Des pays aussi divers que le Brésil, la France, le Japon, la Thaïlande et la Turquie ont réussi à mettre en œuvre la CSU et illustrent comment ces programmes peuvent à la fois agir en tant que dispositifs essentiels d’amélioration de la santé et du bien-être de leurs citoyens et mettre en place les fondations d’une croissance économique basée sur des principes d’équité et de durabilité. L’assurance d’un accès universel à des services de santé abordables et de qualité contribuera de façon importante à l’éradication de la pauvreté extrême en 2030 et à la stimulation d’une prospérité partagée dans les pays à revenu faible et intermédiaire, où vit la majorité de la population pauvre mondiale. 'La couverture sanitaire universelle pour un développement durable inclusif' synthétise les expériences de 11 pays †“ Bangladesh, Brésil, Éthiopie, France, Ghana, Indonésie, Japon, Pérou, Thaïlande, Turquie et Vietnam †“ dans la mise en œuvre de politiques et de stratégies d’atteinte et de maintien de la CSU. Tous ces pays se sont engagés envers l’atteinte de la CSU, considérée comme une aspiration nationale clé, mais ont adopté à cet effet des approches distinctes. L’ouvrage examine les politiques de CSU de chaque pays à partir de 3 thèmes communs : (1) l’économie politique et le processus politique d’adoption, d’atteinte et de maintien de la CSU; (2) les politiques financières d’amélioration de la couverture sanitaire; et (3) les ressources humaines en santé au service des politiques d’atteinte de la CSU. Les constats tirés de ces études pays sont une source d’enseignements auxquels peuvent faire référence les pays qui aspirent à l’adoption, à l’atteinte et au maintien de la CSU. Même si la voie vers la CSU reste spécifique à chaque pays, tous peuvent profiter des expériences des autres en apprenant des différentes approches et en évitant les risques potentiels.

Cameroon faces three main challenges: Developing more robust formal and informal employment opportunities by strengthening human development. Increasing productivity in agriculture, mining, and key value chains (timber, tourism, and information and communication technology). Advancing growth by investing in infrastructure and improving the business climate and regional integration. The Strategy sets a target for reducing underemployment from 76 percent of the workforce to 50 by 2020 by creating tens of thousands of formal jobs. But based on results from the first two years of its implementation, the strategy is far from achieving that target. This report is meant to support Cameroon s efforts to build the skills of its workers. This report reaches conclusions and offers policy recommendations to answer six questions: What has been the trajectory of Cameroon s economic growth? Which sectors have contributed to growth? What jobs are being created? What types of skills are being used in the sectors where the highest percentages of the population are employed? What are the demand and supply barriers to skills? Which policies and institutions are in play? Are they sufficient? What needs to or could be reformed? Cameroon has good prospects for moving to middle-income status. It can create a more dynamic, responsive workforce. But a new strategy is required. It can be done.This report proposes new directions and provides recommendations. Outcomes are expected. Work has never been easy. But many have been working and Cameroon can work."

Cette ascension peut être accélérée par un dividende démographique dû aux changements dans la structure par âge de la population. Les baisses de la mortalité infantile, suivies par la baisse de la fécondité, produisent une génération «renflement» et un grand nombre de personnes d'âge actif, donnant un coup de pouce à l'économie. Dans le court terme, une fécondité plus faible engendre une baisse des taux de dépendance des jeunes et une plus grande participation de la population active féminine en dehors de la maison. La réduction de la taille de la famille signifie également qu’il reste davantage de ressources à investir dans la santé et l'éducation par enfant, augmentant ainsi la productivité future des travailleurs. Au long terme, une durée de vie accrue résultant de l’amélioration dans le domaine de la santé signifie que cette vaste cohorte aux gains élevés sera également désireuse d’épargner pour la retraite ainsi que pour la création de l'épargne et des investissements plus conséquents, conduisant ainsi à d'autres gains de productivité. Deux choses sont nécessaires pour que le dividende démographique génère un décollage économique de l'Afrique. La première consiste à accélérer la baisse de la fécondité qui est actuellement au point mort ou lente dans de nombreux pays. La seconde est les politiques économiques qui profitent de l'occasion offerte par le changement de la démographie. Alors que l'évolution démographique peut produire plus, et des travailleurs de meilleure qualité, cette main-d'œuvre potentielle doit être employée si l'Afrique se doit de récolter le dividende. Cependant, une fois en route, la relation entre l'évolution démographique et le développement humain fonctionne dans les deux sens, c’est-à -dire qu’elle crée un cercle vertueux susceptible d’accélérer la baisse de la fécondité, le développement social et la croissance économique. Les recherches scientifiques montrent trois facteurs clés pour accélérer la transition de la fécondité: la santé des enfants, l'éducation des femmes et l'autonomisation des femmes, notamment par l'accès à la planification familiale. Exploiter le dividende démographique nécessite la création d'emplois pour les grandes cohortes de jeunes qui entrent en âge de travailler et qui stimulent les investissements étrangers jusqu'à faire augmenter l'épargne intérieure et l'investissement. La combinaison appropriée de politiques dans chaque pays dépend de leur stade de transition démographique. Etant la dernière région à subir la transition démographique, l’Afrique peut tirer les leçons des réussites et des échecs des autres régions dans l'exploitation d'un dividende démographique. Le succès exige (i) l'accélération de la transition démographique; (ii) une économie dynamique produisant des revenus et des investissements plus élevés pour une jeunesse mieux éduquée et en meilleure santé.

Book excerpt: The 2018 FAO-OIE-WHO (Tripartite) zoonoses guide, “Taking A Multisectoral, One Health Approach: A Tripartite Guide to Addressing Zoonotic Diseases in Countries” (2018 TZG) is being jointly developed to provide member countries with practical guidance on OH approaches to build national mechanisms for multisectoral coordination, communication, and collaboration to address zoonotic disease threats at the animal-human-environment interface. The 2018 TZG updates and expands on the guidance in the one previous jointly-developed, zoonoses-specific guidance document: the 2008 Tripartite “Zoonotic Diseases: A Guide to Establishing Collaboration between Animal and Human Health Sectors at the Country Level”, developed in WHO South-East Asia Region and Western Pacific Region. The 2018 TZG supports building by countries of the resilience and capacity to address emerging and endemic zoonotic diseases such as avian influenza, rabies, Ebola, and Rift Valley fever, as well as food-borne diseases and antimicrobial resistance, and to minimize their impacts on health, livelihoods, and economies. It additionally supports country efforts to implement WHO International Health Regulations (2005) and OIE international standards, to address gaps identified through external and internal health system evaluations, and to achieve targets of the Sustainable Development Goals. The 2018 TZG provides relevant country ministries and agencies with lessons learned and good practices identified from country-level experiences in taking OH approaches for preparedness, prevention, detection and response to zoonotic disease threats, and provides guidance on multisectoral communication, coordination, and collaboration. It informs on regional and country-level OH activities and relevant unisectoral and multisectoral tools available for countries to use.
