Book excerpt: The COVID-19 pandemic has caused unprecedented disruption worldwide highlighting once again the interdependency of health and socioeconomic development, and the global lack of health systems resilience. Two years into the pandemic, most countries report sustained disruptions across service delivery platforms and health areas with a profound impact on health outcomes. The impact of these disruptions is magnified within marginalized communities and in countries experiencing protracted conflict. There is an urgent need to focus on recovery through investment in the essential public health functions (EPHFs) and the foundations of health systems with a focus on primary health care, and whole-of-government and -society engagement. The aim of this Research Topic is to gather, transfer and promote operationalization of key experiences from COVID-19 to inform global and country level recovery that better promote health; guide policy direction towards building health systems resilience; and thereby ensure economic and social prosperity. Experience with COVID-19 has demonstrated that traditional approaches to health system strengthening have failed to achieve the complementary goals of Universal Health Coverage (UHC) and health security with the divide between the most vulnerable and well-off only widening. Much of what had been learned from previous experiences such as Ebola in West Africa has not been widely applied. This has left health and economic systems vulnerable to 21 st century public health challenges, ranging from conflict and natural disasters to aging demographics and rising rates of non-communicable and communicable diseases and antimicrobial resistance. These challenges require intentional focus and investment as well as whole-of-government and -society engagement with health to build health system resilience. Greater action is needed to prevent the devastating effects of war and conflict on the health of the most vulnerable. This Research Topic will convene the knowledge and practices of leaders in public health, health systems, and humanitarian and development sectors. This is to ensure lessons from COVID-19 inform the recovery agenda and promote sustainable health and socioeconomic recovery for all. Lest we forget and find ourselves again unprepared and vulnerable in the face of an even greater threat.

While these were specific to Pakistan, they can be relevant to a wider audience.

Book excerpt: The United Arab Emirates has seen a phenomenal improvement in health development over the last four decades. The country has registered a quantum leap from the dismal situation in the sixties when it was counted among the ten countries with the highest child mortality rates. Today the health of its population, as measured by the child mortality rate, ranks among the most healthy billion of the world population and is comparable to levels in Western Europe and the United States. This dramatic progress, which is the fastest in recorded history, is attributable not only to the country's rapid economic development, but also to the judicious allocation of national resources, the universal provision of basic amenities such as housing, water and food supplies, the high educational levels of women and the good quality of health services, which include preventive and curative primary health care as well as advanced referral services. The health transition in the UAE has been unique in its pattern and is reflected in the changes in disease patterns. The low incidence of alcohol and drug abuse and the low occurrence of AIDS are noteworthy factors. While many diseases have decreased to almost the zero level, others which were formerly rare have increased in incidence. This health transition is also associated with a fall in fertility rates and one of the lowest proportion of elderly citizens in the world. In combination, these factors have resulted in a dramatic change in the age composition of the population.Taking into account the steady growth in the proportion of the elderly and the possibility of higher occurrence of chronic diseases in the middle-aged population, the per capita cost of health care is likely to increase in the future. The main challenge confronting the country in the health sector will be to maintain the present high quality of health services despite these negative trends.

Universally, healthcare workers were forced to face an unprecedented challenge since the outbreak COVID-19. The World Health Organization (WHO) has been trying to reduce transmission of COVID-19 between different countries by setting regulations and standards in attempt to control the transmission of infection. The protection of healthcare workers working as frontline workers for COVID-19 is a priority. To assess the occupational workplace policies to protect healthcare workers in public university hospitals in Egypt during the first wave of COVID-19, this research was conducted at a major public university hospital. These policies were crucial to protect the frontline workers from COVID-19 infection amid the first wave of COVID-19. In depth interviews were conducted to gather information about the current policies in the case study hospital. Results of this research are vital to all public hospitals which provide service a large segment of the population in Egypt. Defective implementation of the workplace policies, lack of needed managerial support, and poor communication between the administration and the healthcare workers were highlighted. The challenge of substandard infrastructure and insufficient resources in the healthcare system during the pandemic at the case study contributed to the crisis. Additionally, the absence of psychological and mental health support needed to health care workers amid the ongoing stress of COVID-19 with all the mental challenges they are facing. The findings of this research give insights to the protective workplace policies of COVID-19 for healthcare workers at the case study public hospitals and challenges in their application and different policy alternatives.

Despite the efforts made by the Ministry of Health in Oman, the country faced several challenges, such as a lack of available funds and patients perceived in a negative manner which hindered their treatment experience. The study examines factors affecting HQ using the SERVQUAL model; patient satisfaction in Omani public hospitals, including healthcare quality, resources availability (RA), and healthcare facility preparedness (HFP); and the moderation effect of the perceived risk of COVID-19 pandemic. This research uses a mixed method with a sample size of 387 patients (quantitatively) selected using probability random sampling of 8 patients (qualitatively) who visited public hospitals The findings from the quantitative analysis reveals that the tangibility and RA had insignificant effect on the HQ. The moderation effect of perceived risk of COVID-19 between HQ, RA, HFP and patient satisfaction was insignificant. The results of the qualitative analysis indicated other factors that affected patient satisfaction, including communication, helpfulness, compassion, physician professionalism, patient-doctor contact, waiting time, atmosphere, and cleanliness. The study proposed a strategy for technical categories that include professional skills, service outcomes and concrete quality aspects, and functional categories. The study has also recommended implications for further investigation.
