Book excerpt: According to Transforming Health Care Scheduling and Access, long waits for treatment are a function of the disjointed manner in which most health systems have evolved to accommodate the needs and the desires of doctors and administrators, rather than those of patients. The result is a health care system that deploys its most valuable resource-highly trained personnel-inefficiently, leading to an unnecessary imbalance between the demand for appointments and the supply of open appointments. This study makes the case that by using the techniques of systems engineering, new approaches to management, and increased patient and family involvement, the current health care system can move forward to one with greater focus on the preferences of patients to provide convenient, efficient, and excellent health care without the need for costly investment. Transforming Health Care Scheduling and Access identifies best practices for making significant improvements in access and system-level change. This report makes recommendations for principles and practices to improve access by promoting efficient scheduling. This study will be a valuable resource for practitioners to progress toward a more patient-focused "How can we help you today?" culture.

Health Care Utilization as a Proxy in Disability Determination identifies types of utilizations that might be good proxies for "listing-level" severity; that is, what represents an impairment, or combination of impairments, that are severe enough to prevent a person from doing any gainful activity, regardless of age, education, or work experience.

Book excerpt: Retail health clinics and urgent care clinics - generally known as walk-in clinics - have represented a large and rapidly growing sector in the landscape of the delivery of health care. These clinics aim to relieve the excess amount of utilization of emergency rooms and give better access to primary care services. The walk-in clinic market has been shown to promote the delivery of care to patients who want to receive care when it is convenient to them, as well as lower the costs of care. This research project aimed to systematically review the literature to analyze the surge in both retail health clinics and urgent care clinics, their scope of practice, and their impact on the healthcare delivery system. The findings show that walk-in clinics are capable of providing the essential primary care needs in a retail clinics and more serious services in an urgent care clinic. Hospital system partnerships have become a trend that has allowed for the delivery of care to fill in the gaps and provide better coordinated care. The continued use of walk-in clinics has created an influence for more clinics to become regulated and certified so their scope of practice will continue to grow. Therefore, policy makers and stakeholders should provide more support in the areas of licensing and regulatory measures. With the proper licensing and regulations, these clinics can broaden the scope of practice to nurse practitioners and physician assistants. Care Without Coverage examines the real consequences for adults who lack health insurance. The study presents findings in the areas of prevention and screening, cancer, chronic illness, hospital-based care, and general health status. The committee looked at the consequences of being uninsured for people suffering from cancer, diabetes, HIV infection and AIDS, heart and kidney disease, mental illness, traumatic injuries, and heart attacks. It focused on the roughly 30 million-one in seven-working-age Americans without health insurance. This group does not include the population over 65 that is covered by Medicare or the nearly 10 million children who are uninsured in this country. The main findings of the report are that working-age Americans without health insurance are more likely to receive too little medical care and receive it too late; be sicker and die sooner; and receive poorer care when they are in the hospital, even for acute situations like a motor vehicle crash.
