Book excerpt: Excerpt from Transactions of the Society of Tropical Medicine and Hygiene, Vol. 2: March, 1909 I AM not here this evening with any idea of delivering a lecture on spirochetes, for I quite appreciate the fact that there are many present who know very much more about the subject than I am ever likely to do. My reason for putting these specimens of a spirocheta before you is that during my work in India I came across it while examining for tubercle bacilli, and following up the case I was led to believe that it was probably the cause of a distinct type of fever associated with bronchitis. The story of the find is this: In January, 1907, a European guard of the East Indian Railway was sent to me from Delhi as a case of malarial cachexia requiring hospital treatment. He was admitted into the East Indian Railway Hospital at Tundla, and was seen by me the following morning. He was anaemic, very weak, with no definite enlarge ment of the spleen, and had a temperature of 104 F. His blood was examined for malarial parasites with a negative result. On the second or third day I found his temperature on a four-hour chart was continuous, keeping between and 104 F., and that he was troubled with a very irritable cough, bringing up a lot of thin watery expectoration. While in bed the leg becomes comparatively flabby, being less tense than when he is getting about. The patient states that the swelling began in his foot, and that he had to cut his boot. For three years after 1895 the swelling of the leg remained comparatively slight. He can walk about with difficulty. As our knowledge of the life-cycles of these organisms is exceedingly limited, I think it somewhat premature to say they belong to an entirely new genus, and I hope to show you that they do belong to a group which has long been recognized as occurring in the alimentary tracts of insects. From a study of the flagellates of the parasite of kaia-azar in the test tube, as well as in the bed-bug Cimex rotundatus, I have no hesitation in saying it is not a trypanosome; for, like all other observers who have studied this stage, I have never seen anything similar to the Well-known vertebrate trypanosome.
