Book excerpt: The destruction and force of large-scale, climate-related disasters can overwhelm a community's infrastructure and impede access to and delivery of essential healthcare services. Globally, the frequency and scope of these events are increasing. In October 2012, a 12-ft storm surge from Superstorm Sandy, a massive, destructive storm, precipitated severe flooding across a broad geographic region in the northeastern portion of the United States. Flooding in the New York City metropolitan area caused the evacuation and short-term closure of several hospitals. Our investigation explored how nursing staff at a major metropolitan hospital, forced by the storm flooding to evacuate and temporarily close, were impacted before, during, and after the storm. The study goals were to inform health services planning and preparedness efforts for effective response in future disasters and to ascertain how best to support nursing personnel to function effectively in these types of events. We used a community-based participatory research approach with a sequential mixed-methods study design. We conducted semi-structured interviews and administered an online survey on hospital nursing staff. Community-based participatory research ensures that the research is informed by the experiences, perspectives, preferences, and needs of the community, for example, those who share a collective identity. Our use of community-based participatory research contributed to our obtaining critical firsthand information on the nurses' needs, experiences, and actions during the unexpected sequence of events this disaster generated. Hospital nursing staff, the impacted community, were involved in the planning and execution of the study. This partnership contributed to our success in conducting this investigation.

Book excerpt: Natural disasters-including hurricanes, earthquakes, volcanic eruptions, and floods-caused more than 220,000 deaths worldwide in the first half of 2010 and wreaked havoc on homes, buildings, and the environment. To withstand and recover from natural and human-caused disasters, it is essential that citizens and communities work together to anticipate threats, limit their effects, and rapidly restore functionality after a crisis. Increasing evidence indicates that collaboration between the private and public sectors could improve the ability of a community to prepare for, respond to, and recover from disasters. Several previous National Research Council reports have identified specific examples of the private and public sectors working cooperatively to reduce the effects of a disaster by implementing building codes, retrofitting buildings, improving community education, or issuing extreme-weather warnings. State and federal governments have acknowledged the importance of collaboration between private and public organizations to develop planning for disaster preparedness and response. Despite growing ad hoc experience across the country, there is currently no comprehensive framework to guide private-public collaboration focused on disaster preparedness, response, and recovery. Building Community Disaster Resilience through Private-Public Collaboration assesses the current state of private-public sector collaboration dedicated to strengthening community resilience, identifies gaps in knowledge and practice, and recommends research that could be targeted for investment. Specifically, the book finds that local-level private-public collaboration is essential to the development of community resilience. Sustainable and effective resilience-focused private-public collaboration is dependent on several basic principles that increase communication among all sectors of the community, incorporate flexibility into collaborative networks, and encourage regular reassessment of collaborative missions, goals, and practices.

Despite the investments in research and the growing body of empirical literature on a range of preparedness and response capabilities and functions, there has been no national-level, comprehensive review and grading of evidence for public health emergency preparedness and response practices comparable to those utilized in medicine and other public health fields. Evidence-Based Practice for Public Health Emergency Preparedness and Response reviews the state of the evidence on PHEPR practices and the improvements necessary to move the field forward and to strengthen the PHEPR system. This publication evaluates PHEPR evidence to understand the balance of benefits and harms of PHEPR practices, with a focus on four main areas of PHEPR: engagement with and training of community-based partners to improve the outcomes of at-risk populations after public health emergencies; activation of a public health emergency operations center; communication of public health alerts and guidance to technical audiences during a public health emergency; and implementation of quarantine to reduce the spread of contagious illness.

Because resources are limited in the wake of an emergency, it is increasingly recognized that resilience is critical to a community's ability to reduce long recovery periods after an emergency. This report provides a roadmap for federal, state, and local leaders who are developing plans to enhance community resilience for health security threats and describes options for building community resilience in key areas. Based on findings from a literature review and a series of community and regional focus groups, the authors provide a definition of community resilience in the context of national health security and a set of eight levers and five core components for building resilience. They then describe suggested activities that communities are pursuing and may want to strengthen for community resilience, and they identify challenges to implementation.

The team was also cognizant that the experiences of the community and those that had gone through the process previously were important factors in allowing the current process to proceed in a positive fashion. Disaster preparedness is by no means an exact science. Despite this fact, there are ways to eliminate errors and provide an organization with the highest chance to reduce negative outcomes. This is achieved through experience and repetitive training designed to mimic potential scenarios that may be encountered. Analyzing previous successes and failures of the organization and other entities can provide valuable insight into what strategies will lead to the desired outcomes for the planning committee. Failing to consider the merits of past performance is a surefire way to mismanage a potential impending disaster. Even a successful previous plan can yield data about corrections and improvements when analyzed thoroughly.

As part of its ongoing mission to foster dialogue among stakeholders and to confront the challenges inherent in ensuring the nation's health security, the Institute of Medicine (IOM) Forum on Medical and Public Health Preparedness for Catastrophic Events sponsored a town hall session at the 2012 Public Health Preparedness Summit. This event took place February 21-24 in Anaheim, California. In this session sponsored by the IOM, the focus of discussion was sustaining health care delivery beyond the initial response to a disaster and facilitating the full long-term recovery of the local health care delivery systems. Many elements required for recovery are also fundamental to the day-to-day operations of these systems. Investing in improved health care delivery systems, both financially and through collaborative capacity building, can enhance economic development and growth before a disaster, and also prove instrumental in sustaining services and recovering after a disaster. Post-Incident Recovery Considerations of the Health Care Service Delivery Infrastructure serves as a summary of the session and explains the value of regional capacity building; the importance of interagency, intergovernmental, and public-private collaboration; and the significant role that health care coalitions can play in ensuring resilient communities and national health security.

Due to broad-based congressional interest, GAO assessed the evacuation of hospital patients and nursing home residents. Under the Comptroller General's authority to conduct evaluations on his own initiative, GAO examined (1) the challenges hospital and nursing home administrators faced, (2) the extent to which limitations exist in the design of the National Disaster Medical System (NDMS) to assist with patient evacuations, and (3) the federal requirements for hospital and nursing home disaster and evacuation planning. GAO reviewed documents and interviewed federal officials, and interviewed hospital and nursing home administrators and state and local officials in areas affected by Hurricane Katrina in Mississippi and Hurricane Charley in Florida.
