Abstract: The effect of silver diamine fluoride (SDF) on the mineral content of carious dentine and the preventive caries effect of erbium-doped yttrium aluminium garnet (Er: YAG) laser irradiation (2,940 nm) combined with SDF application under cariogenic biofilm challenge model was investigated. Firstly, the SDF mechanism of action in the remineralisation of demineralised dentine process observing changes in the mineral content in vitro was investigated. Secondly, we investigated the effect of various laser irradiations at sub-ablative energy levels on the fluoride uptake of 38% SDF treated dentine. Finally, we evaluated the effects of Er: YAG laser irradiation combined with SDF application on the fluoride uptake by dentine under a cariogenic biofilm challenge. Compared with a 10% sodium fluoride and potassium fluoride solution and a 42% silver nitrate solution with the same content of fluoride and silver respectively, 38% SDF is more effective in promoting remineralisation of demineralised dentine. The dentine samples treated with SDF did not show evident demineralised surface morphology. Although the lesion depth for SDF treated samples was not significantly deeper than for sodium fluoride treated samples, it was demonstrated that SDF has an inhibitory effect on dentine demineralization. The results of the SDF-laser experiment showed that carbon dioxide (CO2) laser irradiation (10,600 nm) followed by Er: YAG laser irradiation at specific sublablative levels promoted the higher fluoride uptake from SDF application than neodymium-doped yttrium aluminium garnet(Nd: YAG) laser (1,064 nm) and diode laser (810 nm) irradiation. This study provides guidelines for the selection of the appropriate laser irradiation parameters and types to promote an optimal level of fluoride uptake for dentine caries prevention. Er: YAG laser irradiation was elected among the laser types in the previous experiment for promoting higher levels of fluoride uptake and for its maximal absorption by water and high absorption by hydroxyapatite. The Er: YAG SDF-laser treated dentine was subjected cariogenic bacterial challenge to investigate the caries preventive effect of this treatment. The morphological surface of the treated samples presented many occluded dentinal tubules. There was a major increase in the hardness and elastic moduli values and the fluoride content also increased significantly after the remineralisation process. To conclude, this study demonstrated that SDF application inhibited dentine demineralization and arrested caries, and that SDF application followed by Er: YAG laser irradiation promoted an increase in fluoride uptake by sound dentine under a cariogenic biofilm challenge. Abstract: Abstract of thesis entitled Effectiveness of silver diamine fluoride and sodium fluoride varnish in arresting dentine caries submitted by Chu Chun Hung for the degree of Doctor of Philosophy at the University of Hong Kong in May 2004 Introduction: Caries prevalence in young children in China is high and most of the decayed teeth are left untreated. Conventional dental treatments are either not available or are unaffordable in many rural areas. Innovative approaches are needed to improve this situation. Aim: This prospective clinical trial investigated the effectiveness of topical fluoride applications in arresting dentine caries. The first null hypothesis to be tested was that there were no differences in the effectiveness of applying a 38% silver diamine fluoride (SDF) solution (44,800 ppm F), a 5% sodium fluoride (NaF) varnish (22,600 ppm F) and water in arresting dentine caries. The second null hypothesis was that caries removal prior to fluoride application had no impact on its effectiveness in arresting dentine caries. Methods: A total of 375 Chinese kindergarten children, aged 3-5 years with carious upper anterior teeth were divided into five groups. Children in the first and second groups received annual applications of SDF. NaF was applied every three months onto the lesions of children in the third and fourth groups. For children in the first and third groups, soft carious tissues were removed with hand instruments prior to fluoride application at the baseline examination. The fifth group of children received annual applications of water. Follow-up examinations were performed every six months. The children's parents completed a questionnaire to assess parental satisfaction with the appearance of the upper anterior teeth of their child before treatment and again at 24 months after treatment. Results: A total of 308 children were followed for 30 months. Primarily developed for gastrointestinal applications for controlling bleeding, at present it is also used for endoscopic treatment of gastrointestinal tumors, endobronchial cancer, and bladder and gynecological lesions and finding applications in otorhinolaryngology and neurosurgery. De velopment of laser scalpels and focusing head-pieces has now allowed the Nd:YAG laser to be used for open surgical procedures in general and plastic surgery, head and neck surgery, urology, gynecology, dermatology, and neu rosurgery. The rapid development in ceramic technology has led to contact surgery allowing physicians a choice of excision, vaporization, coagulation, incision, or combinations thereof by easily changing probes rather than having to select new laser wavelengths. This technology is rapidly replacing the carbon dioxide laser which currently has no adequate flexible waveguide for fiberoptic en doscopy, cannot be used in a water medium (e.g., bladder), and has poor coagulation properties when compared to the Nd:YAG laser. Future developments may see the Nd:YAG laser even replacing electro cautery in the operating room due to its greater safety and efficacy. Local hyperthermia (laserthermia) with computer control, photodynamic therapy, and ophthalmic applications make the Nd:YAG laser the most exciting tech nological advancement in medicine and surgery for the 1980s.
