Book excerpt: This text aids medical students and trainee doctors in developing their knowledge of the conditions covered and improving their ability to clinically assess patients and formulate a management plan. Once doctors are comfortable in interpreting these graphs, they can use them in consultations with patients about their kidney condition and so support self-management by patients, an increasingly important aspect of healthcare. Kidney diseases and the speciality of nephrology have traditionally been regarded as difficult to understand. This is compounded by the use of complex definitions and terminology. These complex definitions result from a need to categorise kidney diseases according to the way kidney function changes over time, and then to express this in numbers and words. Central to the assessment of patients is the measurement of kidney function. In our teaching of medical students and trainee doctors, as well as in our clinical practice and interaction with GPs and consultant colleagues, we have found that viewing graphs of patients’ kidney function (estimated glomerular filtration rate, or eGFR) against time is a much easier way of understanding kidney diseases. It provides the patient’s ‘kidney history’ and leads into a description of the natural history and management of their condition. We were surprised to find that this graphical approach is not used routinely in teaching or in daily clinical practice. We therefore believe there is a need for a book that uses this approach and we thus sought to compile a collection of illustrative case studies covering an array of disease categories, together with patients’ eGFR readings.

The Harnstein-Symposien Bonn-Wien is an organization designed to bring together scientists from the various fields relevant to urolithiasis: medicine, urology, biochemistry, etc. In the framework of this organiza tion an International Workshop was held in Tel Aviv on December 10 and 11 1980 to discuss the various aspects of uric acid lithiasis. Uric acid stones have been known to mankind for thousands of years. Urate-contain ing stones were found in Egyptian mummies and in a three-thousand-year old mummy from Arizona. Evidently, with the constant increase in standard of living, associated with increased amount of purine intake, which we are facing in this century, the fre quency of uric acid lithiasis is on the increase. In the second half of this century, signi ficant progress has been made in the knowledge of the mechanisms of uric acid over production, as well as in the understanding of the etiology, prevention, and tretament of uric acid stones. As a result, uric acid lithiasis can be prevented and when present it can be treated well. Moreover most uric acid stones can be dissolved in vivo. This workshop included reviews from some of the leading authorities on the various aspects of uric acid metabolism in man: biochemistry, pathology, renal handling, nutri tion, etc. , and these reviews and results of new studies are presented here.

In the first 80 years of the Twentieth Century, the 'over-65' population of Americans increased eight fold. By century's end it will have increased 12-fold and shortly thereafter will include one in five Americans. While initially a fact of the developed world, the pace of similar graying is accelerating even more rapidly in the Second and Third Worlds. This gray delta constitutes about 35 million living Americans, who may use one-half or more of the health care resources. A would have to be a lot more foolish than B if they failed to recognize that in the coming decade the causation, case-mix, and area of the gray delta demands a change from early, mid-or even later-century medicine. If Homer Smith was right in saying, "We are what we are because we have the kind of kidneys we have" and "The kidneys make the stuff of philosophy", then the who, what, where, when, and why the gray delta will be cared for must focus on the stuff of geriatric nephrology.
