L'évolution du traitement antihypertenseur chez des patients présentant un Syndrome d'Apnées-Hypopnées Obstructives du Sommeil (SAHOS) appareillés par PPC a été peu étudié sauf chez les hypertendus réfractaires chez qui un bénéfice serait possible. But de l'étude : Évaluer l'évolution du traitement antihypertenseur chez des patients présentant un SAHOS sévère et avec un traitement antihypertenseur, à un an de traitement par PPC. Évaluer le recours au traitement antihypertenseur chez des patients présentant un SAHOS sévère et sans traitement antihypertenseur, à un an de traitement par PPC. Matériel et méthode : Étude quantitative, rétrospective, descriptive portant sur 133 patients présentant un SAHOS sévère (IAH moyen 49,6 ± 15,6 /h), âgés de 63,7 ± 12,5 ans, en majorité masculins (sex-ratio 3,43), avec un IMC moyen de 33,4 kg/m2. Patients séparés en deux sous-groupes : patients avec traitement antihypertenseur (n=94 patients) ; patients sans traitement antihypertenseur (n=39 patients). Analyse de l'évolution du traitement antihypertenseur de ces deux sous-groupes, à un an de traitement par PPC. Résultats : Dans le sous-groupe des patients avec traitement antihypertenseur : 71 % avaient conservé un traitement antihypertenseur strictement identique ; 10 % avaient diminué leur traitement antihypertenseur ; 14 % l'avaient augmenté. Les taux d'IAH, d'IMC et d'observance à la PPC avaient une influence sur ces résultats. Dans le sous-groupe des patients sans traitement antihypertenseur à T0, aucun patient ne recevait un traitement antihypertenseur à T1. Ce sous-groupe paraît avoir un IAH initial moins sévère et un IMC plus faible. Conclusion : Le Médecin Généraliste doit réévaluer régulièrement tout traitement antihypertenseur, particulièrement chez les patients présentant des comorbidités associées dont fait partie le SAHOS. Chez les patients ayant un traitement antihypertenseur, la PPC ne paraît pas influencer le traitement antihypertenseur, mais le taux initial d'IAH faible, l'IMC faible et une observance à la PPC élevée sont des facteurs pouvant amener à la diminution du traitement antihypertenseur. Chez les patients sans traitement antihypertenseur, la PPC pourrait retarder la prescription d'un traitement antihypertenseur.

Le syndrome d'apnées-hypopnées obstructives du sommeil (SAHOS) est un véritable problème de santé publique, en raison de sa prévalence dans la population générale et de ses conséquences sur la qualité de vie. Le SAHOS est responsable de problèmes métaboliques et cardiovasculaires dont l'hypertension artérielle résistante (HTAR). Le SAHOS reste sous-diagnostiqué, alors qu'il existe des tests simples de dépistage tels que le questionnaire de Berlin et l'échelle de somnolence d'Epworth. Dans ce contexte, l'objectif de l'étude DEPISAHT était de mettre en place et d'évaluer une stratégie de dépistage du SAHOS à l'officine chez les patients présentant une HTAR. Parmi les 17 pharmacies participantes à l'étude, les questionnaires de Berlin et d'Epworth ont été soumis à 69 patients prenant au moins trois médicaments antihypertenseurs. Au total, 30% des patients ont été dépistés positifs grâce au questionnaire de Berlin. Ce test ayant une sensibilité de 80%, cela permettrait de diagnostiquer 1 personne sur 4 dans notre population. Ce dépistage à l'officine est réalisable en pratique courante. D'une part le questionnaire de Berlin semble être un outil pratique et rapide pour le pharmacien et facilement compréhensible pour les patients. D'autre part, les pharmaciens sont motivés à 69% à opérer des changements dans leur organisation pour intégrer la mission pharmaceutique qu'est le dépistage du SAHOS.

Le traitement par pression positive continue (PPC) permet une baisse de PA variable selon les études mais qui semble modérée. L'objectif principal de notre travail était d'évaluer l'effet de la PPC sur la PA de patients hypertendus résistants porteur d'un SAOS.Il s'agit d'une étude multicentrique, randomisée, en simple aveugle, avec un groupe témoin. Les patients apnéiques avec indication de traitement par PPC pour leur SAOS ont été randomisés en deux groupes : PPC placebo et PPC active. Le suivi était de 6 mois dans le groupe PPC active, et de 9 mois dans le groupe PPC placebo (3 mois de PPC placebo puis 6 mois de PPC active). Le critère principal d'évaluation était la PA évaluée par MAPA. 61 patients hypertendus résistants ont été inclus dont 33 (54%) avec SAOS.Après 6 mois de ventilation par PPC active, les PAS et PAD nocturnes étaient significativement abaissées par rapport à l'inclusion, respectivement -5,5 mmHg (p=0,022) et -4 mmHg (p=0,015). Il n'y a pas eu de baisse significative sur la PA des 24H et la PA de jour. La PPC a permis de rétablir un profil dipper sur la PAS chez un nombre significatif de patients (64,5% vs 35,5% avant ventilation ; p=0,047). Après 3 mois de traitement, il n'existait pas de différence significative sur les paramètres tensionnels entre le groupe placebo et le groupe PPC active. Chez les patients ayant un SAOS et une HTA résistante, un traitement par PPC pendant 6 mois est efficace pour diminuer la PAS et la PAD nocturnes même si l'effet placebo ne semble pas négligeable.

Méthode : étude monocentrique prospective, sur des patients hypertendus et ventilés pour un SAS, ayant bénéficié d'une mesure ambulatoire de la pression artérielle (PA) au CHRU de Lille. Nous avons étudié l'influence de la prise d'au moins un traitement antihypertenseur le soir sur le contrôle tensionnel. Pour cela, nous avons divisé notre échantillon en 2 groupes, selon le critère d'une prise ou non d'un traitement antihypertenseur le soir. Secondairement, nous nous sommes intéressés aux paramètres associés à un bon contrôle de la PA nocturne. Résultats : 142 patients étaient inclus, dont 48 femmes (33,8 %). L'âge médian était de 62ans [54-68]. L'indice de masse corporel médian était de 32.7kg/m2 [29.5-36.3]. Il y avait 58 diabétiques (40.8 %) et 36 HTA réfractaires (25.4 %). L'IAH médian au diagnostic du SAS était de 40.2/heure [31-56]. 75 patients (52.8 %) prenaient au moins un traitement antihypertenseur au soir. Les 2 groupes n'étaient pas comparables. Ainsi chez les patients prenant au moins un traitement antihypertenseur au soir, nous observions une présence d'au moins un antécédent cardiovasculaire plus fréquente (p=0.023), une clairance de la créatinine plus faible (p=0.027) et un nombre total de traitements antihypertenseurs pris chaque jour plus élevé (p
