaluée par 3 questionnaires validés (EQ-5D, PACT-Q2 et MMAS 8) la qualité de vie, la satisfaction et l'observance rapportées par 200 patients suivis en ambulatoire pour une FA, anticoagulés par AVK ou AOD depuis au moins trois mois. Les patients étaient répartis en 4 groupes: AVK, switchés AVK vers AOD (S-AOD), nouvellement sous AOD (N-AOD) et switchés AOD vers AVK (S-AVK). Résultats : Au total 200 patients ont répondu aux questionnaires : respectivement 89, 50, 52 et 9 dans les groupes AVK, S-AOD, N-AOD et S-AVK. Seuls les 3 premiers groupes ont été comparés en raison du faible effectif S-AVK. La qualité de vie (QDV) et la satisfaction du traitement étaient bonnes dans les 3 groupes sans différence significative de QDV, mais avec une satisfaction du traitement significativement meilleure sous AOD que sous AVK, dans la dimension « commodité » (S-AOD et N-AOD vs AVK, p
