The 77-year old grandfather was struck in a hit-and-run crash while trying to cross a high-speed, six-lane road without crosswalks near his son’s home in Phoenix, Arizona. He was one of the more than 6,000 people killed while walking in America in 2018. In the last ten years, there has been a 50 percent increase in pedestrian deaths. The tragedy of traffic violence has barely registered with the media and wider culture. Disproportionately the victims are like Duarte-Rodriguez—immigrants, the poor, and people of color. They have largely been blamed and forgotten. In Right of Way, journalist Angie Schmitt shows us that deaths like Duarte-Rodriguez’s are not unavoidable “accidents.” They don’t happen because of jaywalking or distracted walking. They are predictable, occurring in stark geographic patterns that tell a story about systemic inequality. These deaths are the forgotten faces of an increasingly urgent public-health crisis that we have the tools, but not the will, to solve. Schmitt examines the possible causes of the increase in pedestrian deaths as well as programs and movements that are beginning to respond to the epidemic. Book excerpt: The fight for the future of the city street between pedestrians, street railways, and promoters of the automobile between 1915 and 1930. Before the advent of the automobile, users of city streets were diverse and included children at play and pedestrians at large. By 1930, most streets were primarily a motor thoroughfares where children did not belong and where pedestrians were condemned as “jaywalkers.” In Fighting Traffic, Peter Norton argues that to accommodate automobiles, the American city required not only a physical change but also a social one: before the city could be reconstructed for the sake of motorists, its streets had to be socially reconstructed as places where motorists belonged. It was not an evolution, he writes, but a bloody and sometimes violent revolution. Norton describes how street users struggled to define and redefine what streets were for. He examines developments in the crucial transitional years from the 1910s to the 1930s, uncovering a broad anti-automobile campaign that reviled motorists as “road hogs” or “speed demons” and cars as “juggernauts” or “death cars.” He considers the perspectives of all users—pedestrians, police (who had to become “traffic cops”), street railways, downtown businesses, traffic engineers (who often saw cars as the problem, not the solution), and automobile promoters. He finds that pedestrians and parents campaigned in moral terms, fighting for “justice.” Cities and downtown businesses tried to regulate traffic in the name of “efficiency.” Automotive interest groups, meanwhile, legitimized their claim to the streets by invoking “freedom”—a rhetorical stance of particular power in the United States. Fighting Traffic offers a new look at both the origins of the automotive city in America and how social groups shape technological change.

Book excerpt: Social isolation and loneliness are serious yet underappreciated public health risks that affect a significant portion of the older adult population. Approximately one-quarter of community-dwelling Americans aged 65 and older are considered to be socially isolated, and a significant proportion of adults in the United States report feeling lonely. People who are 50 years of age or older are more likely to experience many of the risk factors that can cause or exacerbate social isolation or loneliness, such as living alone, the loss of family or friends, chronic illness, and sensory impairments. Over a life course, social isolation and loneliness may be episodic or chronic, depending upon an individual's circumstances and perceptions. A substantial body of evidence demonstrates that social isolation presents a major risk for premature mortality, comparable to other risk factors such as high blood pressure, smoking, or obesity. As older adults are particularly high-volume and high-frequency users of the health care system, there is an opportunity for health care professionals to identify, prevent, and mitigate the adverse health impacts of social isolation and loneliness in older adults. Social Isolation and Loneliness in Older Adults summarizes the evidence base and explores how social isolation and loneliness affect health and quality of life in adults aged 50 and older, particularly among low income, underserved, and vulnerable populations. This report makes recommendations specifically for clinical settings of health care to identify those who suffer the resultant negative health impacts of social isolation and loneliness and target interventions to improve their social conditions. Social Isolation and Loneliness in Older Adults considers clinical tools and methodologies, better education and training for the health care workforce, and dissemination and implementation that will be important for translating research into practice, especially as the evidence base for effective interventions continues to flourish.

Book excerpt: Approximately 1.3 million people die each year on the world's roads, and between 20 and 50 million sustain non-fatal injuries. The Global status report on road safety is the first broad assessment of the road safety situation in 178 countries, using data drawn from a standardized survey. The results show that road traffic injuries remain an important public health problem, particularly for low-income and middle-income countries. Pedestrians, cyclists and motorcyclists make up almost half of those killed on the roads, highlighting the need for these road users to be given more attention in road safety programmes. The results suggest that in many countries road safety laws need to be made more comprehensive while enforcement should be strengthened. The Global status report on road safety results clearly show that significantly more action is needed to make the world's roads safer. [Ed.]

Book excerpt: The United States is among the wealthiest nations in the world, but it is far from the healthiest. Although life expectancy and survival rates in the United States have improved dramatically over the past century, Americans live shorter lives and experience more injuries and illnesses than people in other high-income countries. The U.S. health disadvantage cannot be attributed solely to the adverse health status of racial or ethnic minorities or poor people: even highly advantaged Americans are in worse health than their counterparts in other, "peer" countries. In light of the new and growing evidence about the U.S. health disadvantage, the National Institutes of Health asked the National Research Council (NRC) and the Institute of Medicine (IOM) to convene a panel of experts to study the issue. The Panel on Understanding Cross-National Health Differences Among High-Income Countries examined whether the U.S. health disadvantage exists across the life span, considered potential explanations, and assessed the larger implications of the findings. U.S. Health in International Perspective presents detailed evidence on the issue, explores the possible explanations for the shorter and less healthy lives of Americans than those of people in comparable countries, and recommends actions by both government and nongovernment agencies and organizations to address the U.S. health disadvantage.
