Book excerpt: The International Society on Oxygen Transport to Tissue (ISOTT) was founded in 1973 "to facilitate the exchange of scientific information among those interested in any aspect of the transport and/or utilization of oxygen in tissues". Its members span virtually all disciplines, ex tending from various branches of clinical medicine such as anesthesiology, ophthalmology and surgery through the basic medical sciences of physiology and biochemistry to the physical sciences and engineering. The fifteenth annual meeting of ISOTT was held in 1987 for three days, from July 22 to 24, at Hokkaido University in Sapporo, Japan. Previously, all ISOTT meetings had been held in Europe or the USA alternatively. This time, however, the meeting was held for the first time in an Asian country. When we first started preparing for this meeting some of our members were afraid that the number of those attending would not exceed '30. Fortunately the results were quite different. We had more than 60 participants from abroad and an even greater number from Japan. In addition to three special lectures and two symposia there were a total of 88 posters presented over the three days of the meeting. These covered all aspects of physiological oxygen transport including convection, diffusion, chemical reaction, and control of oxygen demand in blood and various tissues as well as the methods, models and instrumentation for their study. The 92 papers which comprise this volume encompass all of these areas.

It began as a minor technical tool used in a few branches of medicine, notably endocrinology and nephrology. However, throughout the world it has now become established as a clinical discipline in its own right, with specific training programmes, special skills and a particular approach to patient management. Although the practising nuclear medicine physician must necessarily learn a great deal of basic science and technology, a sound medical training and a clinical approach to the subject remains of fundamental importance. It is for this reason that we have attempted in this book to approach the subject from a clinical standpoint, including where necessary relevant physiological material. There exist many excellent texts which cover the basic science and technology of nuclear medicine. We have, therefore, severely limited our coverage of these aspects of the subject to matters which we felt tobe essential, particularly those which have been less well covered in other texts- for example, the contents of Chapter 20 on Measurement by Royal and McNeill. Similarly, we have limited details of methodology to skeletal summaries of protocol (Appendix 1) and have included at the end of some chapters descriptions of particular techniques where we and the authors felt that it would be helpful.

Much of the impetus for research leading to new knowledge has derived from the perception that many respiratory disorders in the newborn infant are due to defective development or maladaption of some component or components of the respiratory system. Thus, to cite one example, surfac tant deficiency is clearly seen to be the cause of atelectasis in hyaline mem brane disease; and to cite another, it is widely accepted that the mechanisms controlling patency of the ductus arteriosus and pulmonary vascular resistance also determine the right-to-Ieft or left-to-right shunting frequently observed in the course of neonatal respiratory disorders. There are, however, areas of physiological knowledge - such as those relating to respiratory control and to liquid formation and absorption - which are clearly of great relevance to lung adaptation at birth but where it has not yet proved possible to link a specific clinical state to the malfunction of a particular mechanism. In planning this symposium an attempt was made to organize the material in an orderly manner, starting with the embryonic and fetal stages of growth and development, continuing with respiratory control and the role of surfactant in lung aeration at birth, and ending with the treatment of neonatal respiratory disorders.

The goal is to help students, physiologists and clinicians understand the control of pressure, vascular volume, and blood flow by examining the cardiovascular system during orthostasis and exercise, two stresses that most affect these variables. A discussion of the passive physical properties of the vascular system provides a basis for explaining how vascular control is modified by mechanical, neural, and humoral factors. Interactive effects of the vasculature on cardiac performance are emphasized; they reveal the importance of autonomic control, supplemented by muscle pumping, in maintaining adequate ventricular filling pressure. The author's detailed analysis of how total oxygen consumption is restricted focuses on limitations in cardiac pumping ability, oxygen diffusion from lungs to blood and from blood to active muscle, oxidative metabolism and neural control of organ blood flow. An unsolved mystery is the nature of the signals that govern the cardiovascular responses to exercise. This is discussed in a new and critical synthesis of ideas and evidence concerning the "error signals" that are sensed and then corrected by activation of the autonomic nervous system during exercise.
