The book is divided into few major sections covering all four valves and other topics. Each section contains several chapters discussing everything related to that valve. Beginning with the pulmonary valve, since it was the first valve to be tackled in the catheterization laboratory, and then moving to the aortic, then the mitral and then finally end with the tricuspid valve. 1.5M US citizens alone have some degree of aortic valve stenosis, with half (750K) requiring aortic valve replacement. Aortic valve replacement, on the whole, is performed by surgeons, requiring bypass machines and technicians, as well as the usual operating team. The operation is expensive and occupies a considerable amount of operating room time. Mostly, the aortic valve is calcified and the usual option available to patients is valve replacement with a variety of choices, ranging from porcine valves to synthetic, for which there are many manufacturers. It should be noted that the aortic valve is the most problematic of valves. Percutaneous procedures are the answer. The bottom line is that given the growing elderly population, many more patients will require valve repair, thus increasing health care costs with not only surgical operations but also hospitalisation. Percutanous valve repair, whilst requiring a cath lab team, does not involve bypass machines nor extended hospitalisation. Like percutaneous transluminal coronary artery interventions (PTCA) has replaced coronary artery bypass grafts (once the golden standard), and now stenting having replaced PTCA and its balloons. We now see drug eluting stents replacing ordinary stents (though at a much higher cost. There will be a huge movement toward percutanous valve repair, which should presumably cut costs but also morbidity and mortality.

Book excerpt: Severe tricuspid regurgitation (TR) is a common valvular heart disease with an incidence of approximately 4% in the adult general population. Most commonly, it results from secondary causes including left ventricular dysfunction, mitral regurgitation and pulmonary arterial hypertension. Isolated severe TR has been identified as an independent predictor of death and cardiac adverse events. Furthermore, TR has been observed to develop late after left-sided valve surgery in up to 10% of patients with adverse impact on clinical outcomes. Perioperative mortality after surgical correction of TR has been reported to range between 2 and 10%. Moreover, a relevant proportion of patients with TR have already undergone open-heart surgical procedures and reoperation has been associated with an excessive risk of mortality. As a result, less invasive treatment options using transcatheter techniques may address an unmet need among high risk and inoperable patients. Different concepts for the percutaneous treatment of symptomatic severe TR have been developed including annular reduction, restoration of coaptation, and edge-to-edge repair. Worldwide, an increasing number of patients is treated using these emerging techniques and early clinical results begin to be available.

Book excerpt: "Valvular heart disease is increasingly prevalent in the aging population. Mitral regurgitation (MR) and tricuspid regurgitation (TR) both affect up to 10% of individuals over 75 years and are associated with increased morbidity and mortality rates. Transcatheter edge-to-edge mitral valve repair (TEER) is indicated in patients with symptomatic MR deemed high-risk for mitral valve surgery. The procedure is being performed more frequently, highlighting the critical importance of optimizing its planning, execution, and effectiveness. Concomitant TR is often present in those with significant MR (mainly due to pulmonary hypertension and right ventricular pressure or volume overload) and is known to affect outcome after TEER. Isolated TR is mostly functional in its etiology as a result of annular dilatation and right ventricular remodeling. Most patients with significant TR are deemed high risk for surgery due to the presence of right ventricular dysfunction, advanced age, and other comorbidities. Until recently, these patients could only be treated with medical therapy. Transcatheter edge-to-edge tricuspid valve repair is now available as well. In this thesis, we investigated (1) clinical decision making for TEER, (2) potential tools to optimize TEER, (3) the role of concomitant TR in MR patients, and (4) the early experience of transcatheter edge-to-edge TV repair."--
