Book excerpt: The induction of immune responses against tumor cells by vaccination is rapidly evolving as a therapeutic modality with new potentials for the treatment of cancer. It is based on the fact that our immune system can identify tumor cells and, once activated, is capable of developing specific immunity against the neoplastic cells. Numerous observations and intense research clearly document the major contribution of the immune system to the prevention of cancer. And there are many re ports of patients suffering from malignant melanoma or other tumors who mount a spontaneous immune response against their tumor cells that results in tumor regression. Based on the recent advances in our understanding of the compo nents of our immune system, their interactions and the regulation of immune responses, we are now able to design vaccination strategies that induce or enhance cell-mediated immunity against tumors. A ma jor advancement came with the identification and characterization of relevant tumor antigens, which are suitable target structures for anti-tu mor immune response. First clinical trials using such vaccine strategies have yielded encouraging results in patients. However, in spite of many reported cases of successful therapy of cancer by vaccination many patients still do not experience relief after such treatments. These initial clinical trials and the accompanying investigations have revealed a number of important results that indicate the direction of future re search and development in the field.

Animal studies confirm that αGal epitopes expressed on allogeneic tumor vaccines elicit a potent T-cell-dependent antitumor immunity. Based on these immunologic reactions, we hypothesized that the hyperacute rejection mechanism could be exploited to alter antigen processing resulting in a novel therapeutic approach to treat human malignancies. Clinical trials data confirm that an immediate hypersensitivity response directed toward a vaccine composed of genetically modified allogeneic tumor cells expressing the xenoantigen αGal (HyperAcute vaccines) constitutes a polyvalent tumor cell vaccine with signs of clinical efficacy, concomitant to eliciting both a humoral IgG response as well as T-cell-mediated antitumor immunity. This conceptually innovative immunotherapy degrades tumoral immune escape and portends a promising genetic engineering tactic for the cost-effective development of a generally applicable human cancer vaccine principle with minimal toxicity. Encouraging results support additional clinical immunotherapy studies using HyperAcute vaccines.

In the United States alone about 50 million people regularly smoke tobacco and another 5 million are addicted to other drugs. In a given year, millions of these individuals attemptâ€"with or without medical assistanceâ€"to quit using drugs, though relapse remains the norm. Furthermore, each year several million teenagers start smoking and nearly as many take illicit drugs for the first time. Research is advancing on promising new means of treating drug addiction using immunotherapies and sustained-release (depot) medications. The aim of this research is to develop medications that can block or significantly attenuate the psychoactive effects of such drugs as cocaine, nicotine, heroin, phencyclidine, and methamphetamine for weeks or months at a time. This represents a fundamentally new therapeutic approach that shows promise for treating drug addiction problems that were difficult to treat in the past. Despite their potential benefits, however, several characteristics of these new methods pose distinct behavioral, ethical, legal, and social challenges that require careful scrutiny. Such issues can be considered unique aspects of safety and efficacy that are fundamentally related to the distinct nature and properties of these new types of medications.
