Book excerpt: En orthophonie, la rééducation de groupe est courante dans le cadre des TSA comme dans celui des troubles de l'oralité alimentaire. Si une grande majorité des enfants avec TSA souffre également de troubles de l'oralité, la rééducation de groupe ne leur est que rarement proposée dans ce cadre, et aucune étude n'existe à ce sujet. Ainsi, il semble pertinent de leur proposer ce type de prise en charge, et d'en étudier l'intérêt. Nous avons donc mis en place un groupe de rééducation orthophonique centrée sur les troubles de l'oralité alimentaire, auprès de trois enfants porteurs de TSA. âgés de 3 à 5 ans. A partir d'outils existants, nous avons créé deux grilles d'évaluation nous permettant de dresser leurs profils alimentaire et sensoriel initiaux. A partir des résultats de cette première évaluation, nous avons élaboré un projet de soins adapté aux objectifs de chacun et créé un protocole d'intervention en groupe d'une durée de 12 séances. A l'issue de ce protocole. nous avons mené une évaluation post-interventionnelle à l'aide des mêmes outils. La comparaison des profils obtenus, associée à nos observations cliniques, nous a permis de mesurer l'évolution des enfants pour étudier l'efficacité de la rééducation. Dans la limite de la fiabilité des tests proposés, les résultats obtenus permettent d'objectiver l'impact positif de la rééducation sur la sélectivité alimentaire et l'hypersensibilité tactile oro-faciale. Des améliorations concernant l'hypersensibilité olfactive, la néophobie alimentaire et les habiletés sociales et de communication ont également été remarquées. Ainsi, une prise en charge des troubles de l'oralité en groupe auprès d'enfants porteurs de TSA semble pertinente et efficace, bien que de nombreux paramètres soient à prendre en compte pour sa mise en oeuvre.

Book excerpt: D'après la littérature, 13 à 50 % des enfants tout-venant sont concernés par des difficultés d'alimentation, pourcentage pouvant osciller entre 56 et 87 % chez les enfants porteurs de Troubles du Spectre de l'Autisme (TSA). La fréquence de ces difficultés souligne l'importance d'une évaluation précoce des aspects pouvant impacter le moment du repas (Nadon, 2011). Toutefois, il n'existe actuellement pas d'outil évaluant les difficultés de l'oralité alimentaire à travers le recueil conjoint de données qualitatives et d'indications quantitatives se référant à une moyenne d'âge établie. Face à ces constatations, cette étude avait pour objectif l'élaboration d'un outil d'évaluation des difficultés de l'oralité alimentaire permettant de mettre en évidence les problèmes d'alimentation de trois cas cliniques d'enfants TSA. Pour cela, nous avons tout d'abord élaboré un protocole d'évaluation des difficultés de l'oralité alimentaire que nous avons proposé à cent enfants tout-venant âgés de trois ans à trois ans onze mois, afin d'établir une moyenne de référence. Nous avons ensuite testé le protocole d'évaluation auprès de trois cas cliniques d'enfants TSA se situant dans cette tranche d'âge et présentant des difficultés de l'oralité alimentaire. Nous avons complété l'évaluation des trois enfants TSA par la passation d'une grille d'observation d'un repas et la passation d'un menu de trois journées types complété par les parents. Les évaluations réalisées ont montré que les scores obtenus par chaque enfant TSA, présentant des difficultés de l'oralité alimentaire, sont significativement inférieurs à la moyenne établie auprès du groupe de référence. Leurs difficultés alimentaires sont également ressorties à travers une analyse qualitative de l'ensemble des données recueillies. L'outil d'évaluation a donc permis de mettre en évidence leurs troubles alimentaires. Afin de donner lieu à la validation du protocole d'évaluation, il serait intéressant de poursuivre cette étude en procédant à la vérification de ses qualités métrologiques

Book excerpt: Les particularités sensorielles et difficultés alimentaires sont désormais mentionnées dans le Manuel Diagnostique et Statistique des Troubles Mentaux (DSM-5) comme manifestations des Troubles du Spectre de l'Autisme (TSA). Les troubles alimentaires sont en effet extrêmement fréquents et bien documentés dans la population autiste pédiatrique, affectant 50 à 90 % des enfants avec TSA. Les données manquent en revanche pour les populations adulte et surtout adolescente. Les objectifs de cette étude sont d'établir une mesure de la prévalence des troubles alimentaires dans la population adolescente autiste de bon niveau cognitif et de caractériser ces troubles en termes de sélectivité, quantités et comportements alimentaires. Ce travail est réalisé au moyen de différents questionnaires ciblant chacun de ces aspects. Une comparaison avec des adolescents sans troubles développementaux majeurs est également menée. C'est une forte prévalence des troubles alimentaires, estimée à 21 à 87% des adolescents autistes de bon niveau cognitif, qui a ainsi été mise en évidence. Ces jeunes consomment une moins grande variété d'aliments que leurs pairs tout-venant et présentent davantage de problèmes de comportement au moment des repas. Les quantités consommées et les apports énergétiques ne différencient en revanche pas les deux groupes d'adolescents. Les troubles alimentaires décrits dans la population autiste pédiatrique persistent donc à l'adolescence et ce, même chez les sujets disposant de bonnes ressources cognitives. Les troubles, sensoriels et cognitifs notamment, sous-jacents à ces difficultés alimentaires mériteraient d'être investigués afin de mieux saisir l'étiologie de ces pro-blèmes alimentaires dans cette population. Cette étude témoigne néanmoins de la né-cessité pour les orthophonistes d'évaluer l'alimentation des sujets autistes, à tout âge et quels que soient leurs potentiels cognitifs, et ce, même si l'alimentation ne fait pas l'objet d'une demande spécifique. Sélectivité, quantités ingérées et comportement ali-mentaire devront être examinés.
