Book excerpt: A nutrition-sensitive food system is one that goes beyond staple grain productivity and places emphasis on the consumption of micronutrient-rich nonstaples through a variety of market and nonmarket interventions. A nutrition-sensitive approach not only considers policies related to macrolevel availability and access to nutritious food, but it also focuses on household- and individual-level determinants of improved nutrition. In addition to agriculture, intrahousehold equity, behavior change, food safety, and access to clean water and sanitation are integral components of the food system. This article provides a detailed review, from an economic perspective, on the multisectoral pathways through which agriculture influences nutrition. A critical challenge is to identify and implement food and nutrition policies that are appropriate to the particular stage of structural transformation in the country of concern.

Investment in nutrition can yield high returns in terms of reduced health costs, increased productivity and improved human resources capacity and economic growth (Covic & and Hendriks 2016; Shekar et al. 2017). Nutrition policy-making and program interventions in developing countries fail to bring together several sectors that contribute to nutrition improvement. Since food systems influence the type of food produced, understanding relevant drivers of a country’s food system with an emphasis on nutrition can help to end malnutrition (Per Pinstrup-Andersen 2012a; HLPE 2017; Babu and Kataki 2003). In this paper, we adopt a food systems perspective to review Myanmar’s current food system. With the help of a review of the literature and two national consultative stakeholder workshops, we examine Myanmar’s current food system. This is a crucial step since it identifies gaps existing in the current policies/ strategies being implemented. After the review, we developed an AIT (analyze gaps, identify priority investment areas, and track progress) operational framework that can be used to increase the nutrition-sensitivity of a food system. Applying this framework to Agriculture Development Strategy (ADS), this paper presents an analysis of the gaps that need to be addressed to make ADS nutrition-sensitive, provide priority investment areas, and a tracking system which monitors the progress of these investments.

Book excerpt: A growing number of governments, donor agencies, and development organizations are committed to supporting nutrition-sensitive agriculture (NSA) to achieve their development goals. Although consensus exists on pathways through which agriculture may influence nutrition-related outcomes, empirical evidence on agriculture’s contribution to nutrition and how it can be enhanced is still weak. This paper reviews recent empirical evidence (since 2014), including findings from impact evaluations of a variety of NSA programs using experimental designs as well as observational studies that document linkages between agriculture, women’s empowerment, and nutrition. It summarizes existing knowledge regarding not only impacts but also pathways, mechanisms, and contextual factors that affect where and how agriculture may improve nutrition outcomes. The paper concludes with reflections on implications for agricultural programs, policies, and investments, and highlights future research priorities.

Book excerpt: The triple burden of malnutrition is growing in low- and middle-income countries (LMICs). Increasing access to affordable ultra-processed foods in the food environment is contributing to this problem. While existing explanations for this triple burden of malnutrition have examined demand-side factors of food choices, the supply-side policies relating to the food environment drivers, ideas and actors’ interests have been neglected. Using a case study of Ghana, this analysis combines the Advocacy Coalition Framework with the narrative policy analysis to unpack the supply-side food environment policies and actors driving the triple burden of malnutrition. Applying a mixed methods analysis of the transcripts, the narratives reveal public, private and civil society organization (CSO) coalitions with different ideas and interests in the food environment. In the private sector coalition, food companies engage in aggressive advertising and are driven by profit motives – leading to the supply of more ultra-processed foods. The public sector is failing to regulate the market because of inadequate policies, limited institutional capacities and coordination, enforcement challenges, inadequate resources, and self-interest. Social activism by CSOs, for example, pressuring food companies to deliver healthy foods and holding the government accountable, is also lacking. The result is a triple sector (public, private and CSO) failure in the urban food environment with consequences on the availability of ultra-processed foods. This has long-lasting implications for the reduction of the triple burden of malnutrition and the achievement of zero hunger. To accelerate nutrition-sensitive food environments that deliver healthier food options, we argue that it is critical to entertain the ideas and interests of stakeholders and implement food environment policies that cover private and public sector initiatives, as well as increase consumer awareness.

Book excerpt: The food systems concept has attracted a considerable amount of attention as it provides an opportunity to better understand and represent the array of factors that explain food security in a comprehensive and holistic manner. The value-added proposition of food systems resilience is that the ability to respond to shocks and stressors may be incorporated into such explanations. The qualities that make food system resilience attractive, however, also make it difficult to model in empirical terms. This paper, by drawing on the literatures of food systems and on the measurement of resilience, demonstrates how food systems resilience can be measured at a country level. Clustering countries into regions shows that North America and Oceania have the highest levels of food systems resilience, followed by Europe and North Africa and Western Asia. Food systems resilience is lower in Latin America and the Caribbean and South Asia and sub-Saharan countries exhibited the lowest levels of food systems resilience. In low- and middle-income countries, increasing market resilience plays a significant role in increasing overall food systems resilience.

Book excerpt: Food systems are being transformed at an unprecedented rate as a result of global economic and social change. Urbanisation, foreign direct investment in markets of developing countries and increasing incomes are prime facilitators for the observed changes, while social changes, such as the increased number of women in the workforce and rural to urban migration, provide added stimulus. Changes are also facilitated in concrete ways by food production based on intensive agriculture, new food processing and storage technologies, longer product shelf-life, the emergence of food retailers such as fast food outlets and supermarkets and the intensification of advertising and marketing of certain products. The sum of these changes has resulted in diverse foods that are available all year for those who can afford them, as well as a shift in home-prepared and home-based meals to pre-prepared or ready-to-eat meals, often consumed away from home. These food system and lifestyle changes are in turn having an impact on the health and nutritional status of people in developing countries. There is an indication of rapid increases in overweight and obesity, particularly among adults, and an increasing prevalence of diet-related non-communicable diseases. At the same time, social inequalities are increasing, particularly in urban areas. The papers appearing in this publication were first presented at the workshop Globalisation of food systems: Impacts on food security and nutrition held at FAO headquarters in Rome from 8 to 10 October 2003. The chapters are arranged in two parts. The first contains overview chapters providing a synthesis of findings from 11 country case studies, an overview of issues related to urban food insecurity, a review of nutritional change in developing countries and some policy options to address these changes. The second section gives a detailed account of the changes in food systems and health and nutrition problems in 11 case study countries, representative of different regions throughout the world.

Book excerpt: Updates for many countries have made it possible to estimate hunger in the world with greater accuracy this year. In particular, newly accessible data enabled the revision of the entire series of undernourishment estimates for China back to 2000, resulting in a substantial downward shift of the series of the number of undernourished in the world. Nevertheless, the revision confirms the trend reported in past editions: the number of people affected by hunger globally has been slowly on the rise since 2014. The report also shows that the burden of malnutrition in all its forms continues to be a challenge. There has been some progress for child stunting, low birthweight and exclusive breastfeeding, but at a pace that is still too slow. Childhood overweight is not improving and adult obesity is on the rise in all regions. The report complements the usual assessment of food security and nutrition with projections of what the world may look like in 2030, if trends of the last decade continue. Projections show that the world is not on track to achieve Zero Hunger by 2030 and, despite some progress, most indicators are also not on track to meet global nutrition targets. The food security and nutritional status of the most vulnerable population groups is likely to deteriorate further due to the health and socio economic impacts of the COVID-19 pandemic. The report puts a spotlight on diet quality as a critical link between food security and nutrition. Meeting SDG 2 targets will only be possible if people have enough food to eat and if what they are eating is nutritious and affordable. The report also introduces new analysis of the cost and affordability of healthy diets around the world, by region and in different development contexts. It presents valuations of the health and climate-change costs associated with current food consumption patterns, as well as the potential cost savings if food consumption patterns were to shift towards healthy diets that include sustainability considerations. The report then concludes with a discussion of the policies and strategies to transform food systems to ensure affordable healthy diets, as part of the required efforts to end both hunger and all forms of malnutrition.
