La discussion porte principalement sur les barrières qui rendent très difficile l'intervention auprès de ces enfants et, plus particulièrement auprès des enfants qui vivent dans le nord de l'Ontario.

Différentes classifications du psycho-traumatisme chez l'enfant existent : regroupements symptomatiques en fonction de l'âge, comme conséquences de traumatismes isolés (type I) ou répétés (type II) voir d'une polyvictimisation ou prenant en compte la diversité des différents troubles successifs avec le développement de l'enfant (psycho-traumatisme complexe et psycho-traumatisme développemental). Ces classifications délimitent une symptomatologie plus vaste empruntant à différents troubles dont ceux issus d'altérations de l'attachement. Le DSM 5 a intégré des critères diagnostique pour les moins de 6 ans et la CIM 11 le diagnostic de psycho-traumatisme complexe en ne reprenant que partiellement la symptomatologie post-traumatique issue de la maltraitance. Après avoir décrit ces classifications, nous proposons une revue des traitements disponibles pour cette symptomatologie qui nécessite la prise en considération des atteintes de l'attachement.

Book excerpt: L'impulsivité et l'instabilité affective impactent lourdement la vie des patients souffrants de trouble de la personnalité limite (TPL). Certains outils de compréhension remontent par ailleurs à la petite enfance. L'objectif de ce travail est d'approfondir l'impact du trouble de l'attachement et des traumatismes infantiles dans le développement du TPL. Tout d'abord, une revue de la littérature affinera le sujet puis une étude de cohorte descriptive multicentrique permettra d'expérimenter les hypothèses dégagées. L'échantillon comprend 19 patients dont le diagnostic a été porté grâce au le questionnaire SCID II. Au cours d'un entretien, l'examinateur recueille les symptômes du DSM 5, les différents types d'attachement (questionnaires RQ et RSQ), et les éventuels traumatismes infantiles (questionnaire CTQ). On observe que 89% des patients souffrent de trouble de l'attachement dont 52,6% avec un attachement préoccupé et 31,6% avec un attachement craintif. Par ailleurs, 72,3% des patients rapportent une histoire traumatique, dont 63,2% de type émotionnel. Les patients préoccupés ont plus d'angoisses d'abandon que le reste du groupe alors que les patients craintifs manifestent plus de relations instables et de sentiments de vide. Ces derniers présentent des antécédents traumatiques plus graves et fréquents. Les patients avec un vécu traumatique manifestent plus de colères et de troubles psychotiques mais moins de trouble de l'identité. Ce résultat inattendu peut s'expliquer par l'influence des expériences psychotraumatiques sur le fonctionnement de l'individu, se manifestant par un évitement et un contrôle plus prononcé affectant ainsi, à terme la structure de sa personnalité.

Book excerpt: The increasing number of refugees, asylum seekers, and irregular migrants poses a challenge for mental health services in Europe. This review found that these groups are exposed to risk factors for mental disorders before, during, and after migration. The prevalence of psychotic, mood, and substance-use disorders in these groups varies but overall resembles that in the host populations. Refugees and asylum seekers, however, have higher rates of post-traumatic stress disorder. Poor socioeconomic conditions are associated with increased rates of depression five years after resettlement. Refugees, asylum seekers, and irregular migrants encounter barriers to accessing mental health care. Good practice for mental health care includes promoting social integration, developing outreach services, coordinating health care, providing information on entitlements and available services, and training professionals to work with these groups. These actions require resources and organizational flexibility.
