Book excerpt: Si les troubles d'allure autistique sont connus en association avec certaines carences (visuelles, auditives, affectives précoces et sévères), ils ne sont pas décrits dans la littérature en association avec les troubles de la relation mère-enfant. Dans ce mémoire, nous exposons le cas d'un enfant qui présentait, à l'âge de 2 ans et demi, une symptomatologie d'allure autistique, et dont l'anamnèse ne permettait de retrouver ni la notion d'une carence sensorielle, ni celle d'une carence affective grave. En revanche, des difficultés générationnelles entre l'enfant et sa mère étaient manifestes d'emblée. Cet enfant a été pris en charge durant quatre années à l'hôpital de jour de notre service (accueillant des enfants de 3 à 7 ans souffrant de trouble autistique ou de trouble envahissant du développement). Harry présentait un trouble de l'attachement désorganisé, type d'attachement qui prédispose à la psychopathologie. La possibilité d'une dépression infantile associée au trouble de l'attachement a été envisagée. De nombreux facteurs parentaux, maternels en particulier, ont pu également être relevés : dépression maternelle probable périnatale, mais également plus ancienne et chronique, avec la présence d'un traumatisme, le deuil non résolu de sa mère biologique ; violence maternelle avec forte suspicion de maltraitance physique ; contexte migratoire. Nous discutons ainsi la pertinence du trouble de l'attachement (en tant que trouble de la relation mère-enfant), comme modèle explicatif partiel des troubles d'allure autistique (distincts de l'autisme). Nous faisons l'hypothèse, dans le cas de Harry, que différents facteurs de risque en conjonction, et en particulier le trouble sévère de la relation mère-enfant qu'est l'attachement désorganisé, ont abouti à une symptomatologie autistique. Le type d'évolution favorable et le repérage précis des progrès de cet enfant à l'hôpital de jour ont étayé de façon convaincante cette hypothèse. Nous insistons sur l'importance du travail qui peut être fait lors de troubles de la relation antre une mère et son enfant. Nous soulignons également la nécessité du dépistage précoce, dans les maternités, de la dépression périnatale, la nécessité de la prévention de la carence affective et des troubles de l'attachement en résultant chez le bébé, tout particulièrement dans les populations à risque, comme c'est le cas dans l'immigration

A partir des données de la littérature, de l'observation clinique qualitative de deux enfants et en basant notre travail sur la grille de Coventry, nous avons réfléchi aux spécificités cliniques de ces deux troubles. Notre travail met en avant des différences qualitatives marquées dans le regard, le babil, la sensorialité et des différences dans l'intention liée aux comportements de répétition. Ces éléments pourraient permettre l'élaboration d'un outil d'aide au diagnostic qui serait utilisé comme étayage pour l'observation fine des comportements du jeune enfant. Il permettrait ainsi d'améliorer la précision du diagnostic précoce de ces troubles et de mieux définir le soin et les interventions dont ils relèvent respectivement.

La partie centrale de ce CAHIER ne dérogera pas à l'habitude et sera consacrée à des observations cliniques signées par les équipes qui en ont assuré le suivi. Nous espérons que ces témoignages et leurs commentaires permettront d'argumenter la pertinence et l'impact thérapeutique du patient travail accompli par ces équipes en matière de prévention des troubles relationnels précoces, en PMI, dans l'accompagnement des bébés nés sous X, en pouponnière, en crèche, mettant en lumière les rapports entre la pédiatrie hospitalière et les prises en charge dans les placements familiaux thérapeutiques
