The diagnostic process is a complex, collaborative activity that involves clinical reasoning and information gathering to determine a patient's health problem. According to Improving Diagnosis in Health Care, diagnostic errors-inaccurate or delayed diagnoses-persist throughout all settings of care and continue to harm an unacceptable number of patients. It is likely that most people will experience at least one diagnostic error in their lifetime, sometimes with devastating consequences. Diagnostic errors may cause harm to patients by preventing or delaying appropriate treatment, providing unnecessary or harmful treatment, or resulting in psychological or financial repercussions. The committee concluded that improving the diagnostic process is not only possible, but also represents a moral, professional, and public health imperative. Improving Diagnosis in Health Care, a continuation of the landmark Institute of Medicine reports To Err Is Human (2000) and Crossing the Quality Chasm (2001), finds that diagnosis-and, in particular, the occurrence of diagnostic errorsâ€"has been largely unappreciated in efforts to improve the quality and safety of health care. Without a dedicated focus on improving diagnosis, diagnostic errors will likely worsen as the delivery of health care and the diagnostic process continue to increase in complexity. Just as the diagnostic process is a collaborative activity, improving diagnosis will require collaboration and a widespread commitment to change among health care professionals, health care organizations, patients and their families, researchers, and policy makers. The recommendations of Improving Diagnosis in Health Care contribute to the growing momentum for change in this crucial area of health care quality and safety.

Book excerpt: Popularized by the hit television show, the phrase “breaking bad” is defined in urban slang as someone who challenges convention, defies authority, or rejects moral and social norms. Running from 2008 to 2013 on AMC, Breaking Bad featured one of the most unforgettable characters in television history: Walter White, a high school chemistry teacher, husband, and father, who is diagnosed with terminal cancer. For five seasons, fans watched as Walter White tried to secure financial security for his family by using his chemistry skills to manufacture drugs. Throughout the series’ run, Walter White was the epitome of the phrase “breaking bad”, as he broke the law and continually rejected the social mores that he had dutifully followed until his cancer diagnosis. Taking its cue from Walter White, this volume explores the various ways in which artists, patrons, and art historians throughout history have broken bad by defying authority, challenging convention, or rejecting the norm. For example, artists also sometimes break away from tradition by using unconventional iconography, as is the case in Chapter Two, which investigates how Etruscan tomb reliefs show mourning rather than celebration. The book also includes a chapter in which an art historian breaks bad by challenging the conventional interpretation and date of an object, thus eschewing tradition and defying authority. In this case, Chapter Three disputes the largely accepted Hellenistic date and interpretation of the Tazza Farnese, and instead asserts that the cameo must be Roman. Spanning the art of ancient Etruria to the twentieth century, the eight chapters here explore the theme of breaking bad from a variety of time periods and artistic media, from Etruscan mirrors and Roman cameos to Baroque portraits and Italian Pop Art. Scholars approach the topic of breaking bad from a number of perspectives, including examining the artist, patronage, reception, propaganda, iconography, methodology, and use.

Yet despite some three decades of critical museology and photographic theory, and an extensive debate on the politics of representation, outside art museums, almost no critical attention has been given specifically to the roles, purposes and lives of these photographs within museums. This book brings into focus the ubiquitous yet entirely unconsidered work that photographs are put to in museums. The authors' argument is that there is an economy of photographs in museums which is integral to the processes of the museum, and integral to the understanding of museums. The international contributors, drawn from curators and academics, reflect a range of visual and museological expertise. After an introduction setting out the range of questions and problems, the first part addresses broad curatorial strategies and ways of thinking about photographs in museums. Shifting the emphasis from curatorial practices and anxieties to the space of the gallery, this is followed by a series of case studies of exhibitionary practices and the museum strategies that support them. The third section focuses on the role of photographs in the museum articulation of ‘difficult histories’. A final section addresses photograph collections in a digital environment. New technologies and new media have transformed the management, address and purposing in photographs in museums, from cataloguing practices to streaming on social media. These growing practices challenge both traditional hierarchies of knowledge in museums and the location of authority about photographs. The volume emerges from PhotoCLEC, a HERA funded project on museums and the photographic legacy of the colonial past in a postcolonial and multicultural Europe.

As the title of this book indicates, my own interest in religious conversion is not sociological, historical, nor anthropolog ical. My primary interest is not even in the domain of the psychology of religion. That is, this book is not a comprehensive review of the social psychological factors that shape religious beliefs in general and religious conversions in particular. Rather, my primary interest is in the experience of conversion as an instance of a meaningful, sudden change in the course of individu al lives. Religious conversion is examined in this book prinwrily from the point of view of the psychology of the self. My aim is to elucidate the experience of religious conversion as a change in the self and to raise suggestions for the study of the self that derive from the data on religious conversion. This interest dictated the scope as well as the methods of the present investigation. Namely, I have chosen to study individuals who have indeed changed visibly as a result of their conversion. My inquiry was based on self-report, assuming the importance of the person's own point of view. Finally, my inquiry was semi-clinical, vii viii PREFACE based on the assumption of an underlying structure to the varieties of conversion experiences.
