Book excerpt: Excerpt from Tumors of the Nervus Acusticus, and the Syndrome of the Cerebellopontile Angle In the course of preparation of a monograph dealing with a series of meningeal fibro-endotheliomata, a careful review was necessitated of the pathological as well as the clinical aspects of these interesting tumors. They have their point of origin in certain definite regions, and a tentative subdivision had been made of those arising from the spinal meninges, those from the basilar meninges, and those from the superior envelopes of the brain. It was apparent that the spinal and basilar lesions usually arose from the meninges at the point of exit of a spinal or cerebral nerve root, and it was anticipated that many of the tumors of the cerebellopontile angle which involve the acoustic nerve would be included in the series, for the majority of them had previously been diagnosed from their gross appearance, though admittedly with some reservation on histological grounds, as endotheliomata. Hesitation was felt in regard to the inclusion in the series of some of the spinal cord tumors, and these doubts became intensified when the lateral recess tumors came to be assembled and closely inspected. A thorough rehearsal of the material at hand, comprising twenty-nine histologically certified cases, together with a much larger number of probable though unverified ones, which nevertheless were useful from the standpoint of their clinical data, so clarified many obscure matters relating to these peculiar and unmistakable tumors of the VIIIth nerve that they have been made the subject of this separate study, and a report upon the 60 endotheliomata proper must await its turn. Unquestionably the acoustic tumors are most distinctive growths and such relationship as they have to the meningeal tumors occurring in the lateral recess will be pointed out in its proper place. Some important monographs on the subject have already been published, of which Folke Henschen sInaugural Dissertation, 1910, is the most noteworthy, but in all of them the various tumors of the cerebellopontile angle have been incorporated, whereas the acoustic neuromas will alone occupy our attention to the exclusion of other tumors of the recess except in so far as they are of interest from the standpoint of differential diagnosis. Most publications on the topic of brain tumors attempt to cover the entire subject from one aspect or another, but the time is ripe for special studies of special tumors in special localities, particularly if the surgical treatment of these difficult lesions is to be perfected. Unquestionably the author profits most by an analytical study of a given group of cases, but inasmuch as the series of acoustic tumors herein reported is apparently the largest to have been verified in a single operative clinic, the study may be of some interest and value to others as well, and the future will doubtless see many improvements in the surgical procedure, with a still greater lowering of the operative morality, which in the past has been so high as to be prohibitive. Book excerpt: The cerebello-pontine angle has always posed a challenge to the neurosurgeon, the otoneurosurgeon, and the neuroradiologist. Angle masses which are very small and difficult to detect frequently produce symptoms, but may remain silent while growing to exceptional size. The neuroradiologist must have firm knowl edge of the clinical manifestations of the diverse angle lesions in order to tailor his studies to the patients' needs. The majority of angle lesions are benign; thus successful surgery has the potential for complete cure. Angle lesions typically arise in conjunction with vital neurovascular structures, and often displace these away from their expected positions. Large lesions may attenuate the vestibulocochlear and facial nerves and thin them over their dome. Since the nerves often remain functional, the surgeon then faces the need to separate the tumor from the contiguous nerve, with preservation of neurological function. Depending on the exact location and extension of the lesion, resection may best be attempted via otologic or neurosurgical approaches. The neuroradiologist must determine - precisely -the presence, site, size, and extension( s) of the lesion and the displacement of vital neurovascular structures as a guide to selecting the line of surgical attack. Since the arteries, veins, and nerves that traverse the angle are fine structures, the neuroradiologist must perform studies of the highest quality to do his job effectively.
