Patients with low health literacy are unable to understand, read, comprehend, or discuss the information provided by their healthcare providers. Poor health literacy directly affects disease management and leads to poor patient outcomes. The increase rate of obesity worldwide has quadrupled, the unhealthy lifestyle is one of the risk factors causing adults to develop Type II Diabetes Mellitus (T2DM). T2DM is a condition in where the pancreas does not produce sufficient insulin to absorb the glucose consumed. T2DM can be prevented or controlled with proper management of the disease along with lifestyle changes. The purpose of the project is to identify if the use of the teach-back education method with Type II diabetic patients and health literacy can improve education, communication, and patient outcomes. An analysis of the literature further supports the need to communicate effectively with patients who have T2DM and poor health literacy.

It has been predicted that the number of patients with diabetes worldwide will increase to 360 million by the year 2030, twice the current number. This disease is associated with significant morbidity and mortality resulting from micro and macro vascular complications, ranked as the seventh leading cause of death in the United States, and the number one cause of lower limb amputation. Both the American Diabetes Association (ADA) and the American Association of Diabetes Educators agree that the core component of management and control of the disease is adequate self-care management and education. This project offers an evidenced based practice change project that focuses on a teaching method that has been proven to be effective in teaching patients with diabetes, self-care management. An integrative review of the literature revealed that using the teach-back method when educating patients with diabetes about self-care measures is an invaluable tool. Teach-back is a method of providing health information to the patient on a level that is more easily understood. This is a three-part process that involves assessment, feedback, and evaluation for comprehension of health information. Staff reported knowledge gained after an educational intervention about how to use teach-back when educating clients. And clients reported increase in self-efficacy and confidence with self-care management of diabetes. By enhancing staff knowledge and client confidence, potentially debilitating and/ or fatal complications are decreased, adherence to disease management is increased and clients feels more accountable for their own health. Book excerpt: The present study explores patient perspectives about healthcare providers' effects on health literacy and type 2 diabetes (T2D) from patients' perspectives. As a result, the study gives specific communication and pedagogical strategies that healthcare providers can consider when teaching their patients how to manage their illness. Researchers interviewed or held focus groups with a total of 42 individuals living with T2D. All interviews and focus groups were recorded, transcribed verbatim, and analyzed using constant comparative qualitative techniques. The study shows that, according to patients, providers can both direct and facilitate T2D health literacy development in a number of ways including explaining T2D and how to manage it, considering the context of those explanations, tailoring T2D-related messages to individual patients, providing additional T2D educational resources, and following up with patients. This study extends current literature on patient-provider communication and health literacy by using patients' perspectives to identify specific strategies that healthcare providers can employ in their practices.

While communicating in a health literate manner is important for everyone, it is particularly important when communicating with those with limited health literacy who also experience more serious medication errors, higher rates of hospitalization and use of the emergency room, poor health outcomes, and increased mortality. Over the past decade, research has shown that health literacy interventions can significantly impact various areas including health care costs, outcomes, and health disparities. To understand the extent to which health literacy has been shown to be effective at contributing to the Quadruple Aim of improving the health of communities, providing better care, providing affordable care, and improving the experience of the health care team, the National Academies of Sciences, Engineering, and Medicine convened a public workshop on building the case for health literacy. This publication summarizes the presentations and discussions from the workshop, and highlights important lessons about the role of health literacy in meeting the Quadruple Aim, case studies of organizations that have adopted health literacy, and discussions among the different stakeholders involved in making the case for health literacy.

Book excerpt: Type 2 diabetes mellitus is a significant health issue, as it requires patients to perform daily self-management activities. One in every three Americans with Type 2 diabetes has limited or low health literacy skills. Limited or low health literacy skills can impact a patient’s overall health outcome; therefore, a critical element for disease management and patient adherence is health literacy. The purpose of this three-month scholarly project was to determine if the utilization of an educational intervention specific to one’s level of health literacy, combined with a short message service reinforcement tool, could improve glycemic control and self-efficacy in Type 2 diabetes mellitus patients with limited and low HL. A sample of four participants was recruited for this scholarly project. Participants completed pre and post self-efficacy and health literacy questionnaires. During the three-month intervention phase, participants interacted with the project leader utilizing the text messaging reinforcement tool. After the three-month intervention phase, participants had an overall 9% increase in self-efficacy confidence, a 42% overall increase in health literacy knowledge, and an overall HbA1c mean value reduction of 0.5%. This combination therapy intervention is easy and convenient for health care providers and Type 2 diabetes mellitus patients with limited or low health literacy. This intervention is an efficient and effective method to improve patients self-efficacy and diabetes knowledge; furthermore, this method improves patient self-management skills, glycemic control, and patient longevity.

To successful management diabetes and avoid complications, one must have proficient health literacy ability to understand and follow recommend management techniques efficiently and effectively (Liang & Brach, 2017). Evidence-based diabetes self-management programs that use low health literacy tools have proven to lower the glycated hemoglobin (A1C) levels and improve diabetic knowledge and practices of participants. In this capstone project, a multimodal, low health literacy, diabetes self-management toolkit was implemented in an inner-city clinic among persons with diabetes and various health literacy levels. From the clinic, twelve patients with diabetes were recruited, consented, and enrolled into a tiered diabetes education intervention. Participants were mostly female (n=7), mean age 52 years old, all high school graduates and most having high health literacy. Participant's health literacy level was measured using the Rapid Estimate of Adult Literacy in Medicine (DM-REALM) survey and knowledge was assessed via the Brief Diabetes Knowledge survey pre-intervention and post-intervention. Compliance was measured by self-report and pre-intervention and post-intervention A1C levels. Participants improved medication adherence and endorsed eating less carbohydrates pre to post intervention. The group mean A1C declined pre-intervention to post-intervention while knowledge scores increased across all literacy levels. This three-month evidence-based, diabetes education intervention showed that patients with diabetes could successfully manage their diabetes when provided with low health-literacy tools. More research is needed to explore the impact of diabetes toolkits on diabetes knowledge and A1C's longitudinally in this population. Book excerpt: To maintain good health, patients with Type 2 diabetes (T2DM) must perform self-care activities (SCA), many of which require health literacy – the ability to obtain, find, and use healthcare information to make decisions about health. In this dissertation, using the information-motivation-behavioral skills theoretical framework, I assess and examine health literacy in relation to SCA among underserved people with T2DM. This descriptive correlational study is a secondary analysis of data from a larger study of participants from six federally qualified health centers in an urban county in South Central Texas. The sample comprised 388 patients with T2DM (261 English-speaking, 127 Spanish-speaking; 53±10.33 years old), with the majority reporting an annual income of less than $10,000. Functional health literacy was measured with the Newest Vital Sign screening tool; diabetes-specific oral health literacy, with the DM-REALM – a modification of the original REALM instrument. Only 18.3% of participants exhibited adequate functional health literacy; 10.4%, adequate oral health literacy. In bivariate analysis, functional health literacy was related to age, gender, language, acculturation, income, education, and marital status. Oral health literacy was related to gender, employment status, and type of insurance. Significant predictors of functional health literacy included gender, education, and type of insurance. Significant predictors of oral health literacy included gender, employment status, and years with T2DM. In bivariate analysis, diabetes self-efficacy and health information-seeking behaviors were related to diabetes SCA. In hierarchical multiple regression, functional health literacy, health information-seeking behaviors, and diabetes self-efficacy were significant predictors of diabetes SCA. Although health literacy did not exhibit a significant direct effect on diabetes SCA, health information-seeking behaviors mediated the relationship between oral health literacy and diabetes SCA. These findings add to previous findings showing the high prevalence of limited health literacy among underserved people and the negative influence of limited health literacy on SCA among people with T2DM. Further studies of the impact of limited health literacy on diabetes SCA in diverse, underserved populations of people with T2DM are warranted. Future health literacy interventions should incorporate individual- and systems-level approaches in addressing promotion of diabetes SCA
