Nous étudierons les différentes techniques opératoires à notre disposition et l'évolution des nouvelles technologies afin de limiter les échecs thérapeutiques et faciliter la mise en œuvre pour le praticien. Enfin nous étudierons les différents matériaux biocompatibles à notre disposition nous permettant d'obtenir une étanchéité apicale du traitement et permettre ensuite la régénération tissulaire osseuse.

Cet échec, souvent accompagné d'une lésion inflammatoire péri-apicale d'origine endodontique (LIPOE), peut avoir différentes origines : irrigation insuffisante, sur ou sous obturation, oubli d'un canal... Face à l'échec d'un premier traitement endodontique, deux grandes approches existent pour conserver la dent sur l'arcade : le retraitement par voie orthograde et le retraitement par voie rétrograde. Ces différentes options thérapeutiques peuvent nous amener à nous interroger sur la meilleure attitude à adopter en fonction de la situation clinique. Dans un premier temps nous avons décrit les indications des retraitements endodontiques ainsi que les protocoles des deux grandes approches possibles : la voie orthograde et la voie rétrograde. Dans un second temps, nous nous sommes intéressés aux indications et contre-indications spécifiques de chacune de ces deux méthodes. Pour conclure, nous nous sommes appuyés sur les données avérées de la science pour évaluer les résultats cliniques des retraitements endodontiques réalisés par voie orthograde et par voie rétrograde.
