Infants with ESRD often require renal replacement therapy (RRT), commonly known as dialysis, for treatment. Although they are a minute subset of children, like other children with medical complexity (CMC), infants who require renal replacement therapy (RRT) are medically complex and need high levels of professional and home care. Although treatments are proving more promising, infants with ESRD have historically poor outcomes. Medical decision making for parents of infants with end stage renal disease can be a process fraught with conflict, uncertainty, discord, and stress. Questions of which treatment to pursue or whether or not to forego life-sustaining treatment, weigh heavily. Knowing which decision to make is not a simple task. To aid in decision making, healthcare professionals have promoted Shared Decision Making (SDM) as a model, however, the vast majority of individuals who become patients, still are not supported with the SDM model. The SDM model and others have not been adequately studied from the parent and patient perspective. Parents of infants with ESRD and other CMC, are affected emotionally, financially, physically and relationally by the everyday parenting and care of their child. Decision making is another task that increases the burden to caregivers. This dissertation will provide an explanation of the experience of parents making decisions for infants with ESRD, the meaning made for mothers within that experience, situating both in current research. The dissertation the provides the implications of experience and meaning findings--to social work practice, policy, and education. Article 1 describes the experience of mothers, resulting in four essential themes: (a) information and communication are important, (b) I want to give my baby every chance, (c) it's hard to know for certain what to do, and (d) mothers and fathers take different roles in care giving and decision making. Article 2 provides an explanation of the meaning that mothers of infants with ESRD make of their decision making and caregiving experience: 1) Information has its limits, and 2) Mothers pull the load. Understanding the broader experience and meaning of decision-making for parents, and specifically, mothers of infants with CMC, may increase the effectiveness of decision-making techniques, interventions, and tools developed for parent caregivers in the future. In addition this dissertation and the articles included in it provide a close look at author reflexivity in qualitative study as well as a template for studying the broad experience of parent decision makers that includes beginning with experience, adding meaning in order to reveal a practical purpose and meaningful implications for social workers, social work reserachers and other healthcare professionals

Until the 1950s, chronic renal failure was considered to be an inexorably lethal condition. This is no longer the case. In addition, the disease, severe uremic syndrome, is now extremely rare, if existent at all, in industrialized countries. Physicians of my generation who saw patients hospitalized with hemor raghes, pericarditis, severe anemia, cardiac failure, "malignant hypertension," pruritus, vomiting, generalized edema, and convulsions are particularly grate ful for this progress. I well remember seeing such patients hospitalized in the last days or weeks of their lives and also remember the sense of impotence I suffered for the com plete lack of efficient measures I had at my disposal to manage their condition. Nowadays, hemodialysis, peritoneal dialysis, and kidney transplantation allow patients with chronic renal failure to survive for very long periods of time in a satisfactory condition. Why then is there still a sense of dissatisfaction and why should we study dietary management? The drawbacks of dialysis and transplantation are the main reasons, but the certainty that dietary therapy is complementary to dialysis and even better than dialysis in certain conditions, is also very important.

A number of them are interested in problems of renal insufficiency and kidney replacement therapy. Therefore, many vital meetings concerning this vibrant speciality have been included in international pediatric congresses. But the discussion of ten-minutes-papers in a large auditorium was often limited, criticism tended to be muted, lively discussions have only occasionally succeeded for the reason that the topics have tended to be too specialized for the audiences in relation to the large number of other discussions about questions and results in pedeatric science. In Cologne, therefore, we decided to hold a conference concerning problems, advances and controversies in renal insufficiency in children, which would have a limited but general audience in rooms of a modest size, topics that tended to polarize a world-wide faculty of experts who liked their basic views to be opposed. The session would be chaired by persons well-known for inciting discussion and even difficult questions in order to stimulate a vital debate between delegates and audiences. One important part of the meeting was therefore the discussion in an open and familiar way as it is known in other workshops. We called the conference "KaIner Kinderdialyse-Symposium". The first conference was held in November 1975, and evoked an extra ordinary enthusiastic response from both audiences and delegates, so we held a second one in 1978 and the third meeting was in 1981.
