Book excerpt: « Peu de nourrissons ont le privilège d'être allaités durant leurs premiers mois de vie, contrairement à ce qui est souhaité par l'Organisation mondiale de la santé pour l'ensemble de ceux-ci (2020). En Chaudière-Appalaches, les taux d'allaitement demeurent inférieurs à ceux du reste du pays (Lavoie & Dumitru, 2011), malgré de nombreuses initiatives en promotion et soutien en allaitement. La littérature scientifique révèle que plusieurs facteurs sociodémographiques et psychosociaux sembleraient forger l'intention d'allaiter des femmes enceintes. Toutefois, l'influence d'éléments culturels tels que les rapports de sexe et l'objectification sexuelle des femmes n'aurait pas été étudiée avec l'intention d'allaiter dans cette région. Objectif : L'objectif de l'étude est d'explorer les facteurs psychosociaux, culturels et sociodémographiques associés à l'intention d'allaiter des femmes enceintes de la région de Chaudière-Appalaches. Cadres de référence : Ce travail s'appuie sur la théorie du comportement planifié (Ajzen, 1991) et la théorie de l'objectification sexuelle des femmes (Fredrickson & Roberts, 1997). Méthode : Le questionnaire réalisé selon ces cadres a été distribué dans quatre cliniques de Chaudière-Appalaches où s'effectuent les suivis de grossesse. Ce questionnaire auto-administré a permis de mesurer les données sociodémographiques et cliniques des participantes primipares à leur troisième trimestre de grossesse et les facteurs psychosociaux et culturels pouvant influencer leur intention d'allaiter. Des analyses descriptives, corrélationnelles et de régression ont été réalisées. Résultats : Dans l'échantillon (N = 47), la plupart des participantes (80,9 %) sont totalement en accord avec l'intention d'allaiter leur enfant à sa naissance et moins de la moitié d'entre elles (40,4 %) ont un objectif d'allaitement de six mois. Les analyses de corrélation démontrent que les croyances comportementales (rs = 0,52, p = 0,000), la perception du contrôle comportemental (rs = 0,45, p = 0,002) et la perception de facteurs facilitant l'allaitement (rs = 0,39, p = 0,007) sont associées à l'intention d'allaiter. En raison du peu de variabilité dans les données obtenues de la variable dépendante, l'analyse de régression opérée ne démontre aucune significativité entre les facteurs psychosociaux et l'intention d'allaiter. Discussion : Les résultats obtenus révèlent la pertinence d'intégrer les concepts de l'objectification sexuelle des femmes dans les construits psychosociaux de la théorie du comportement planifié pour comprendre l'intention d'allaiter en Chaudière-Appalaches et pour promouvoir l'allaitement par les infirmières de façon appropriée. -- Mot(s) clé(s) en français : Allaitement maternel, intention, comportement planifié, auto-objectification, sexualité, rapports de genre, homme. »--

Diverses actions de protection, de soutien et de promotion de l'allaitement ont depuis été réalisées aux niveaux régional et local en vue de favoriser l'adoption de pratiques favorables à l'allaitement maternel. En 2000, une révision de la planification s'est avérée nécessaire afin de mieux orienter les actions et l'évaluation du programme en lien avec des objectifs précis et réalistes. Le modèle théoretique Precede-Proceed a été retené et adapté pour guider le processus de révision de la planification du programme régional. Selon Green et Kreuter, un définition claire des comportements à changer et un diagnostic de ces derniers constituent une étape nécessaire pour la planification sociosanitaire (étape 3 du modèle). De plus, il faut agir sur les facteurs qui déterminent un comportement pour arriver à le changer (étape 4 du modèle). Cette étude présente les résultats des diagnostics prévus par les étapes 3 et 4 du modèle théoretique retenu. Il vise donc à connaitre la prévalence de l'allaitement maternel dans la région et à cerner les facteurs qui l'influencent. Ces diagnostics permettront de réviser les objectifs du programme à partir des données reflétant davantage la situation montérégienne.
