Book excerpt: As Canadian healthcare organizations implement electronic health records (EHRs), nurses are expected to use the technology in their practice. Findings of a literature review suggest that usability (ease of use, functionality, navigation and impact on workload), the organizational context (support from leadership, level of training, level of on-going support, physical environment and implementation process) and individual nurse characteristics (sex, age, nursing unit, years of experience as a registered nurse, country of nursing education, years of experience using an EHR, previous EHR use and formal informatics training) influence nurses' use of these systems. Thus, the purpose of this doctoral research was to better understand the relationships between the variables that make up usability, organizational context, individual nurse characteristics, and nurses' perceptions of EHR use. This study was conducted using a sequential mixed methods design with two phases. Phase One consisted of a cross sectional survey that was piloted and then administered to nurses in an acute care teaching hospital in Toronto, Canada. The aim of the survey was to obtain information about nurses' perceptions of the usability of the EHR, the organizational context, their individual nurse characteristics and their use of the system. Phase two involved focus groups to better understand the findings identified in the survey. Multivariable and hierarchical linear regression was conducted. A multivariable model made up of the variables ease of use, navigation and impact on workload, explained 13% of the variance in nurses' perceptions of EHR use, however navigation was the only significant predictor in the model. In the data from the focus groups, nurses described how they navigated through the EHR, and which functionalities supported or hindered their use of it. Results of this study provide insights into factors that may influence nurses' use of EHRs in an acute care hospital setting that have implications for research, nurse leaders, vendors, healthcare settings and nursing practice.

Technology advancements have impacted the way nursing students are educated. Many education institutions use academic EHRs and simulation as tools to develop Health information technology (HIT) skills in students. The challenge with hospital-based EHRs is that institution EHRs follow institution guidelines and protocols customized for that institution. This can create challenges and barriers for nursing faculty, students, and the University to provide adequate EHR training and to develop HIT skills. This study explored nursing students’ experiences when using an EHR at different clinical sites. In this study, Colaizzi’s methodology was used to develop themes and patterns about nursing students’ experiences with EHRs and utilized Edmund Husserl’s descriptive phenomenological approach. Husserl’s philosophical approach allows discovery of senior nursing student’s thoughts, ideas, and experiences about using an EHR that leads to a better understanding of how nursing students use an EHR in clinical settings. Nursing students need to be able to use electronic tools efficiently within the EHR to locate and use data effectively when caring for patients. Understanding the nursing students’ experiences with EHR can give insight into different teaching strategies and interventions that can improve training and experiences during clinical rotations.

A team of nurses is needed to provide around-the-clock patient care, and this process requires transfer of patient care responsibilities, a process known as a "handoff." The present study explored the role of electronic health records (EHRs) in structuring handoff communication and maintaining situation awareness among nurses during handoff. Several theoretical frameworks and models were used to guide this study, which aims to assess the nurses' satisfaction with the EHRs during handoff, to explore the contents and the context of handoff, and identify the artifacts the nurses use to in order to ensure the continuity of care and the delivery of quality handoff reports. A mixed-method approach was used to study the role of the EHRs, Al-Shifa, in structuring handoff communication and to explore handoff practices in Oman. The findings of this study demonstrated that the nurses had positive perceptions about the usefulness and ease of use of EHRs for structuring handoff communication and promoting effective and efficient transition of patient care during handoff. The study also revealed that nurses use a variety of artifacts to document, communicate, and coordinate patients' care reasonability during handoff. The use of electronic health records during handoff was reported to increase the accessibility to patients' medical records and ensure both the continuity of care and the delivery of quality handoff reports.

Approximately one-quarter of community-dwelling Americans aged 65 and older are considered to be socially isolated, and a significant proportion of adults in the United States report feeling lonely. People who are 50 years of age or older are more likely to experience many of the risk factors that can cause or exacerbate social isolation or loneliness, such as living alone, the loss of family or friends, chronic illness, and sensory impairments. Over a life course, social isolation and loneliness may be episodic or chronic, depending upon an individual's circumstances and perceptions. A substantial body of evidence demonstrates that social isolation presents a major risk for premature mortality, comparable to other risk factors such as high blood pressure, smoking, or obesity. As older adults are particularly high-volume and high-frequency users of the health care system, there is an opportunity for health care professionals to identify, prevent, and mitigate the adverse health impacts of social isolation and loneliness in older adults. Social Isolation and Loneliness in Older Adults summarizes the evidence base and explores how social isolation and loneliness affect health and quality of life in adults aged 50 and older, particularly among low income, underserved, and vulnerable populations.
