Book excerpt: The Human Immunodeficiency Virus (HIV)/Acquired Immune Deficiency Syndrome (AIDS) pandemic burdens Sub-Saharan Africa (SSA) and continues to constrain its social and economic advancement. Joint United Nations Program on HIV/AIDS (UNAIDS) has estimated that in southern Africa alone, 930,000 adults and children died of AIDS in 2005. This represents about one-third of AIDS deaths recorded globally that year. In addition, about 12 million children below the age of 17 in SSA are estimated to have lost one or both parents to AIDS. Africa Technical Transport Sector Unit (AFTTR) has made progress in mainstreaming HIV/AIDS in its portfolio. However, there is still more work ahead in ensuring that all projects are mainstreamed as needed. In this context, the transport sector board needs to continue supporting such future mainstreaming efforts by establishing a sector board strategy for HIV/AIDS activities on Bank-financed transport projects. The diverse nature of transportation activities implies that mainstreaming is both challenging and urgent. In 2000, the Africa transport team gave high priority to its contribution to the campaign against the HIV/AIDS pandemic and pledged to mainstream HIV/AIDS actions in the Bank's lending operations and at country level in the transport sector. The transport sector contributed significantly through integrating simple activities into its operations (such as HIV/AIDS contract clauses into bidding documents for road construction site workers). Similarly, the Bank financed a first-round workshop to prepare HIV/AIDS prevention policy in the workplace for Ministry employees. Its main objective is to develop and implement highly focused prevention interventions to reduce HIV/AIDS prevalence and slow down the spread of the disease in the transport sector. This document is subdivided in four sections. The first section gives background information on the transport sector and HIV/AIDS. The second section describes the Bank's transport sector activities, with particular focus on the Africa region and its achievements regarding HIV/AIDS. The third section presents the process and the results of the assessment of the Africa transport sector portfolio for HIV/AIDS mainstreaming. The fourth section enumerates the lessons learned as well as the recommendations made to the World Bank's Transport Sector Board and to stakeholders in the client countries.

Book excerpt: The transport sector is especially vulnerable to HIV/AIDS. Transport workers including long distance truck drivers, seafarers, airline crews and infrastructure construction workers spend long periods of time away from home, often endure harsh working conditions, and may engage in unsafe behavior that can lead to infection. Their mobility makes it difficult to access health information and treatment, or to maintain drug regimen. Transport hubs and construction sites are often considered hot spots due to the influx and interaction that take place among the mobile workers. Furthermore, the sector works as a vector for HIV spread as the opening of new roads connects low and high prevalence areas. The World Bank transport group has been proactively mainstreaming HIV response by assisting client governments to design and implement sector-level interventions. Programs are most developed in the highest prevalence areas, initially in Sub-Sahara Africa and now also in Asia. Support to the sector includes organizing training events to increase staff awareness and knowledge, providing access to research materials, and securing funds to develop or scale up intervention programs. To institutionalize the response, explicit provisions for HIV prevention have been embedded in the standard bidding document. The group also collaborates with local workers organizations with the help of International Transport Workers Federation and the International Labor Organization. Some lessons learned are: (i) strategies must be crafted to meet the specific needs of the country in accordance with the local culture and unique epidemic situations; (ii) coordination with the health sector and the national AIDS authority is necessary for effective program design; and (iii) involving stakeholders from local communities is key to program success.

Book excerpt: Findings of international studies of the HIV/AIDS pandemic at work places suggest that, the transport sector is a major vector for the disease. The reason is simple. People working in the transport sector are mobile, they spend weeks and months away from their families and their homes and many satisfy their sexual needs "on the road." Migration, short-term or long-term, increases opportunities for sexual relationships with multiple partners, transforming transport routes to critical links in the propagation of HIV/AIDS. International studies also suggest that long-haul truck drivers are the highest risk group in the road sector. Clearly, social capital is at risk. In Africa, studies assessing the relationship between transport and HIV/AIDS are still partial and embryonic. Yet, situational analyses undertaken to date suggest that HIV/AIDS has become a major threat to the social capital of the transport sector and to transport operations, but few actions are taken to address the insurgence of the pandemic. Nonetheless, investing adequately in combating HIV/AIDS in Africa is now a precondition for all other development investments to succeed. The transport sector faces four major challenges: 1) Reduction of social capital 2) Poor safeguard policies addressing HIV/AIDS at work places 3) Absence of standard HIV/AIDS clauses in works contracts 4) Limited sector analytical work on HIV/AIDS. These challenges can be addressed. Committed leadership, continuous dialogue with clients, and strategic partnerships could make a difference.

The study was based on two quantitative surveys conducted with truckers, sailors and sex workers in Poti, Batumi and Gori in Georgia, and on qualitative surveys conducted with health and transport sector institutions and NGOs.
