The need for bone grafting to replace bone defects has become more prevalent recently. There are many bone graft options available for the surgeon each of which has spesific biological and mechanical properties. Autogenous bone is the type used most frequently in oral and maxillofacial surgery. It can be obtained from a host of sites in the body and can be taken in several forms. The tooth can be used for autogenous bone graft because of the larger number of living cells that are transplanted, the more osseous tissue that will be produced.Aim: This study aimed to evaluate the clinical usefulness of autogenous freshly extracted tooth graft for alveolar bone grafting during dental implant surgery.Material Method: Five patients (3 females and 2 males the mean age was 37.2u00b17.3 years) who were referred to our clinic for tooth extraction because of periodontitis. The patients requiring both tooth extraction and alveolar bone regeneration for dental implant placement were included the study. Their freshly extracted teeth (8 teeth) used as an autogenous bone graft after immediate implant (7 implants) placement were enrolled in the study. The autogenous bone grafts inserted between implants and alveolar bone defect.Results: A new bone tissue was seen on the grafting area after 6 months. All implants were osseointegrated and successful. None of implants had any complications ie infection or discomfort.Conclusion: Teeth after extraction can be a useful alternative as an autogenous bone graft materials for the immediate of alveolar bone defects to implant placement.

Book excerpt: Introduction The results of immediate implant placement, with a flapless approach, graft of the residual gap and immediate tooth replacement (FIITR) has been extensively reviewed, showing efficacy in terms of the preservation of the alveolar ridge and good implant survival. However, very little information exists on the clinical results of the immediate implants in total or partial absence of the vestibular bone plate (FIITR-AC). 421Objective Analyse retrospectively the success and aesthetic outcomes of 60 consecutive cases of FIITR-AC with a follow-up of 1 to 7 years, evaluated by means of the Albrektsson criteria (Ac), interproximal bone level changes (IPBLC) and the Pink Esthetic Score (PES). 257Material and methods 59 consecutive patients received a FIITR-AC in the anterior region (60 implants), from 2008 to early 2015, were included in the study. Surgery was performed by only one experienced surgeon (AS) with the help of a surgical microscope and microsurgical instruments. All patients were undergoing periodontal maintenance therapy. Special care was taken during the tooth extraction to preserve as much of the cortical plate as possible, after the flapless placement of the implant, an autogenous tunnel soft tissue graft was made to cover internally the bone dehiscence defect, and a deproteinized bovine bone mineral graft was made in the remaining gap. After achieving adequate implant stability, impressions were taken and a lab-made screw-retained provisional prosthesis was fabricated and delivered to the patient within the following 2 hours.Success (Ac), IPBLC (calibrated periapical digital radiographs), and aesthetic aspects compared to the contralateral tooth (PES) were evaluated. Results 40 implants supported single tooth restorations, while 20 were splinted in multiple-implant supported prosthesis. One of the cases (operated in 1988) failed (1.7%) after 1 month of healing. Interproximal peri-implant bone levels (IPBL) remained stable during the years of follow up (mean 0.7 mm single vs 0.26 mm splinted). The success rate was 98.3% (95%cr= 91.1-99.7%). Good aesthetic results were observed, with even significant better aesthetic outcomes in the single tooth implants than in the contralateral teeth (PES: 12.4 vs 10.7, p=0.002), both at the level of the global PES and its components. 78.3% of the cases (95% cr= 62.8-88.6) showed no buccal soft tissue marginal recession (PES=2), while 21.6% showed a PES of 1 (less than 2 mm). Evaluation was made by a calibrated examiner (AP, Kappa> 0.70). 813Conclusions With the limitations of a retrospective study, it has been observed that FIITR-AC may be a reliable option of treatment for alveoli with partial loss of the buccal plate when adequate implant stability may be obtained, showing a good success rate, the stability of IPBL and good aesthetic results in comparison with contralateral teeth.

Abstract: Background: Type I immediate implant placement has gained popularity because it may reduce treatment time, number of surgeries and post-extraction bone loss. However, this is potentially challenged by inadequate keratinized mucosa for flap adaptation and difficulties in achieving primary stability. Moreover, it has been proven that post-extraction bone loss is an inevitable biological process, which affects treatment outcomes. Objectives: To estimate survival and success rates of implants and the implantsupported prostheses, the prevalence of biological, technical and aesthetic complications, and the magnitude of soft and hard tissue changes following implant placement immediately into fresh extraction sockets. Material & methods: An electronic search in MEDLINE (PubMed) and the Cochrane library from 1991 to July 2010 was performed to include prospective studies on immediate implants with a mean follow-up time of at least 1 year. The survival rates were computed using the STATA statistical software. Weighted means of soft and hard tissue changes were obtained by the inverse variance method. Results: A total of 46 prospective studies, with a mean follow-up time of 2.08 years, were included. The annual failure rate of immediate implants was 0.82% (95% CI: 0.48%-1.39%), translating into the 2-year survival rate of 98.4% (97.3%-99.0%). Among the 5 factors analyzed [reasons for extraction, antibiotic use, position of implant (anterior versus posterior, maxilla versus mandible), type of loading], only the regimen of antibiotic use affected the survival rate significantly. Lower failure rates were found in groups that were provided with a course of post-operative antibiotics. The success of implant therapy was difficult to assess due to scarce reporting on biological, technical and aesthetic complications. Soft tissue changes occurred mostly in the first 3 months after the provision of restoration, and then stabilized towards end of the first year. Marginal bone loss predominantly took place in the first year after implant placement, with a magnitude generally less than 1mm. Controversy on hard tissue preservation with platform-switching technique remained unsolved. Conclusions: Despite the high survival rate observed, more long-term studies are necessary to determine the success of implant treatment provided immediately after tooth extraction. Book excerpt: The placement of implants immediately after tooth extraction has proven to be a predictable treatment with a high success rate. This procedure offers several advantages for the patient as well as for the clinician, including shorter treatment time and fewer surgical procedures. A correct three-dimesional implant position and a management of peri-implant gap are key factors in order to achieve successful functional and esthetic outcomes.The aim of the present 1-year prospective single cohort study was to evaluate the success rate, marginal bone level (MBL) changes and soft tissue stability of tissue-level implants placed in fresh extraction sockets.This prospective cohort study included a total of 10 patients (7 women and 3 men) ranging in age from 23- to 63-years-old (mean 40.1u00b113.3) with at least one tooth in need of tooth extraction and of immediate implant placement. Tooth extractions and implant placements were carried out with a flapless procedure. The tissue level implants (Prama, Sweden & Martina, Due Carrare, Italy) were placed with the machined-collar at the buccal bone wall level and the implants healed transgingivally. The peri-implant bone defects between the implant surface and bone walls were grafted with corticocancellous porcine bone particles and stabilized with a resorbable membrane. Five months after placement the implants were restored. Outcome evaluations were: implant success, MBL changes, width of keratinized gingiva (WKG), facial soft tissue (FST) levels, modified Plaque Index and modified Bleeding Index. The level of statistical significance was set at 0.05 for all analyses.10 Implants were placed in 10 patients, no complication occurred during the healing, no immediate post-operative infections were observed. No patient had implant failure within the first year of treatment. Therefore, a total of 10 patients with 10 implants were followed for the study period. The cumulative survival rate was 100% at 1 year-evaluation. The mean MBL was -0.6u00b10.49 mm at baseline and 0.3u00b10.36 mm at 1 year follow up. The FST Level was 0.4u00b10.69 mm at baseline and 0.02u00b10.70 mm at 1-year follow-up. The Width of Keratinized Gingiva was 3.8u00b1 0.47 mm at baseline and 4.1u00b10.42 mm at 1-year follow-up. Modified bleeding and plaque indexes had low values during the course of the study.The use of an implant with a machined-transgingival concave neck has shown satisfying results in terms of hard and soft tissue levels at 1-year evaluation. Particularly the increase of Width of Keratinized Gingiva can lead to a stable and esthetically successful results. Immediate implant placement can be successfully performed with a tissue-level implant according to a correct three-dimensional position and to a correct management of peri-implant soft tissue healing.
