Thymic senescence significantly lowers the production of naive T cells, leading to increased incidence of infections, cancer and autoimmune diseases. Certain external factors can accelerate thymic senescence. These include chemicals (copper-chelators), hormones (androgens), infections (viruses, fungi, protozoa). Others may slow the aging process of the thymus including perturbations to the hormonal (sex-steroid) system, genetic alterations (PPARgamma deficiency) or chemical compounds (PPARgamma antagonists). Thymic senescence research may provide insight to underlying molecular events and potentially appoint novel therapeutic targets for senescence intervention strategies. These hold promise to postpone thymus senescence and enhance T cell production. That would result in a decreased incidence of infections, cancer and autoimmune diseases, currently affecting the elderly. The attributed drop in healthcare costs and gain in quality of life share tremendous economic and social interest.

During this time, the abundance of fascinating literature on the immunological and functional aspects of this organ has been in contrast to the paucity of reports on its role in pathology. The causal or formal pathogenesis even of many of the well-documented pathological fea tures is so far unexplained or at least uncertain. In spite of some concep tual progress related to the work of CASTLEMAN, LEVINE, and ROSAI, we regrettably have to say that in pathology the thymus remains almost as inconspicuous as 25 years ago. Only the new techniques developed in recent years have made it possible to look as closely into the complicated structural organization of the thymus as it appears to be necessary to observe and document abnormalities. Major steps have been taken with the advent of mono clonal antibody immune histochemistry and with detailed ultrastructur al studies in embryology. At this point, pathologists and researchers with a special interest in the thymus were asked to give overviews of their respective fields of interest in light of recent findings in immunology and basic insights into the structural-functional interrelationship of the human thymus. The results of this initiative have been brought together in this volume.

Book excerpt: This chapter summarised current knowledge on thymic senescence, a central immune tissue that suffers significant morphological changes and functional impairment during ageing. The epithelial network is in focus that provides the niche for developing thymocytes until adipose involution begins. We have discussed physiological thymic epithelial senescence in detail with respect to the signalling pathways involved in the process (Kvell et al. 2010). It has also been shown that steroid induced accelerated rate thymic epithelial senescence quite resembles physiological rate senescence (except for its speed) at the molecular level (Talaber et al. 2011). The data presented confirm that Wnt4 can efficiently rescue thymic epithelial cells from steroid-induced adipose involution at the molecular level (Talaber et al. 2011). Since physiological and steroid-induced thymic epithelial senescence are identical at the molecular level, it is anticipated that sustained Wnt4 presence in the thymic context can efficiently prolong FoxN1 expression, maintain thymic epithelial identity and prevent transdifferentiation towards adipocyte lineage. The same works identify LAP2[alpha] as a pro-ageing molecular factor promoting the trans-differentiation of thymic epithelial cells into preadipocytes via EMT. The thymus selective decrease of LAP2[alpha] activity through small molecule compounds could theoretically shift the delicate molecular balance towards the same direction as increased Wnt4 presence.

To the gerontologic research community, therefore, immunity also becomes the ability of an organism to resist age. Were the immune and other protective systems of the body able to maintain themselves over the course of time, and if there were no degradation related to age, the everyday loss of energy and vitality that occurs in the lives of older people as a consequence of viruses, arthritis, and other debilitating circumstances would be greatly lessened. The objective of gerontologists is not just to extend the life span but rather to improve the vigor, health, and quality oflife. To date, we have not developed a single index to measure immunity that is of use clinically in the evaluation of older people and of their immunologic compe tence. It may not be surprising that just such a clinical index may be available in the not-too-distant future. We can also look forward to the assembling of a greater body of information explaining how and why the immune system fails with age while, paradoxically, the incidence of autoimmune diseases increases with age. It is this latter phenomenon that may playa part in a wide range of chronic diseases from rheumatoid arthritis to senile dementia.
