This cross-national study provides empirical evidence of some of the trends in health status, health services, and health care financing that have occurred in countries undergoing economic transition in Central and Eastern Europe. Moreover, in those that have increased health care spending, the resources have either not been properly mobilized or have not led to improved quality in care. This interest has been manifested either by developing national procedures to assess students' achievements or by participating in international studies of student achievement. This book provides introductory information on ways of assessing the learning outcomes of education systems. It considers the nature, choice, and use of indicators and describes numerous approaches to assessing learning outcomes in selected industrial countries (the United States and the United Kingdom) and developing countries (Chile, Colombia, Mauritius, Namibia, and Thailand). The authors also review systems of comparative international assessment and debate whether developing countries should participate in them. The book contains boxes and tables; an appendix features a national assessment checklist.

Morbidity and mortality from chronic and infectious diseases have increased. Women's reproductive health has been compromised by an increase in the incidence of sexually transmitted diseases and by lack of access to up-to-date, high quality maternity and family planning services. This paper summarizes this current situation and reports on the findings of one element of the World Banks assistance to improve health care in the Russian Federation: a field testing of revised protocols for the care of women and infants.

However, they still suffer from some of the worst health conditions in the world. This study sets out to make available national-level information on health expenditures, health service outputs, and health outcomes in a way that could assist health planning and policy development in Africa. It outlines broad patterns of health spending, service delivery, mortality, fertility and malnutrition in Africa in the early to mid 1990s. By also exploring gaps in information available and potential uses of health information, the paper intends to stimulate discussion on how better to monitor progress and use information for better health outcomes within and among different African countries. The data covered in the study include major macroeconomic indicators, such as real GDP, rate of GDP growth, inflation rate, and per capita official development assistance. Key social indicators are presented, including the level of education, and access to safe water and sanitation. The detailed data contained in the annex tables from which the analytic results are derived invite readers to make additional analyses of their own.

Book excerpt: The demand-driven rural investment fund (DRIF) is a new mechanism for decentralizing decisionmaking authority and financial resources to local governments and communities to use for investments of their choice. To counteract the local government's weak capacity to choose and implement projects well, central governments have often constrained the choices of communities by limiting the types of projects eligible for financing and requiring specific procedures for procurement and disbursement. This study explores the extent to which well-designed DRIF rules and incentive structures can substitute for central control. It looks at the different and often conflicting motivations of donors, central governments, and communities and explores how rules can be devised to allow actors to achieve their objectives.

Book excerpt: China is in the throes of two transitions: from a command economy to a market-based one and from a rural, agricultural society to an urban, industrial one. So far, both transitions have been spectacularly successful. China is the fastest-growing economy in the world, with per capita incomes more than quadrupling since 1978, achieving in two generations what took other countries centuries. Although swift growth and structural change have resolved many problems, they also have created new challenges: employment insecurity, growing inequality, stubborn poverty, mounting environmental pressures, rising costs of food self-sufficiency, and periods of macroeconomic instability stemming from incomplete reforms. Unmet, these challenges could undermine the sustainability of growth, and China's promise could fade. China 2020, a seven-volume set, examines China's recent history, where it is today, and the path it should follow during the first two decades of the 21st century. The volume in the set entitled, At Chinas Table: Food Security Options focuses on how China will avoid national chronic food insecurity. The report evaluates solutions such as food storage and other alternatives for addressing the problems of transitory food insecurity from drought or other seasonal calamity. It discusses national food security constraints and the investments required to maintain total factor productivity of 1.0 percent per year. The study also models and projects food supply and demand for 2020.
