A park ranger observed the forced landing and after extinguishing a small fire near the engine, called for assistance via his radio. Between 0747 and 0752 hours several rescue units arrived on scene, removed the victims from the helicopter and transported the two injured victims to a nearby hospital. NIOSH investigators concluded that, in order to prevent similar incidents, fire departments utilizing helicopters for various reasons (e.g. air ambulance, water rescue, etc.) should review operators manuals, training bulletins, and pertinent information regarding air operations safety and procedures and update standard operating procedures as appropriate. Additionally, manufacturers should take steps to ensure that manuals regarding helicopter operation and maintenance are clear.

The ambulance was responding with lights and siren to a car fire with injuries. A motorist in a privately owned vehicle (POV) turned into the path of the ambulance as it was passing the POV. The ambulance struck the POV, lost control, and left the right side of the roadway. The ambulance overturned three times, and the victim was ejected. The driver sustained minor injuries. The victim's use of a seat belt could not be determined with certainty. The victim was airlifted by helicopter to a local trauma center where blunt force chest injuries, and underlying medical conditions, contributed to his death on December 28, 2009.

Book excerpt: On June 02, 2011, a 48 year-old career lieutenant and a 53 year-old fire fighter/paramedic died in a multi-level residential structure fire while searching for the seat of the fire. Note: The residential structure where the fatalities occurred was built on a significantly sloped hillside common throughout the city. The fire floor was one floor below street level. Six companies and three command chiefs were dispatched to a report of an electrical fire at a residential home. When Engine 26, staffed with a lieutenant, fire fighter/paramedic (the victims), and driver arrived at approximately 1048 hours, they noticed light smoke showing as they made entry through the front door, side A, street level, of the building. Minutes later, the incident commander (IC) tried contacting them over the radio, but received no response. A battalion chief (BC) assigned to "the fire attack group" followed the hoseline through the door and spoke to the victims on the street level floor. The lieutenant stated to the BC that the fire must be a floor below them. The BC stated they would attack the fire from the side B of the structure and exited the front door. The victims did not follow. A few minutes later the IC again tried to contact Engine 26 via radio with no response. The crew from Engine 24, assigned to back up Engine 26, and a split crew from Rescue 1 tried to make entry through the door in the garage but could not advance due to the heat. The BC went to the side B door, located one floor below street level, and forced the door with the Engine 11 crew on the hoseline. They immediately felt a blast of heat from the fully involved basement area. The Rescue 1 crew backed out of the garage and re-entered on side B after the Engine 11 crew knocked down the large room and contents fire. At about the same time, the Engine 24 crew also backed out of the garage and followed the Engine 26 crew's hoseline through the front door. In zero visibility conditions, separate members of the Engine 24 crew independently found a downed member of the Engine 26 crew. The Incident Commander was alerted of a downed fire fighter but, did not initially realize, until moments later that it was actually two downed fire fighters. Both victims were removed from the structure and immediate medical treatment was provided. The victims were transported to the local medical center where the lieutenant was pronounced dead and the fire fighter/paramedic died two days later.

In most countries, injury is the leading cause of death and disability for children and young adults age 1 to 39 years. Each year in the United States, injury claims about 170,000 lives and results in over 30 million emergency room visits and 2.5 million hospitalizations. Injury is medically defined as organ/tissue damages inflicted upon oneself or by an external agent either accidentally or deliberately. Injury encompasses the undesirable consequences of a wide array of events, such as motor vehicle crashes, poisoning, burns, falls, and drowning, medical error, adverse effects of drugs, suicide and homicide. The past two decades have witnessed a remarkable growth in injury research, both in scope and in depth. To address the tremendous health burden of injury morbidity and mortality at the global level, the World Health Organization in 2000 created the Department of Injury and Violence Prevention, which has produced several influential reports on violence, traffic injury, and childhood injury. The biennial World Conference on Injury Control and Safety Promotion attracts a large international audience and has been successfully convened nine times in different countries. In the United States, the National Center for Injury Prevention and Control became an independent program of the federal Centers for Disease Prevention and Control in 1997. Since then, each state health department has created an office in charge of injury prevention activities and over a dozen universities have established injury control research centers. This volume will fill an important gap in the scientific literature by providing a comprehensive and up-to-date reference resource to researchers, practitioners, and students working on different aspects of the injury problem and in different practice settings and academic fields.

The driver of one of the vehicles, a highway patrol officer, the fire fighter, and the Victim were standing near the roadway to the rear of the parked fire department patrol vehicle when an out-of-control pickup truck slid toward them. The fire fighter ran to the front of the fire patrol vehicle and was not injured. The highway patrol officer suffered injuries from diving to the ground and falling. The Victim and the civilian driver were unable to get out of the truck's path and were struck as the truck rolled down the embankment. The highway patrol officer and the civilian driver were injured and transported to a local medical center. The Victim was transported by ambulance to the local medical center where he was pronounced dead from multiple blunt force trauma.

The engine's auxiliary braking system (engine brake) was engaged at the time of the incident. The victim was found lying unresponsive under the running board on the passenger's side of the vehicle. The victim was found to have lacerations around his mid-section and no pulse.

At 1228 hours, dispatch sent four companies (2 engines, 1 truck, and a squad company) to an automatic fire alarm at a multistory medical building. Engine 2 reported a working fire with heavy smoke and fire showing on the top floor rear of structure and requested a second alarm. Instead, the dispatch center sent a Division Chief, Battalion Chief, Safety Officer, Engine Company, Rescue company, and Fire Marshal as the complement for a working fire first alarm. Engine 2 fire fighters connected 100 feet of 2 1/2-inch hoseline to the standpipe in the stairwell on the top floor. Engine 2 flowed water for several seconds when their low-air alarms went off and they exited the structure. Rescue 3 took the hoseline but had issues getting water to flow. The top two floors and north stairwell were now heavy with smoke. Ladder 1 was setting up at the A/B corner to access a window on the 6th level (labeled 5th floor), side B. Two of Rescue 3's members were getting low on air, and the crew moved to the stairwell to exit the building. Two of the members exited the stairwell, but the captain (the victim) went down the hallway and two fire fighters followed him. The low-air alarms of the victim and two fire fighters were sounding when they reached the hallway that was connected to the fire rooms. One of the fire fighters grabbed the victim, who was acting confused, and started back to the stairwell. The other fire fighter got separated in heavy smoke, went toward the fire room, and made it to a window where the platform of Ladder 1 was located just as he ran out of air. The victim ran out of air and told his partner they needed to buddy breath. The victim unclipped his regulator as his partner connected the buddy breather and all the partner's air escaped through the victim's SCBA. The victim transmitted a Mayday and activated his PASS (personal alert safety system). The fire fighter also attempted to activate his PASS, called several Maydays prior to removing his mask and began looking for an escape path. At some point, the victim vomited in his facepiece and removed it. The fire fighter crawled down a hallway trying to open several doors unsuccessfully until he found a door he could open to the south stairwell. The fire fighter placed his axe in the door and then returned to the victim. The fire fighter grabbed the victim and pulled him to the south stairwell where he collapsed and the pair fell down a flight of stairs. The victim and the fire fighter were eventually located in the south stairwell by another officer who had exited the north stairwell and heard the victim's PASS alarm sounding. Medic units transported the victim and the fire fighter to the hospital.
