Book excerpt: Quantitative magnetic resonance imaging for cancer is an invaluable noninvasive imaging tool with superior tissue contrast and greater specificity for disease than other imaging modalities; however, MRI is costly and lengthy, making it difficult to deploy quantitative imaging techniques alongside existing clinical protocols. Moreover, radiologists require high spatial resolution images for anatomical interpretation, which is not often reconcilable with the need for high temporal resolution in dynamic quantitative imaging schemes. The purpose of this dissertation is to develop novel analysis and acquisition methods that support fast quantitative imaging for substantiating clinical protocols without necessitating significantly longer scan times, thereby providing opportunity for clinical deployment of quantitative MRI protocols. This purpose is achieved through three distinct approaches. First, we take high temporal resolution dynamic contrast-enhanced MRI data from women with locally advanced breast cancer and retrospectively abbreviate the data to varying degrees. We subsequently analyze the full-length and abbreviated datasets with perfusion models and assess the agreement between the quantitative parameters from the full-length and abbreviated analyses, showing that scans can be shortened by up to 58%. Second, using fully sampled variable-flip angle raw data of the brain from healthy volunteers, we retrospectively accelerate the raw data by 8- to 36-fold and apply an untrained deep learning method with physics-based regularization to reconstruct the anatomical images while simultaneously computing the relevant quantitative parameter map. We achieve images with high spatial resolution and quantitative maps that agree strongly with the quantitative information computed from the fully sampled data. Finally, with the purpose of noninvasively describing tumor heterogeneity, we identify three distinct tumor habitats by clustering multiparametric quantitative information from diffusion-weighted and dynamic contrast-enhanced MRI data from a murine model of glioma collected at multiple imaging visits. We subsequently model the growth of the tumor habitats over time with a system of ordinary differential equations, thereby establishing a computationally fast pipeline for noninvasive tumor habitat growth prediction. Overall, these approaches yield novel methods that enable fast acquisition of quantitative MRI data as well as pipelines for retrospective abbreviation and analysis of existing quantitative MRI data

Book excerpt: Among medical imaging modalities, magnetic resonance imaging (MRI) stands out for its excellent soft-tissue contrast, anatomical detail, and high sensitivity for disease detection. However, as proven by the continuous and vast effort to develop new MRI techniques, limitations and open challenges remain. The primary source of contrast in MRI images are the various relaxation parameters associated with the nuclear magnetic resonance (NMR) phenomena upon which MRI is based. Although it is possible to quantify these relaxation parameters (qMRI) they are rarely used in the clinic, and radiological interpretation of images is primarily based upon images that are relaxation time weighted. The clinical adoption of qMRI is mainly limited by the long acquisition times required to quantify each relaxation parameter as well as questions around their accuracy and reliability. More specifically, the main limitations of qMRI methods have been the difficulty in dealing with the high inter-parameter correlations and a high sensitivity to MRI system imperfections. Recently, new methods for rapid qMRI have been proposed. The multi-parametric models at the heart of these techniques have the main advantage of accounting for the correlations between the parameters of interest as well as system imperfections. This holistic view on the MR signal makes it possible to regress many individual parameters at once, potentially with a higher accuracy. Novel, accurate techniques promise a fast estimation of relevant MRI quantities, including but not limited to longitudinal (T1) and transverse (T2) relaxation times. Among these emerging methods, MR Fingerprinting (MRF), synthetic MR (syMRI or MAGIC), and T1‒T2 Shuffling are making their way into the clinical world at a very fast pace. However, the main underlying assumptions and algorithms used are sometimes different from those found in the conventional MRI literature, and can be elusive at times. In this book, we take the opportunity to study and describe the main assumptions, theoretical background, and methods that are the basis of these emerging techniques. Quantitative transient state imaging provides an incredible, transformative opportunity for MRI. There is huge potential to further extend the physics, in conjunction with the underlying physiology, toward a better theoretical description of the underlying models, their application, and evaluation to improve the assessment of disease and treatment efficacy.

Book excerpt: Owing to its exquisitely sensitive contrast mechanism, diffusion magnetic resonance imaging is a powerful non-invasive approach for studying the microstructural properties of the human brain in vivo. Magnetic resonance images are made sensitive to the microscopic displacements of water molecules that take place in brain tissue as part of the natural, physical diffusion process. Tissue water is used as an intrinsic probe, revealing important clues into the subtle architectural features of normal and pathologic brain tissue. Typical inferences include the intravoxel orientation distribution of neuronal fibers and changes in diffusion resulting from cell swelling in acute stroke. However, despite the many important advances made in the field of diffusion magnetic resonance imaging over the past decade, quantitative inference in the human brain remains somewhat limited due to the lack of direct quantitative validation against realistic biological architectures and practical limitations in data collection due to sub-optimal design parameters and artifacts caused by patient motion during scanning. In addition, current methods to resolve neuronal fiber orientations are unable to disambiguate fiber structures at different microscopic length (size) scales. In this dissertation I present a series of studies addressing each of these important limitations, starting with a general real-time image-based technique for motion correction in magnetic resonance images and ending with a series of studies on inferring complex fiber orientations from diffusion data, addressing issues such as quantitative histological validation, optimal acquisition, and improved multi-scale analysis.

Book excerpt: Magnetic Resonance Imaging, MRI, is a powerful imaging modality that is frequently used in both clinical and academic settings. With its advantages of flexibility in signal encoding, we can use MRI to non-invasively visualize various soft-tissue contrasts, showing not only anatomical but also metabolic and functional information. In addition, MRI is a radiation-free modality which makes it favorable in numbers of clinical applications because of the reduced radiation-risk compared with other radiology modalities such as X-ray, Computed Tomography (CT), Positron Emission Tomography (PET) etc. Despite the advantages of MRI techniques, there are still several challenges preventing MRI from becoming more efficient and accessible. First, the scan time for MRI is usually longer than other modalities such as X-ray and CT, since it requires enough measurements to resolve high-quality images for diagnostic tasks. In order to accelerate MRI, various fast-imaging techniques, such as Parallel Imaging (PI) and Compressed Sensing (CS) have been proposed to speed up MRI acquisition using under-sampling. However, it is still unclear what is the best approach to conduct the under-sampling as different under-sampling patterns may result in different reconstruction quality. Second, the reconstruction methods for under-sampled MRI need further improvement. The reconstruction algorithms are formed as nonlinear optimization problems using iterative optimization that can be time-consuming. Fixed and handcrafted penalty terms are usually used to regularize the optimization, which are hard to tune. There are often trade-offs between the speed of the algorithm and the quality of resulting images. In many cases, the imperfect artifact suppression or over-smoothing slows down the clinical adoption of these fast-imaging techniques. Third, MR images are typically not quantitative. Most clinical MRI protocols used nowadays are contrast-weighted sequences, which incorporate the tissue contrasts in qualitative ways. Therefore, the resulting MR images may vary a lot between different protocols and scanners, which makes it very difficult for radiologists to conduct quantitative analysis or longitudinal comparison. In this work, we propose to resolve these remaining challenges to further improve MRI technologies. We utilized state-of-the-art Machine Learning and Deep Learning algorithms to significantly improve these three essential components in MRI: faster acquisition, better reconstruction, and more accurate qualification. Specifically, we firstly propose a machine learning based method to optimize the undersampling pattern for accelerated acquisition. The results, validated on in-vivo multi-contrast brain and prostate MRI datasets, demonstrate that the proposed method can generalize well for different anatomy. It enables efficient (5sec-10sec) and adaptive under-sampling pattern optimization at per-subject/per-scan level, and achieves 30%-50% lower PI+CS reconstruction error at the same acceleration factor. To improve MRI acquisition with a safer protocol and lower contrast dose, a deep learning model is developed to enhance the MRI. The proposed Deep Learning method yielded significant (N=50, p 0.001) improvements over the low-dose (10%) images ( 5dB PSNR gains and > 11.0% SSIM). Following the initial work of PAUL LAUTERBUR and RAYMOND DAMADIAN in the early 1970s demonstrating that it was possible to use NMR to produce im ages, progress in the medical fields was relatively slow. Recently, however, with the availability of commercial systems, progress has been very rapid, with increasing acceptance of MRI as a basic imaging technique, and the develop ment of exciting new applications. MRI is a relatively complex technique. First, the image depends on many more intrinsic and extrinsic parameters than it does of in techniques like X-ra diography and computed tomography, and secondly, the intrinsic parameters such as T1 and T2 are conceptually complex, involving ideas not usually de scribed in traditional medical imaging courses. In order to produce good MR images efficiently, and to obtain the maximum information from them, it is necessary to appreciate, if not to fully understand, these parameters. Further more, knowledge of how the image is produced helps in appreciating the ori gin of the artifacts sometimes found in MRI due to effects like patient motion and fluid flow.
