It is part of a larger research agenda to develop policy aiding decision tools and address inefficiencies of medical markets. The Transaction Cost Economics framework is applied for a project on re-engineering of California primary care; then an emerging theoretical framework called the "3Ps" develops further a Transaction Cost Political theory with a business approach; it combines Politics, Patients and Products from companies for market access, with 3P from the demand side: Physicians, Pharmacists and Patients. These actors choose individual cost reduction strategies for patients, this explicit information complements the implicit cost cues used in the cost sensitivity simulators (Huttin-Endepusresearch, 2017). Then, the author uses this "3P" framework in two case studies: France and Germany. Results identified that physicians were key players in Germany versus pharmacists in France, with their choices of individual cost reduction strategies. The Transaction Cost approach remains useful for Random Utility Models in health insurance.

The study is the result of the research on the transaction costs of the pricing of health services. The primary objective of the study is to identify the transaction costs occurring during the pricing of health services and characterize the factors that affect the level of transaction costs. An additional objective of this study is to link the conclusions about the level of transaction costs to the conclusions regarding the accuracy of the cost-based pricing in health care. The empirical part of the work was carried out based on the analysis of primary and secondary sources. Its aim was to present how pricing is organized in seventeen countries selected.

It rather focuses on <I>microeconomics and how health systems (private entities) organize themselves - cooperate, integrate, and disintegrate - in a specific market, the San Francisco Bay Area, U.S.A. Based on transaction cost economics alternative modes of governance in the health care market are examined. For this purpose a long-term case study has been conducted in the Bay Area in order to evaluate <I>how health care transactions should be organized to economize on transaction costs, which experiences have been made over the last years and which strategies are recommendable for future reorganization in the U.S. health care market and other countries. In addition, implications for the German health care market are discussed as well. A conclusion finally summarizes the findings and outlines further areas of research.

Book excerpt: President Clinton's health care reform proposals of 1993 represented the most far-reaching program of social engineering attempted in the United States since the passage of Medicare and Medicaid in 1965. Under the guise of reforming the health care system, the Clinton plan would have herded almost all Americans under age sixty-five into large, government-sponsored health insurance purchasing alliances that would have contracted with insurers to offer a standard set of benefits at regulated prices. The plan came under fire from both Republicans and Democrats, including moderates from both parties, but it soon became apparent that what doomed it was a public unwilling to trust government to manage their health care. The critical literature has failed to offer a cogent analysis of why government control of health care does not work. American Health Care delivers that analysis. This volume examines why untoward consequences usually follow when government sets out to do good things. The problem with these methods is that their theoretical foundations are unclear. This has led to confusion about how to define the costs and health effects and how to interpret the results of these studies. In the environmental and traffic safety fields it is instead common to carry out traditional cost-bene:fit analyses of health improving programmes. This striking difference in how health programmes are assessed in different fields was the original motivation for writing this book. The aim of the book is to tty and provide a coherent framework within cost-bene:fit analysis and welfare economics for the different methods of economic evaluation in the health care field. The book is written in an easily accessible manner and several examples of applications of the different methods are provided.
