This report specifically reviews the evidence on the potential mechanisms by which smoking causes diseases and considers whether a mechanism is likely to be operative in the production of human disease by tobacco smoke. This evidence is relevant to understanding how smoking causes disease, to identifying those who may be particularly susceptible, and to assessing the potential risks of tobacco products.

Book excerpt: Data suggest that exposure to secondhand smoke can result in heart disease in nonsmoking adults. Recently, progress has been made in reducing involuntary exposure to secondhand smoke through legislation banning smoking in workplaces, restaurants, and other public places. The effect of legislation to ban smoking and its effects on the cardiovascular health of nonsmoking adults, however, remains a question. Secondhand Smoke Exposure and Cardiovascular Effects reviews available scientific literature to assess the relationship between secondhand smoke exposure and acute coronary events. The authors, experts in secondhand smoke exposure and toxicology, clinical cardiology, epidemiology, and statistics, find that there is about a 25 to 30 percent increase in the risk of coronary heart disease from exposure to secondhand smoke. Their findings agree with the 2006 Surgeon General's Report conclusion that there are increased risks of coronary heart disease morbidity and mortality among men and women exposed to secondhand smoke. However, the authors note that the evidence for determining the magnitude of the relationship between chronic secondhand smoke exposure and coronary heart disease is not very strong. Public health professionals will rely upon Secondhand Smoke Exposure and Cardiovascular Effects for its survey of critical epidemiological studies on the effects of smoking bans and evidence of links between secondhand smoke exposure and cardiovascular events, as well as its findings and recommendations.

Book excerpt: The fact that tobacco ingestion can affect how people feel and think has been known for millennia, placing the plant among those used spiritually, honori?cally, and habitually (Corti 1931; Wilbert 1987). However, the conclusion that nicotine - counted for many of these psychopharmacological effects did not emerge until the nineteenth century (Langley 1905). This was elegantly described by Lewin in 1931 as follows: “The decisive factor in the effects of tobacco, desired or undesired, is nicotine. . . ”(Lewin 1998). The use of nicotine as a pharmacological probe to und- stand physiological functioning at the dawn of the twentieth century was a landmark in the birth of modern neuropharmacology (Limbird 2004; Halliwell 2007), and led the pioneering researcher John Langley to conclude that there must exist some “- ceptive substance” to explain the diverse actions of various substances, including nicotine, when applied to muscle tissue (Langley 1905). Research on tobacco and nicotine progressed throughout the twentieth century, but much of this was from a general pharmacological and toxicological rather than a psychopharmacological perspective (Larson et al. 1961). There was some attention to the effects related to addiction, such as euphoria (Johnston 1941), tolerance (Lewin 1931), and withdrawal (Finnegan et al. 1945), but outside of research supported by the tobacco industry, addiction and psychopharmacology were not major foci for research (Slade et al. 1995; Hurt and Robertson 1998; Henning?eld et al. 2006; Henning?eld and Hartel 1999; Larson et al. 1961).

Proponents of e-cigarette use also tout the potential benefits of e-cigarettes as devices that could help combustible tobacco cigarette smokers to quit and thereby reduce tobacco-related health risks. Others are concerned about the exposure to potentially toxic substances contained in e-cigarette emissions, especially in individuals who have never used tobacco products such as youth and young adults. Given their relatively recent introduction, there has been little time for a scientific body of evidence to develop on the health effects of e-cigarettes. Public Health Consequences of E-Cigarettes reviews and critically assesses the state of the emerging evidence about e-cigarettes and health. This report makes recommendations for the improvement of this research and highlights gaps that are a priority for future research.

Lung cancer in neversmokers is on the rise and is an important issue of public health yet is relatively much less studied. This paper reviews the research on SHS exposure on neversmokers and its relationship to lung cancer. Methods: Searches were conducted using PubMed for articles published between January 1, 2008 and July 25, 2013. Search terms included "lung cancer and secondhand smoke", "lung cancer and environmental tobacco smoke", "lung cancer risk and never smokers", "lung cancer and passive smoking", "lung cancer and nonsmokers", "smoking bans and secondhand smoke", "lung cancer prevention", and "global cancer statistics". Results: In total, 12 articles were found to satisfy the inclusion criteria. Studies included measured significant effects of SHS by particulate matter, levels of cotinine and NNAL, metabolites of nicotine and reliable biomarkers of recent nicotine intake. One study discussed the mutagenic effects of secondhand smoke. Three articles discussed effects of outdoor SHS exposure in the setting of nonsmoking bans. Conclusions: Secondhand smoke exposure and its effects on neversmokers including lung cancer is a significant public health issue deserving of more research for risk management guidelines relevant for urban planning, healthcare practitioners and patients.

Book excerpt: There are concerns about the usefulness of cotinine and trans-3'-hydroxycotinine (3HC), as biomarkers of risk in populations with light tobacco smoke exposure. Using CHILD cohort study data, multiple linear and logistic regression was used to determine how well questionnaire responses explained urinary concentrations of cotinine and 3HC in infants, whether these concentrations predicted childhood asthmatic and allergic disease risk, and whether breastmilk facilitated nicotine exposure. Predictive models explained 31% and 41% of the variation in cotinine and 3HC, respectively. Only 23% of the infants had urinary concentrations consistent with second-hand smoke (SHS) exposure. The majority (92%) of household smoking occurred outdoors. While smokers breastfed less often, breastmilk did facilitate nicotine intake. The implications of dietary nicotine sources through breastmilk were inconclusive. Subclinical impacts and the pervasiveness of thirdhand smoke pose a challenge for public health and we should re-evaluate our use and interpretation of nicotine biomarkers in low-smoke exposure settings.

Book excerpt: This textbook focuses on the nascent field of Immunoepidemiology that addresses how differences in immune responses among individuals affect the epidemiology of infectious diseases, cancer, hypersensitivity, and autoimmunity. The idea for the book originated from a course entitled “Immunology for Epidemiologists“ at the Yale School of Public Health. While many fine textbooks are available that address the immunological responses of individuals to pathogens, these provided very little information regarding how immunological variation among populations affects the epidemiology of disease. And yet, it has long been recognized that there is great immunologic diversity among people, which can have a profound effect on the epidemiology of disease. Careful review of the immunologic and epidemiologic literature revealed that there have been relatively few publications concerning immunoepidemiology and that no textbook is available on the subject. This textbook therefore aims to fill this void by providing a much-needed tool to comprehensively and efficiently teach immunoepidemiology. The book includes a section on the basic principles of immunology, and then applies them to particular examples of disease in human populations. The target audience for this text book are Masters of Public Health students. Others who should also find it of interest include PhD students in epidemiology, immunology, medical students, generalists, and specialists in immunology, infectious diseases, cancer, and rheumatology.
