Several techniques have been suggested to reconstruct bone defect including guided bone regeneration (GBR), based on the use of a subperiosteal barrier. Recently Ti-mesh received growing attention. Hence the creation and the maintenance of a space for graft placement could prevent.The aim of this study was to evaluate alveolar ridge augmentation using bone graft associated with Ti-mesh.the collapse of the biomaterial and provide GBR.A total of 4 patients were selected for alveolar ridge reconstruction for implant placement purposes. Patients were healthy, with nosystematic or local contraindication to intraoral surgery and implant placement. Each patient underwent a bone augmentation by using a Ti-mesh filled with autogenous bone graft mixed with a Xenograft. The peri-implant bone gain was retrospectively evaluated at a mean follow-up of 12 months concerning 4 dental implants.The overall clinical and radiographic results of the present study show a survival and success rate of 100% according to Albrektssonet al.u2019s proposed criteria .CBCT were performed before each surgical procedure and during the follow-up recalls. Software was used to measure the bone gain around each implant. The mean horizontal bone gain was 2.175 mm and 2.875 mm vertically at a mean follow-up of 12 months.The use of Ti-mesh allows the regeneration of sufficient bone volume for ideal implant placement. The clinical advantages related to this technique include the possibility of correcting vertical and horizontal bone defects and the lack of major complications if soft-tissue dehiscence and mesh exposures do occur.

Therefore, horizontal or vertical alveolar bone augmentation procedures usually cannot be avoided. Many studies have shown predictable results for GBR on infrabony defects. However, the GBR performed on extrabony defects still lacked predictability. The use of three-dimensional, preformed titanium membranes can achieve GBR by ensuring graft stability and space maintenance so as to ensure optimal neovascularization on extrabony defects as well as infrabony defects.u25a0 ObjectivesThis study aims to clinically and radiographically evaluate the results of bone gain in extrabony defects after GBR using three-dimensional, preformed Ti-membranes. u25a0 Materials and MethodsThis study was conducted clinically on 74 patients (114 implants) with extrabony defects which existed around dental implants at the time of implantation.Patients were evaluated clinically and radiographically by using cone beam computed tomography (CBCT) at baseline, Implant 1st surgery, after 5 months each. At first, the exposure of three-dimensional, preformed Ti-membrane is evaluated between first surgery and second surgery. Secondly, Horizontal bone gain was evaluated at implantu2019s platform level on 114 implants(95 only horizontal GBR cases , 19 vertical GBR cases) at 3 times as described above.u25a0 ResultsIn this study, no severe pain was recorded postoperatively and no signs of inflammation or swelling were observed in almost all of the patients. Membrane exposure occurred in 3 (4%) of 74 patients and occurred in 3 (3%) of the total 114 implants. At the platform level, the mean horizontal bone gain in only horizontal GBR cases was 3.3 (u00b1 1.2) mm at 5 months postoperatively. The mean horizontal bone gain in vertical GBR cases was 3.3 (u00b1 1.3) mm at 5 months postoperatively. In almost all cases, it was possible to assure more than 2 mm of buccal bone width which is important for the long-term prognosis of the implant.u25a0 ConclusionThree-dimensional, preformed Ti-membrane should be considered as a valuable option for GBR on extrabony defects.
