Book excerpt: For over half a century, policymakers committed to population control have perpetrated a gigantic, costly, and inhumane fraud upon the human race. They have robbed people of the developing countries of their progeny and the people of the developed world of their pocketbooks. Determined to stop population growth at all costs, those Mosher calls "population controllers" have abused women, targeted racial and religious minorities, undermined primary health care programs, and encouraged dictatorial actions if not dictatorship. They have skewed the foreign aid programs of the United States and other developed countries in an anti-natal direction, corrupted dozens of well-intentioned nongovernmental organizations, and impoverished authentic development programs. Blinded by zealotry, they have even embraced the most brutal birth control campaign in history: China's infamous one-child policy, with all its attendant horrors. There is no workable demographic definition of "overpopulation." Those who argue for its premises conjure up images of poverty - low incomes, poor health, unemployment, malnutrition, overcrowded housing to justify anti-natal programs. The irony is that such policies have in many ways caused what they predicted - a world which is poorer materially, less diverse culturally, less advanced economically, and plagued by disease. The population controllers have not only studiously ignored mounting evidence of their multiple failures; they have avoided the biggest story of them all. Fertility rates are in free fall around the globe. Movements with billions of dollars at their disposal, not to mention thousands of paid advocates, do not go quietly to their graves. Moreover, many in the movement are not content to merely achieve zero population growth, they want to see negative population numbers. In their view, our current population should be reduced to one or two billion or so. Such a goal would keep these interest groups fully employed. It would also have dangerous consequences for a global environment.

The NSP translates global, regional and national commitments into national and subnational targets and activities to be implemented to achieve these targets, and provides the basis for mobilizing domestic and external resources for the TB response. It outlines the overall goal(s), strategies and priority interventions, and provides guidance on how these are coordinated across sectors. This guidance is intended for use in developing standalone TB strategic plans, or TB interventions as part of multidisease or health sector plans. It describes key considerations and steps for strategic planning for TB in line with the World Health Organization’s End TB strategy, and the proposed structure of the NSP. The target audience of this publication are all stakeholders involved in national strategic planning for TB (e.g., ministry of health, other government ministries, private sector, civil society, affected communities, academic and research institutions, and technical and funding partners). The current document is an update to the 2015 Toolkit to develop a national strategic plan for TB prevention, care and control. It was developed to better align with global commitments and latest developments in TB and in public health, and builds on lessons learned from using the 2015 toolkit.

Book excerpt: Neglected tropical diseases (NTDs) affect more than 1 billion people globally, causing serious, long-term health effects. Timely, effective interventions such as preventive chemotherapy, which involves the regular administration of safe, effective medicines to a defined population irrespective of its infection status, reduce morbidity and mortality. The new NTD road map advocates preventive chemotherapy as a core intervention to reduce the burden of NTDs. Effective administration of preventive chemotherapy relies largely on operational planning which addresses the need for programmes to reach all eligible people and maximize community demand for the intervention. Local, bottom-up planning (“microplanning”) fosters effective implementation and monitoring of public health programmes. Originally, public health implementers developed microplanning for use in vaccination campaigns, but various programmes, including those for control and elimination of NTDs, now use it. This manual is based on these and other country experiences in microplanning of preventive chemotherapy for NTDs. It describes the basic concepts of microplanning and provides steps and examples of tools to develop and implement a microplan. Since microplanning originates at the lowest level of implementation, and is a bottom-up process, the main target audience of this manual is individuals who are responsible for oversight of preventive chemotherapy against NTDs.
