Book excerpt: In the past decade, few subjects at the intersection of medicine and sports have generated as much public interest as sports-related concussions - especially among youth. Despite growing awareness of sports-related concussions and campaigns to educate athletes, coaches, physicians, and parents of young athletes about concussion recognition and management, confusion and controversy persist in many areas. Currently, diagnosis is based primarily on the symptoms reported by the individual rather than on objective diagnostic markers, and there is little empirical evidence for the optimal degree and duration of physical rest needed to promote recovery or the best timing and approach for returning to full physical activity. Sports-Related Concussions in Youth: Improving the Science, Changing the Culture reviews the science of sports-related concussions in youth from elementary school through young adulthood, as well as in military personnel and their dependents. This report recommends actions that can be taken by a range of audiences - including research funding agencies, legislatures, state and school superintendents and athletic directors, military organizations, and equipment manufacturers, as well as youth who participate in sports and their parents - to improve what is known about concussions and to reduce their occurrence. Sports-Related Concussions in Youth finds that while some studies provide useful information, much remains unknown about the extent of concussions in youth; how to diagnose, manage, and prevent concussions; and the short- and long-term consequences of concussions as well as repetitive head impacts that do not result in concussion symptoms. The culture of sports negatively influences athletes' self-reporting of concussion symptoms and their adherence to return-to-play guidance. Athletes, their teammates, and, in some cases, coaches and parents may not fully appreciate the health threats posed by concussions. Similarly, military recruits are immersed in a culture that includes devotion to duty and service before self, and the critical nature of concussions may often go unheeded. According to Sports-Related Concussions in Youth, if the youth sports community can adopt the belief that concussions are serious injuries and emphasize care for players with concussions until they are fully recovered, then the culture in which these athletes perform and compete will become much safer. Improving understanding of the extent, causes, effects, and prevention of sports-related concussions is vitally important for the health and well-being of youth athletes. The findings and recommendations in this report set a direction for research to reach this goal.

Book excerpt: Few online concussion education programs have been developed specifically for parents, and those that have been tend to neglect the fact that concussion rates are higher in female rather than male student athletes (Donaldson et al., 2016; Macdonald & Hauber, 2016; Williamson et al., 2014). This quantitative study is important because it addressed gaps in the concussion education literature. The purpose of this study was to examine whether concussion symptoms knowledge and general youth sports-related concussion knowledge was significantly higher among parents of female student athletes who watched the Center for Disease Control and Prevention’s Heads Up concussion videos (intervention group) than among parents of female student athletes who read an online concussion awareness fact sheet (control group). A quasi-experimental, posttest-only control-group design was used to compare concussion knowledge differences between parents who watched the videos and those who read the standard concussion awareness fact sheet. The experiment was conducted using the online survey platform Qualtrics®. The required sample size was 128 participants, with 64 in the intervention group and 64 in the control group. The participants came from the Qualtrics® study pool of participants who met study criteria and were randomly assigned to the intervention or control group. After random assignment and informed consent, parents clicked the screen link that took them to either the videos or the fact sheet. After completing the awareness fact sheet or watching the videos, parents answered an information survey and two questionnaires measuring parental knowledge of concussion symptoms and youth sports-related concussions, respectively. Descriptive statistics were run on participant data and the study dependent variables. Independent samples t-tests were conducted for hypothesis testing. After some professional athletes revealed that their retirement was connected to concussions, public awareness of concussions and identifying the need to protect these high school athletes emerged. Between 2009 and 2013, all 50 states in the U.S. passed a state concussion management policy modeled after the Lystedt Act. In July 2012, the New York State (NYS) Concussion Management and Awareness Act went into effect for all students in public and charter schools, with most schools applying their policies to interscholastic sports. This policy - a mixture of requirements and guidelines - is aimed toward raising awareness and providing students a consistent recovery process. However, the guidelines NYS provides school districts leaves room for variation in implementation across districts. This variation in implementation is called policy translation: the creation and modification of the NYS guidelines to fit each schools’ needs. While some of the variation might help accommodate differences among schools, other variation may negatively impact policy effectiveness. The focus of this thesis is understanding the degree and cause of variations in implementation across high school boundaries. Additionally, implications for future policy developments will be discussed."--Abstract.

Book excerpt: The seventh in a series of congressionally mandated reports on Gulf War veterans health, this volume evaluates traumatic brain injury (TBI) and its association with long-term health affects. That many returning veterans have TBI will likely mean long-term challenges for them and their family members. Further, many veterans will have undiagnosed brain injury because not all TBIs have immediately recognized effects or are easily diagnosed with neuroimaging techniques. In an effort to detail the long term consequences of TBI, the committee read and evaluated some 1,900 studies that made up its literature base, and it developed criteria for inclusion of studies to inform its findings. It is clear that brain injury, whether penetrating or closed, has serious consequences. The committee sought to detail those consequences as clearly as possible and to provide a scientific framework to assist veterans as they return home.
