Dans cet ouvrage ils abordent plusieurs questions fondamentales: comment évaluer les résultats et lacunes de l'action sanitaire des missions dans le passé? Les communautées chrétiennes actuelles doivent-elles porter le poids d'institutions mises en place hier? Quels rapports les Églises doivent-elles entretenir avec les organisations médicales officielles? Comment les Églises se situent-elles dans la dialectique 'foi-guérison' développée par certains groupes religieux? Plus profondément, comment poser les jalons d'une théologie de la santé et de la guérison? Trouver les reponses sur ces questions complexes c'est une tache très délicate.

Book excerpt: Drawing on various disciplinary perspectives, this book re-focuses the debate on what makes a good health system, with a view to clarifying the uses of social science research in thinking about health care issues in Africa. The explosion of the HIV/AIDS pandemic, the persistence of malaria as a major killer, and the resurgence of diseases like tuberculosis which were previously under control, have brought about changes in the health system, with implications for its governance, especially in view of the diminished capacity of the public health facilities to cope with a complex range of expanded needs. Government responsibilities and objectives in the health sector have been redefined, with private sector entities (both for profit and not-for profit) playing an increasingly visible role in health care provisions. The reasons for collaborative patterns vary, but chronic under-funding of publicly financed health services is often an important factor. Processes of decentralisation and health sector reforms have had mixed effects on health care system performance; while private health insurance markets and private clinics are pointers to a growing stratification of the health market, in line with the intensified income and social differentiation that has occurred over the last two decades.These developments call for health sector reforms.

But what does health mean to women living in a poor neighborhood of an African city? Women in Dar es Salaam see health as primarily related to livelihood, hygiene and care. To stay healthy one has to fulfill basic needs for food, water and shelter, to keep the body and home clean and to take good care of the family. Since the state and newly privatized services hardly reach them and husbands often fail in their role as breadwinners, women bear a growing burden in daily health practice. They become increasingly vulnerable, unless they manage to create a new balance by improving their knowledge, becoming economically more independent and raising support within the household, in social networks and organizations. By shifting the focus from illness to local meanings of health and vulnerability, anthropology can make a unique contribution to the rapidly expanding field of urban health research. Such an actor-centered approach provides fascinating insights and fosters innovative theoretical debates for both scholars and practitioners. With regard to medical anthropology, this study opens new lines of inquiry which may eventually lead to an anthropology of health.

L’expérience des économies développées et émergentes démontre que le produit intérieur brut (PIB) par habitant augmente avec la progression de l’urbanisation. Néanmoins, l’économie de la Côte d’Ivoire affiche des performances insuffisantes sur le plan de l’urbanisation. L’urbanisation et le revenu par habitant ont une corrélation négative depuis la fin des années 1970, et la pauvreté augmente. Au lieu d’envisager le développement des villes individuellement, un plan d’urbanisation réussie devrait considérer les villes de la Côte d’Ivoire comme un portefeuille d’actifs qui se distinguent les uns des autres par leur taille, leur emplacement et la densité de leur population. Les auteurs de L’Urbanisation diversifiée : Le cas de la Côte d’Ivoire identifient trois types de villes, fondées sur leurs contributions à la croissance et à la création d’emplois : les connecteurs globaux ; les connecteurs régionaux, situées le long des corridors d’échanges et des transports régionaux ; et les connecteurs locaux, qui génèrent les économies de localisation nécessaires à l’agro-industrie. Les parties prenantes des administrations nationales et infranationales et du secteur privé ont formulé une vision commune de l’urbanisation en Côte d’Ivoire : « villes planifiées, structurées, compétitives, attractives, inclusives et organisées autour de pôles de développement ». Afin de réaliser cette vision et d’atteindre le statut de pays à revenu intermédiaire, les décideurs politique ivoiriens doivent agir de toute urgence pour promouvoir une urbanisation diversifiée pour tous les types de villes. Cette présente étude identifie des contraintes principales et des enjeux dans quatre domaines : la planification, les connexions, l’écologisation, et le financement des villes.
