Book excerpt: This monograph contains the proceedings from the Advanced Study Institute on "Vascular Endothelium: Physiological Basis of Clinical Problems" which took place in Corfu, Greece in June 1990. The meeting consisted of twenty-eight lectures, most of them adapted as full length papers in this volume, as well as numerous short oral and poster communications which are abstracted and also included in alphabetical order (pages 239-302). There were ninety-six participants from ten NATO and four other European countries. The meeting was the second in as many years dealing with a specific subject in Endothelial Cell biology. Following the 1988 discussion on "Receptors and Transduction Mechanisms", the present ASI recognized and tried to deal with the increasing overlap in interest between basic scientists studying endothelial cell functions and clinicians facing problems of known or suspected endothelial pathological involvement. As with any similar effort, we opted to be selective, rather than fail by trying to be inclusive, in the subjects covered. We chose to discuss diseases, such as atherosclerosis, sepsis, ARDS and stroke, based on their relevance to endothelial cell function and urgent need for new insights into their pathogenesis and treatment. Similarly, we examined endothelial cell functions by considering their relevance to disease and their potential for elucidating important pathologies. Obviously, some areas were covered superficially or not at all; this should not distract from their importance, but rather reflect on the constraints of time and -not at all negligibly -the bias of the organizing committee.

This development arises not only from the well known power of thermodynamics to predict the direction of chemical change, but also from the realization that knowledge of quantitative thermodynamic parameters provides a deeper understanding of many biochemical problems. The present treatise is concerned primarily with building up a reliable data base, particularly ofbiothermodynamic and related quantities, such as partial specific volumes and compressibilities, which will help scientists in basic and applied research to choose correct data in a special field that may not be their own. Most chapters reflect this emphasis on data provision. However, it was also felt that the expert user deserved information on the basic methodology of data acquisition and on the criteria of data selection. Therefore all tables are preceded by a critical evaluation of the techniques as well as a survey of the pertinent studies in the corresponding areas. The surveys are usually self-consistent and provide references to further sources of data that are important but not covered in the present volume. The reader will realize that in different chapters, different symbols have be~n used for the same properties. This unfortunate situation is particularly obvious in those chapters where partial specific or molar quantities had to be introduced; however, it also occurs in those contributions concerning phase changes of macromolecules.

In clinical practice the serum level of albumin continues to be used as an important marker of the presence, progress or ofthe improvement of many diseases, even though it is the complex end result of synthesis, degradation a. nd distribution between intra- and extravascular space. The clinical history of albumin began as early as in 1837, when Ancell first recognized "albumen" and noted that this protein is needed for trans port functions, for maintaining fluidity of the vascular system and for the prevention of edema. However, the important physiological properties of serum proteins and their role in the regulation ofthe oncotic pressure were demonstrated later by the physiologist E. H. Starling in 1895. In 1917 the clinician A. A. Epstein first described the edema in patients with the nephro tic syndrome as being a result of a very low level of serum albumin. Al though the determination of serum albumin concentration became more popular after Howe in 1921 introduced the technique of separation of serum globulins from albumin by sodium sulfate, the first preparations of human serum albumin were made available for clinical use in only 1941 by the development of plasma fractionation by Cohn and his coworkers at Harvard Medical School.
