Book excerpt: This report details the main findings of the implementation of the WHO-UNODC Stop-Overdose-Safely (S-O-S) project in Kazakhstan, Kyrgyzstan, Tajikistan and Ukraine. It provides an overview of the extent of the overdose problem and the development of responses, followed by a description of the methods used in implementation, monitoring and evaluation as measured through a prospective cohort study, focus groups, and qualitative interviews conducted in the four project countries. The S-O-S project resulted in the rapid distribution of take-home naloxone to 14,263 potential opioid overdose witnesses who were trained during the eight-month implementation phase. The cohort study found that 34.5% (out of 1388 study participants) witnessed an overdose during the implementation phase, and 90% of them reported using naloxone in cases of overdose. In almost all instances it was recorded that the victim survived (in 98.3% cases). Extrapolation of the naloxone use figures from the cohort study to the entire S-O-S project sample suggested that the implementation may have resulted in naloxone administration by as many as 4,388 individuals, with many lives saved as a result.

Opioid overdose is easily reversed with the opioid antidote naloxone and with basic life support. Such care is generally only available in medical settings, however. These guidelines recommend that people who are likely to witness an opioid overdose, including people who use opioids, and their family and friends should be given access to naloxone and training in its use so that they can respond to opioid overdose in an emergency if a medical response is not available. Naloxone can be injected or administered intra-nasally and has minimal effects in people who have not used opioids. While naloxone administered by bystanders is a potentially life-saving emergency interim response to opioid overdose, it should not be seen as a replacement for comprehensive medical care.

In 1993, the first version of these guidelines, titled: ‘An outline for the management of alcohol problems: Quality assurance in the treatment of drug dependence project’ was published (Mattick & Jarvis 1993). The Australian Government commissioned an update a decade later (Shand et al. 2003) and a further edition in 2009 to integrate the Guidelines with the Australian Guidelines to Reduce Health Risks from Drinking Alcohol (National Health and Medical Research Council, NHMRC 2009; Haber et al., 2009). The present version of the Guidelines was also commissioned by the Commonwealth of Australia to remain current and integrated with the updated NHMRC consumption guidelines (2020). In order to ensure that guidelines remain relevant, the next set of guidelines should be updated in 2025, consistent with NHMRC recommendation that guidelines be updated every five years. These guidelines aim to provide up-to-date, evidence-based information to clinicians on available treatments for people with alcohol problems and are largely directed towards individual clinicians in practice, such as primary care physicians (general practitioners, nursing staff), specialist medical practitioners, psychologists and other counsellors, and other health professionals. Some chapters highlight service or system level issues that impact on clinicians and their patients. These include recommendations concerning Aboriginal and Torres Strait Islander peoples, culturally and linguistically diverse groups, stigma, and discrimination. Elsewhere, organisation capacity is implied, such as medical resources for withdrawal management where recommendations indicate use of medications. As all forms of treatment will not be readily available or suitable for all populations or settings, these guidelines may require interpretation and adaptation.

In all these, I will always love and remember Africa, the home of my birth and must respect her cultures and traditions as an AFRICAN AUTHOR.

The first two concern movement between the countries of the region, one for general labour and one for sexual exploitation. The third concerns the smuggling of migrants from the region to the rich countries of the West, and the last focuses on migrants smuggled through the region from the poor and conflicted countries of South and Southwest Asia. Drug trafficking: The production and use of opiates has a long history in the region, but the main opiate problem in the 21st century involves the more refined form of the drug: heroin. In addition, methamphetamine has been a threat in parts of East Asia for decades (in the form of yaba tablets), but crystal methamphetamine has recently grown greatly in popularity. Virtually every country in the region has some crystal methamphetamine users, and some populations consume at very high levels.Resources: Resource-related crimes include those related to both extractive industries, such as the illegal harvesting of wildlife and timber, and other crimes that have a negative impact on the environment, such as the dumping of e-waste and the trade in ozone-depleting substances. In all cases, the threat goes beyond borders, jeopardizing the global environmental heritage. These are therefore crimes of inherent international significance, though they are frequently dealt with lightly under local legislation.Counterfeit goods: The trade in counterfeit goods is often perceived as a "soft" form of crime, but can have dangerous consequences for public health and safety. Fraudulent medicines in particular pose a threat to public health, and their use can foster the growth of treatment resistant pathogens.
