Among these may be mentioned the short lapse of time between injury and surgical assistance; the temperature of the day, which was high, prevented chill ing of the exposed abdominal viscera, thereby diminishing the tendency to shock; and the thorough douching with hot sterile water during the operation and hourly flushing of the abdominal cavity for two days afterwards. Then, too, by the use of Murphy's button much time was saved in making the anastomosis, thus obviating undue manipulation of the bowel, which always tends to produce for a time a certain amount of paresis of its coats which complicates the after treatment of the case. Excerpt: ... the patient was readmitted to the Hospital for examination. There was no sign of an increase in the size of the tumor. There had been no return of pain or lameness. A skiagraph taken at this time shows the tumor to be about the same as at the time of his discharge in November. At the present time the patient goes to school, he rides a bicycle, plays base-ball and has no difiiculty in running and playing. There has been no return of pain. He now weighs 103 pounds; while at the time of his discharge he weighed 76% pounds. A skiagraph taken May 1, 19! 5, shows that there has been no change in the size of the tumor since his discharge in November, 1914. DR. A. P. C. ASHHURST called attention to the marked contrast between the cases reported by Dr. Gill and by Dr. Denit, the former a typically malignant sarcoma, and the latter a so-called giant-cell sarcoma. In both the tumor was curetted, and the patients were treated by Coley's fluid. In both patients, likewise, the tumors recurred after operation in spite of this treatment. But in the case of the woman (Dr. Gill's case) death followed the operation for the second recurrence, only 7 months after the first operation; while in the boy (Dr. Denit's case) the recurrent tumor gradually melted away under the influence of the toxins, seemed to become more and more bony, and the patient is at present apparently in perfect health, more than 7 months after cessation of treatment, and 17 months after the first and only operation. As to the real nature of giant-cell sarcomas, many papers have been written by surgeons setting forth their views; but the pathologists really are wiser, for like Socrates they admit that they know nothing. Excerpt from book: Case II.?A domestic, aged 32, was admitted to the Pennsylvania Hospital, August 12, 1905. This patient was sent into the hospital at night as a case of appendicitis and was seen in my absence, by Dr. Francis T. Stewart. Dr. Stewart did not think the case one of appendicitis or one demanding immediate operation. I saw her the next day and could not make up my mind that there was any inflammation of the appendix. She gave a history of repeated attacks of pain in the right side of the abdomen in the appendiceal region, accompanied by vomiting. The pain was more or less fixed, there being no radiation to the back or to the bladder: there were no urinary symptoms. The examination of the urine, however, on the day after admission showed a few red-blood corpuscles and a trace of albumin. Repeated examinations of the urine continued to show red-blood corpuscles. Examination of the abdomen the day after admission showed considerable tenderness over the right kidney. This, however, disappeared and the only tenderness was in the iliac region. A few days later the patient passed both blood and mucus by the bowel and the caecum and sigmoid were quite tender. These symptoms did not persist, as did the microscopic blood in the urine. A differential diagnosis between appendicitis, colitis, renal calculus and ureteral calculus had to be made. With the disappearance of the renal tenderness and of the blood and mucus in the bowel movements, and in view of the absence of the characteristic symptoms of appendicitis, I made a diagnosis of a probable ureteral calculus on August 3Oth, and advised operation. This diagnosis was based on the continued tenderness in the iliac region, the repeated attacks of pain accompanied by slight rise of temperature, and especially the microscopic blood in the urine.
