Malgré cette multiplicité discursive, l'exclusion sociale des sourds, longtemps reflétée par une offre de soins médicaux et psychiatriques inadaptée et centrée sur la déficience proprement dite, domine l'histoire, la surdité venant troubler les modalités habituelles du développement du langage et de la communication et provoquer ainsi une situation de handicap partagé. La prise en compte des troubles psychiatriques et des difficultés psychologiques concernant la période de l'adolescence des individus sourds s'inscrit dans l'essor récent d'une politique de soins globale et adaptée, passant notamment par la reconnaissance de l'importance de la Langue des Signes Française. Les adolescents sourds semblent plus souffrir de troubles psychiatriques que leurs homologues entendants. Sur le plan clinique, les troubles émotionnels et la problématique de l'agir sont au premier plan, mais l'établissement d'un diagnostic précis, en particulier d'une pathologie psychotique est délicat comme bien souvent à cette période de la vie. Aux difficultés propres liées à l'adolescence s'ajoutent ici pour le psychiatre l'intrication de nombreux facteurs de comorbidité, la fréquence des troubles du langage et des conditions inhabituelles de la pratique psychiatrique. La perturbation des interactions parents-enfant sourd, l'accès retardé au langage, et les situations de maltraitance sont des facteurs de risque fréquemment retrouvés chez les adolescents sourds qui doivent à cette période prendre conscience des limitations imposées par la surdité dans le champ social en premier lieu ce qui les confronte à un travail complexe de remaniements identitaires et des liens avec leur parents.
