Book excerpt: Orthognathic surgery encompasses various surgical procedures used for the correction of dentofacial deformities from malocclusions, prognathism, retrognathism, obstructive sleep apnea, to TMJ disorders and others. While the primary objective of orthognathic surgery is to optimize the occlusion and improve overall masticatory function, enhancement of the facial aesthetics is also an outcome of concern. However, tissue manipulation can result in unpredictable and sometimes, unfavorable outcomes. Thus, it is important to understand the impact hard tissue movement has on the soft tissue. Purpose: The primary outcome of this study was to determine the hard to soft tissue ratio following orthognathic surgery. Secondary outcomes focused on evaluating changes in soft tissue. Methods: Pre and post-operative cone beam computed tomography (CBCT) scans were taken prior to surgery and between 2 and 4 weeks post-operatively. 3D Photos were captured pre-operatively and at 3-months, 6-months and 12-months post-operatively. Image analysis was done using Dolphin® software. Three separate analysis were done: the magnitude of bony movement (HT), comparing hard to soft tissue movement (HT: ST), and changes in soft tissues (ST). The primary and secondary outcomes were then evaluated using these analyses stratified by time, sex, procedure type, age and BMI. Both a Wilcoxon signed-rank test and a regression model fit were used to evaluate the data. All patients planned for orthognathic surgery at Dalhousie University were screened and enrolled if they met the inclusion criteria. Results: 12 total patients completed the 12-month follow up, 19 patients completed the 6-month follow up and 44 patients completed the 3-month follow up. There were 3 treatment groups: LF, BSSO and LF BSSO. There was no statistically significant change in the HT: ST ratio at 12-months post-operatively. There was a trend in the data that showed maximal increase in the HT: ST ratio at 6 months (1: 1.53, n=19, p=0.38), however it was not statistically significant. The nasolabial angle showed the greatest change with maxillary surgery (2.56mm, n=7, p=0.03) and in males (1.45mm, n=17, p=0.04). The mentolabial angle changed more in patients in the BSSO treatment group (5.08mm, n=11, p=0.03). The alar width increased after surgery (2.58mm, n=14, p=0.001), with the greatest changes in the LF treatment group (1.77mm, n=14, p=0.06). There were no significant differences seen in the mouth width, height of philtrum, and upper and lower lip thicknesses. Conclusion: The hard to soft tissue ratio does not change significantly 12-months post-surgery. Minor soft tissue changes were seen at the level of the nasolabial angle, mentolabial angle and the alar width. The patient attrition rate was significant in this study, and the study findings should be interpreted accordingly.

Abstract: Data on the three?dimensional (3?D) morphology of normal faces and facial deformities as well as objective techniques for evaluating postoperative changes are currently unavailable. With the advent of stereophotography and cone?beam CT (CBCT), it is possible to acquire 3?D images of soft and hard tissues of the maxillofacial complex. A series of studies were conducted aimed at 1) establishing 3?D facial anthropometric norms for Hong Kong young adults, 2) determining 3?D facial anthropometric features in skeletal Class II and III deformities, 3) characterizing the oropharyngeal space in Class II and III skeletal deformities, 4) exploring clinical applications of 3?D colour maps, 5) establishing a non?invasive technique for estimating serial volume changes and 6) creating virtual craniofacial models by fusing 3?D photographs and CBCT images. Study 1: A stereophotographic system was used to capture 3?D images of 103 Hong Kong Chinese young adults with normal balanced faces. An anthropometric analysis protocol with linear, angular and proportional measurements was developed to establish a normative database and quantify dysmorphology. The Hong Kong Chinese norms were distinct from Caucasians, especially with regard to ocular and nasal measurements. Facial height and nasolabial measurements differed significantly between Hong Kong males and females. Study 2: Anthropometric analyses of 3?D facial images from 41 skeletal Class II and 43 Class III subjects were performed. The Class II subjects had increased lower facial height compared with Class III, who had longer total facial heights and narrower faces. While Class II deformity primarily resulted from mandibular deficiency with a normal maxilla, Class III presented as combined midfacial hypoplasia and mandibular hyperplasia. Study 3: Anthropometric characteristics of the oropharygeal space in skeletal Class II and III were evaluated using 62 CBCT scans. The retroglossal (RG) and retropalatal (RP) volumes and average cross sectional areas were significantly larger in Class III than Class II skeletal deformity. The RP compartment was larger but less uniform than the RG compartment in both Classes. Study 4: 3?D photographs or CBCT images acquired at two different time points were superimposed using a common unaffected area. 3?D colour maps were generated depicting distance differences between superimposed images in a graphical format. These maps were used as an objective tool for treatment planning and assessing outcomes after orthognathic surgery, bimaxillary distraction and facial trauma. Study 5: 3?D photogrammetry was employed for planning soft tissue expansion (STE) and transplantation of a vascularised scapular flap in hemifacial microsomia. This technique facilitated the identification of extent and degree of tissue deficiency, selection of the appropriate tissue expander, monitoring volumetric changes during STE and estimation of the free flap dimensions. Study 6: 3?D facial photographs and CBCT scans of 29 subjects were merged to create virtual craniofacial models with natural surface texture. Accuracy was assessed with 3?D

Book excerpt: Objectives: The objective of this study was to find out if the soft tissue changes after the correction of dentofacial asymmetry with orthognathic surgery will tend to regress to the original deformity. Materials and Methods: A total of 20 patients were recruited in this retrospective study which was performed at Yale New Haven Hospital and University of Connecticut. Patients were divided into two groups: Experimental (8) and Control (12). Stereophotogrammetric Images were obtained for each patient at five timepoints: (T0) Pre-Surgery, (T1) Post-surgery, (T2) 1 month after, (T3) 6 months after, (T4) 1-year after. Fifteen landmarks were identified on faces of each patient at each timepoint. Each landmark had its own of X, Y, and Z coordinates. The coordinates of each timepoint were then compared within each group and between groups. Results: Statistically significant differences were found for points in the lower chin area (Point 10, 11, 12 and 15) in the transverse plane (X-axis) for experimental patients between T0 and T1. Control patients had statistically significant changes in the transverse plane for Points 1 and 15 when comparing T0 and T1. All the bilateral points in our study were separately analyzed for their vertical (Y) and anterior-posterior (Z) changes over time. For vertical changes in the experimental group, Points 13 and 14 had a statistically significant difference before surgery and at 1-month post-surgery when compared to each other. For vertical changes in the Control group, comparison between Points 8 and 9 at 6-month timepoint were statistically significant. When analyzing changes in anterior-posterior direction, we did not see any statistically significant differences in our Experimental group. However, in the Control group, Points 13 and 14 were statistically significant different at Pre-Surgery, 1-month and 6-months for the Z values. Conclusions: Our data suggests that asymmetry improved significantly after surgery for all the lower chin points in the Experimental group after orthognathic surgery when compared to patients in the Control group. Since we did not see any significant changes occurring post-surgery for the lower chin points in the experimental group, we conclude that the asymmetry of the chin does not regress over time back to its original location in asymmetric patients. No other clinically significant results were observed in this study.
