Book excerpt: Les deux Plans Cancer ont établi des recommandations en France concernant l'annonce du diagnostic de cancer. Cependant les services non spécialisés en oncologie paraissent peu familiers du dispositif d'annonce, même si des cancers y sont régulièrement découverts et en particulier chez le sujet âgé. A partir de questionnaires destinés aux médecins et aux infirmières, nous avons analysé le déroulement et la traçabilité de l'annonce de cancer pour 34 patients âgés de plus de 75 ans dans 7 services, gériatriques ou non, pendant 19 semaines. Puis nous avons complété par un entretien post-annonce avec le patient. L'enquête préalable révèle que la quasi-totalité des services interrogés ne dispose pas d'un protocole concernant l'annonce de cancer alors que 81% des médecins interrogés déclarent trouver un intérêt à l'étude de cette question. Lors de l'étude prospective, la majorité des patients était autonome sans trouble cognitif, et vivant à domicile. En moyenne l'annonce diagnostique a concerné un patient par mois et par service. Dans 83% des cas le diagnostic a été annoncé au patient, dont 65% concernaient des découvertes de cancer. Les annonces se sont déroulées en majorité le matin lors de la visite, pendant quinze minutes dans la chambre du patient, et en présence du voisin de chambre dans un cas sur quatre.Le mot cancer a été cité dans 70% des cas. Une annonce sur cinq a été faite en premier à la famille. L'existence de troubles cognitifs a paru modifier le comportement du médecin, facilitant l'annonce lorsqu'elle est faite ou au contraire incitant à ne pas annoncer. Au final, 88% des patients se sont déclarés satisfaits de l'information qui leur a été donnée, mais exprimant la possibilité d'une amélioration dans un cas sur quatre. La traçabilité a été majorée par la sensibilisation sur ce sujet puisque la moitié des courriers de sortie comprenaient une information sur l'annonce contre un tiers avant l'étude. Notre étude met en avant les conditions de l'annonce du cancer chez le sujet âgé qui pourraient être améliorées par un cadre mieux défini s'inspirant des directives du dispositif d'annonce, associé à un document assurant une meilleure traçabilité. De même les infirmiers interrogés souhaiteraient une formation relative à l'annonce leur permettant de mieux prendre en charge les patients dans les suites de l'entretien médical. De plus le vocabulaire à employer reste incertain, la bibliographie décrit qu'il faut s'adapter au patient ce qui peut être une difficulté majorée par l'âge et les troubles cognitifs. Il convient également de s'interroger sur le cas particulier des patients porteurs de troubles cognitifs sévères, l'enjeu étant de respecter la qualité de vie du patient sans négliger son droit à l'information. De plus si l'entourage a une place importante dans la prise en charge du sujet âgé, celui-ci doit pouvoir donner son consentement avant la transmission des informations à sa famille. Cette étude nous a donc permis de soulever des problèmatiques diverses sur lesquelles il sera intéressant de travailler en vue d'améliorer nos pratiques et de s'approcher au mieux des conditions de l'annonce recommandées par les plans cancer.

Book excerpt: Introduction : À ce jour, le cancer est une priorité nationale de santé publique et représente la première cause de mortalité en France à savoir près de 30% des décès. Alors, comment annoncer cette maladie sans traumatiser le patient ? L'objectif de ce travail est d'analyser le processus de dispositif d'annonce ainsi que la première évocation du cancer à savoir la pré-annonce et enfin de recueillir l'expérience des patients sur ces deux points. Matériels et méthodes : Enquête observationnelle descriptive, rétrospective, par questionnaire auprès des patients suivis en consultation dans le service d'oncologie du centre hospitalier R. Boulin à Libourne de décembre 2014 à Mai 2015. Résultats : Cette étude a inclus 79 patients suivis en consultation dans le service d'oncologie. Il en ressort que l'évaluation du dispositif d'annonce est positive dans le service avec 75% des patients très satisfaits et 25% satisfaits. Les patients sont peu ou pas satisfaits à 40% de la pré-annonce effectuée en majorité par les médecins généralistes. Discussion : Concernant le dispositif d'annonce, bien qu'encore perfectible, il est bien accueilli par les patients qui souhaiteraient en partie une intégration plus importante du médecin généraliste dans celui-ci. Concernant la pré-annonce, le constat est différent. Cette pré-annonce effectuée majoritairement en ville et par le médecin traitant demande peut-être à être structurée à la manière du dispositif d'annonce afin de mieux remplir ses objectifs.
