Book excerpt: Excerpt from Typhoid Fever Considered as a Problem of Scientific Medicine This treatise is an attempted exposition of the problem of typhoid fever. It aims to treat historically the development and present status of our knowledge concerning this important malady as viewed from the standpoint of its mechanism. It is not primarily designed to aid directly in the clinic or the laboratory, but should serve to point out the relations of one to the other, to indicate the dependence of practice on theory, and the happy applicability to human need of investigation that may have seemed to aim merely at the gratification of intellectual curiosity. There are numerous admirable clinical treatises on typhoid fever, such as those of Curschmann, of Vincent and Muratet and of McCrae. The purely laboratory and the public health aspects of the disease are well summarized in such works as those of Kutscher, of Chapin and of Whipple. This book aims to strike a balance between these sources of information, following the life history of the typhoid bacillus rather than the manifestations of the disease it produces, in order to gain insight into the nature of the problem as a whole. No attempt has been made to make this work encyclopedic in character, although the references to original sources are sufficiently ample to lead a prospective investigator to full sources of information. Although we may have succeeded in maintaining correct proportions in our estimate of the accuracy and value of facts, methods and theories that have been shaken down by time into their proper relations to one another, we shall unconsciously or wilfully have exaggerated the importance of recent contributions. We may well have overemphasized the importance of modern investigations in respect to antityphoid vaccination and specific therapy in typhoid fever through personal enthusiasm. The data on these aspects of typhoid fever are so ample and so recent that a critical summary of their essentials seems desirable, even though somewhat premature. In addition to the use that has been made of all the general treatises and original sources of information to be cited, particular indebtedness is felt to Charles Murchison's "Treatise on the Continued Fevers" as furnishing the backbone of the discussion of the earlier historical aspects of typhoid fever. Temperature and danger need not correspond. The very feeble are not as a rule subject to high temperatures any more than the very old; and quite often the worst cases of sepsis are those which exhibit low temperatures. That is a fact best known to those who see much diphtheria or much puerperal fever. Observations of high temperatures previous to the appear ance and recognition of the Symptoms are not frequent. While an adult would be about his work, the infant or child is seldom considered sick enough to claim attention and attendance. That is why chilliness and chills are readily overlooked; indeed, the latter are not marked as a rule in any illness of the young. The rise of the temperature in the typhoid of the young is mostly gradual it is high in the second stage with slight remissions and gradually falls toward the end of the disease. This rule, if it can be called so, is, however, subject to many exceptions. The temperature of small infants may be very irregular, is in many cases rather low and uniform, in others high with few and short remissions. Irregularities, moreover, often depend on complications. After all, neither those are always right who consider the typhoid of the nursling and infant as a uni formly severe disease, like Baginsky and Roemheld, nor those who make light of it. The degree of individual infection, and the nature of the epidemic are factors that have to be considered. Anyone who has had even a limited experience with typhoid fever has met with cases in which the manifestations wandered so far from the classical descriptions of the disease as to be puzzling and obscure, or with instances in which the malady has been so altered in its course by intercurrent affections as to be unusual and to call forth all the diagnostic knowledge and therapeutic skill of the physician. The following pages deal with these aberrant forms of the dis ease and the courses which they pursue. The preceding paragraph, taken from the preface of the first edition of this book, gives in a concise form the reason for the appearance of the present volume. During the ten years which have elapsed since the first edition appeared the literature dealing with typhoid fever has greatly increased, and the advances which have been made in the study of the disease by bacteriological methods have added to our knowledge of many of its complica tions and sequels. The present time, therefore, seemed auspicious to bring the text of the first edition up to date, and this has been accomplished with the assistance of the unior author. During the last decade much interest has also been taken in the complications and sequels of the exanthematous fevers other than typhoid, and for this reason chapters dealing with these phases of Variola, scarlet fever, measles, chickenpox, and rubella have been carefully prepared and added to the original text. The special interest and importance, in a surgical sense, of the great European War lies not so much in the fact that examples of every form of gross lesion of organs and limbs have been seen, for if we read the older writers we find little in the moderns that is new in this respect, but is to be found in the enormous mass of clinical material which has been presented to us and in the production of evidence sufficient to eliminate sources of error in determining important conclusions.
