Book excerpt: "A clinic population in Northern Texas had increased risk of type 2 diabetes mellitus (T2DM) due to food choices, sedentary lifestyles, and stress, contributing to increased morbidity and mortality. This paper presents an evidenced-base practice (EBP) change project addressing uncontrolled T2DM in adults at a private practice in Northern Texas involving the implementation of an educational intervention for patients with uncontrolled T2DM at the clinical practice setting. Outcomes were measured by using the Revised Diabetes Knowledge Test as the measurement tool and tracking changes in glycemic index of the participants pre and post intervention. Data analysis revealed an increase in knowledge acquisition and a reduction in glycemic index.. As a result of the findings of this EBP project, policy changes were made in the clinical practice setting, to include diabetes education as part of the treatment regimen. Health care professional goals for diabetic patients should be to prevent complications caused by the disease. Also professional goals should be to educate the patient on the importance to achieve and maintain optimal control to increase their quality of life. This research reviews the impact of diabetes education for patients with type II diabetes and then presents a project for the presentation of diabetic education and importance of adherence to a volunteer group of type II diabetic participants from randomly selected churches in a central Louisianaa community. Sixteen people participated in the educational intervention. Ager each participant signed a consent form, and completed a survey for demographical information, a pretest was given before a sixty minute lecture/discussion on diabetes type II and importance of adherence to medical treatment. After completion of the discussion, a posttest was given to evaluate the effectiveness of the program. Eight weeks ofter the program, a telephone survey was completed. The intervention showed that diabetes education in successful in helping patients adhere to their medical regimen.

It has been predicted that the number of patients with diabetes worldwide will increase to 360 million by the year 2030, twice the current number. This disease is associated with significant morbidity and mortality resulting from micro and macro vascular complications, ranked as the seventh leading cause of death in the United States, and the number one cause of lower limb amputation. Both the American Diabetes Association (ADA) and the American Association of Diabetes Educators agree that the core component of management and control of the disease is adequate self-care management and education. This project offers an evidenced based practice change project that focuses on a teaching method that has been proven to be effective in teaching patients with diabetes, self-care management. An integrative review of the literature revealed that using the teach-back method when educating patients with diabetes about self-care measures is an invaluable tool. Teach-back is a method of providing health information to the patient on a level that is more easily understood. This is a three-part process that involves assessment, feedback, and evaluation for comprehension of health information. Staff reported knowledge gained after an educational intervention about how to use teach-back when educating clients. And clients reported increase in self-efficacy and confidence with self-care management of diabetes. By enhancing staff knowledge and client confidence, potentially debilitating and/ or fatal complications are decreased, adherence to disease management is increased and clients feels more accountable for their own health. Book excerpt: Over 30 million Americans have been diagnosed with diabetes with 90 to 95% of these patients having Type II (Centers for Disease Control and Prevention [CDC], 2021). Diabetes self-management education (DSME) is a cost-effective but underutilized intervention for patients (CDC, 2021). There have been few studies conducted to examine the effect of simulation as a patient education method. Purpose: This study aimed to examine the effect of a simulation-based educational intervention on diabetes self-care management. Methods: Subjects were adults diagnosed with diabetes with a glycated hemoglobin test (HbA1c) greater than or equal to eight, obtained within the past six months. The participants were randomized into the simulation or educational handout group. The instrument used was the Diabetes Self-Management Questionnaire (DSMQ), which assessed self-care interventions required for glycemic control (Schmitt, 2013). Both groups completed the DSMQ pretest and demographic survey, and then completed the DSMQ again at six weeks post intervention. Results: No formal comparisons were made due to low sample. The mean total DSMQ score was in 7.6 the simulation group and 5.9 in the handout group at pretest. The mean total DSMQ score was 9.1 in the simulation group and 7.5 in the handout group at posttest. Conclusions: The study suggested that immersive learning experiences may be effective in adults with diabetes, regardless of time since diagnosis. More formal education is needed for patients with type II diabetes, especially regarding nutrition. Further studies are needed to further explore the gaps on the use of simulation in patient education.

The purpose of this evidence-based practice (EBP) change project was to implement Purpose: The purpose of this evidence-based change project was to provide diabetes self-management education (DSME) to participants in a federally qualified health clinic (FQHC) to increase patient knowledge of diabetes self-management behaviors and decrease fasting blood glucose levels. Methods: Thirteen adults with uncontrolled Type 2 Diabetes; able to read, write, and speak English; and who had access to a telephone and a computer or tablet with a stable internet connection were voluntarily recruited at a FQHC. Four bi-weekly DSME one-on-one virtual sessions lasting one hour occurred over a nine-week period, with a 30-minute follow-up phone session during weeks when DSME was not scheduled. Using a pretest/posttest approach, participants' diabetes self-management knowledge, using the Revised Michigan Diabetes Knowledge Scale (RMDKS), and fasting blood glucose were measured before and after the educational intervention session. Statistical analysis utilized a paired t-test for comparison. Results: The data showed a statistically significant post-intervention improvement in both patients' diabetes knowledge with a mean pretest knowledge score of 60% and mean posttest score of 77% (p
