It first identifies four key problems in the health care systems in developing countries: 1) public facilities, which provide the bulk of secondary and tertiary health care services in most countries, offer services of poor quality; 2) providers cannot be enticed to rural and urban marginal areas, leaving large segments of the population without adequate access to health care; 3) the composition of health services offered and consumed is sub-optimal; and 4) coordination in the delivery of care, including referrals, second opinions, and teamwork, is inadequate. The paper examines each problem in turn and assesses the extent to which changes in provider payments might address it.

And yet it has turned out to be surprisingly difficult to provide a fully satisfactory egalitarian theory. The cutting-edge articles in Egalitarianism move the debate forward. They are written by some of the leading political philosophers in the field. Recent issues in the debate over equality are given careful consideration: the distinction between 'telic' and 'deontic' egalitarianism; prioritarianism and the so-called 'levelling down objection' to egalitarianism; whether egalitarian justice should have 'whole lives' or some subset thereof as its temporal focus; the implications of Scanlon's contractualist account of the value of choice for egalitarian justice; and the question of whether non-human animals fall within the scope of egalitarianism and if so, what the implications are. Numerous 'classic' issues receive a new treatment too: how egalitarianism can be justified and how, if at all, this value should be combined with other values such as desert, liberty and sufficiency; how to define the 'worst off' for the purposes of Rawls' difference principle; Elizabeth Anderson's feminist account of 'equality of relations'; how equality applies to risky choices and, in particular, whether it is justifiable to restrict the freedom of suppliers who wish to release goods that confer different levels of risk on consumers, depending on their ability to pay. Finally, the implications of egalitarianism and prioritarianism for health care are scrutinized. Those disparities are caused not only by fundamental differences in health status across segments of the population, but also because of inequities in factors that impact health status, so-called determinants of health. Only part of an individual's health status depends on his or her behavior and choice; community-wide problems like poverty, unemployment, poor education, inadequate housing, poor public transportation, interpersonal violence, and decaying neighborhoods also contribute to health inequities, as well as the historic and ongoing interplay of structures, policies, and norms that shape lives. When these factors are not optimal in a community, it does not mean they are intractable: such inequities can be mitigated by social policies that can shape health in powerful ways. Communities in Action: Pathways to Health Equity seeks to delineate the causes of and the solutions to health inequities in the United States. This report focuses on what communities can do to promote health equity, what actions are needed by the many and varied stakeholders that are part of communities or support them, as well as the root causes and structural barriers that need to be overcome.

Embracing a new institutionalist approach, it develops two analytical perspectives: determination, which identifies the mechanisms of social health insurance, and strategic interaction, which explores the interaction among social health insurance agencies, healthcare providers, patients, and institutions. It reveals the poor performance of social health insurance in decreasing out-of-pocket health expenditures caused by a trade-off between the reimbursement, behavior management, and purchasing mechanisms of social health insurance programs. Further, it finds that the inequitable allocation of healthcare resources and patients’ concerns regarding the benefits offset the strategies used by social health insurance agencies to manage care-seeking behavior. It also discovers that the complex interactions between insurance agencies, doctors, patients and a larger disenabling institutional surrounding restricts the purchasing efficiency of social health insurance. This book is characterized by its unique synthesis of the role of the institutional arrangement of social health insurance in China, the interaction between the stakeholders in health sectors, and of the relationship between healthcare institutions, actors, and policy outcomes. Providing a comprehensive overview, it enables scholars and graduate students to understand the ongoing process of social health insurance reform as well as the dynamics of health cost inflation in China. It also benefits policymakers by recommending a single-payer model based on an evidence-based investigation.
