However, before cardiologists could fully validate its clinical utility, they were confronted with a further development, namely color-coded Doppler flow imaging. This new technique allows a comprehensive study of the direction, velocity, uniformity and timing of intracardiac blood flow, while simultaneously revealing cardiac struc tures and their movement. 'Color Doppler' facilitates the diagnosis of a variety of cardiac conditions, and an overwhelming amount of information is available at any instant. Interpretation of these fascinating images requires a substantial experience and theoretical background. This monograph updates the application of color Doppler to both congenital and acquired heart disease. The work comprises 15 chapters written by authorities in the field, each of whom presents his most recent experience in the field. In addition, further advantages in the clinical use of pulsed and continuous wave Doppler in pediatric and adult cardiol ogy are presented. This book contains, of course, many color plates. 1. Book excerpt: Just a very few years after Edler and Hertz had described the clinical use of M-mode echocardiographyl Satomura reported the application of Dop 2 pler ultrasound to the study of cardiac function. Yet Doppler ultrasound has been integrated into diagnostic practice in cardiology much more slowly than conventional (M-mode and two-dimensional) echocardiogra phy. Now, however, tremendous growth in the interest of clinicians in the diagnostic use of Doppler ultrasound can be observed and may in fact be due to the recent advent of color flow imaging. The reason for this growth may be that this method makes it possible to directly visualize the blood flow in the cardiovascular system in cross-sectional views. Moreover, the results are reproducible and much easier to understand than the older mapping techniques using a single-gate Doppler. In its short existence many different names have been used to describe this method, for instance, color Doppler, color flow imaging, real-time two-dimen sional Doppler echocardiography, and Doppler flow imaging. This diver sity reflects the large interest that many researchers have shown in this method. The technical development of color blood flow imaging (CBFI) - as this method will be called in this book - has not yet reached a universally accepted standard of performance in cardiology. Despite this state of flux and the uncertainty about future developments, I think it is justified to dedicate an entire book to this fascinating method.

The work of Dr. Liv Hatle of Trondheim, who was then using analogue tracings for cardiac Doppler, inspired me to try CW Doppler combined with spectral analysis and high-pass filtering techniques. The results with this system, without benefit of standard echocardiography were so pro ductive that a special instrument was produced by Carolina Medical Electronics for worldwide marketing. Our experience led to the development of an interna tional symposium and training program. These symposia gave birth to the Inter national Cardiac Doppler Society and Volume I of this book series. This second volume on Cardiac Doppler Diagnosis is an outgrowth of the successofVolume I and the excellent work of many clinical scientists. Doppler instrumentation continues to improve with integration ofboth contin uous wave and pulsed wave Doppler with echocardiography. Color directional Doppler imaging techniques led byourJapanese colleagues is finding acceptance as a rapid screening method to find special regions of interest where more quantitative continuous wave with its quantitative capabilities of Doppler can be brought to bear.
