Book excerpt: Over the course of the last two decades, improved practices in child and adolescent mental healthcare have led to a decreased environment of stigma, which also led to an increased identification and treatment of mental health disorders in children and youth. Considering that treatment and outcomes are improved with early intervention, this is good news. However, the success gained in the field of child and adolescent psychiatry leads to a new challenge: transitioning from adolescent care to adult care. It has been known for some time that children, adult, and geriatric patients all have unique needs where it comes to mental healthcare, yet limited work has been done where it comes to the shifting of the lifespan. Where it comes to the child-adult transition—defined as those in their late teens and early/mid-20s—there can be multiple barriers in seeking mental healthcare that stem from age-appropriate developmental approaches as well as include systems of care needs. Apart from increasing childhood intervention, the problem is exacerbated by the changing social dynamics: more youths are attending college rather than diving straight into the workforce, but for various reasons these youths can be more dependent on their parents more than previous generations. Technology has improved the daily lives of many, but it has also created a new layer of complications in the mental health world. The quality and amount of access to care between those with a certain level of privilege and those who do not have this privilege is sharp, creating more complicating factors for people in this age range. Such societal change has unfolded so rapidly that training programs have not had an opportunity to catch up, which has created a crisis for care. Efforts to modernize the approach to this unique age group are still young, and so no resource exists for any clinicians at any phase in their career. This book aims to serve as the first concise guide to fill this gap in the literature. The book will be edited by two leading figures in transition age youth, both of whom are at institutions that have been at the forefront of this clinical work and research. This proposed mid-sized guide is therefore intended to be a collaborative effort, written primarily by child and adolescent psychiatrists, and also with adult psychiatrists. The aim is to discuss the developmental presentation of many common mental health diagnoses and topics in chapters, with each chapter containing clinically-relevant “bullet points” and/or salient features that receiving providers, who are generally, adult-trained, should keep in mind when continuing mental health treatment from the child and adolescent system. Chapters will cover a wide range of challenges that are unique to transition-age youths, including their unique developmental needs, anxiety, mood, and personality disorders at the interface of this development, trauma and adjustment disorders, special populations, and a wide range of other topics. Each chapter will begin with a clinical pearl about each topic before delving into the specifics.

Book excerpt: Healthcare transitions among vulnerable populations, like adolescents with autism spectrum disorder, are complex and necessary for the launch into adulthood. Many patients are not receiving planned care. A policy, timeline, and resource guide did not exist in the clinical site. The lack of resources and policies available to this center and the complexity of medical visits in this population have led to inconsistent transition planning for adolescents with autism. The evidence reviewed supported an evidence-based transition program, such as a toolkit, to support and facilitate the process of healthcare transitions among adolescents with autism spectrum. A quality improvement project was designed to improve the transition process with this population and improve healthcare provider adherence to an adolescent transition plan. The intervention was a toolkit, which included a policy and resource guide. The intervention was then implemented into the clinical site. Post-intervention chart audits were used to examine the rate of transition documentation among healthcare providers after the toolkit is implemented. A total of 12 transition conversations occurred and were successfully documented by HCPs in the clinical site while a total of 13 were "missed" opportunities for transition planning. When variables were assessed by completion status, there was a statistically significant difference between transition plan documentation completion in the age 12-15 year old group (5/17, 29%) versus the age 16 years and older group (7/8, 88%). There was also a statistically significant difference by transition plan documentation completion by provider type with NPs completing 60% visits and MDs completing 25% of the time. Transition planning improvement for adolescents on the autism spectrum remains a national and local priority.

Book excerpt: Due to early intervention efforts and improved diagnoses, more individuals with high functioning Autism Spectrum Disorder (ASD) are able to attend postsecondary education than ever before (Adreon & Durocher, 2007). However, due to the inherent nature of autism, the transition to postsecondary education can be of great difficulty and cause a great deal of anxiety for these individuals. The purpose of the current research was to explore what is being done to aid in the transition to postsecondary education for this specific population and what is helpful. All participants were male and either had or currently are receiving special education services. Participants consisted of 3 high school students, 1 Blue Ridge Community College student, and 3 James Madison University students. Participants completed a demographic questionnaire and participated in an individual interview with the researcher. Overall, it was found that the majority of participants were not involved in the construction of their transition services plans and none of the participants knew the role of a school psychologist. All of the university participants named one person who took charge of their transition to college and are experiencing academic success as a result. Further results are discussed as well as implications for future research.
