Book excerpt: Aim : The purpose of this study was to evaluate the clinical efficacy of four oral hypoglycemic agents combination therapy (4-OHA) in patients with poorly controlled type 2 diabetes mellitus (T2DM) who cannot accept insulin therapy.Methods : Forty-seven patients with T2DM were enrolled according to the following criteria: 1) HbA1c > 8.5% (69 mmol/mol); 2) ongoing treatment with 3-OHA combination (metformin, sulfonylurea, and dipeptidyl peptidase-4 inhibitor); 3) limitations for the application of insulin therapy. The patients were administered the fourth OHA (pioglitazone) in addition to the previous treatment for 24 months. We evaluated changes in HbA1c, body weight, hypoglycemia events, and side effects. The control group consisted of 50 patients who underwent basal insulin therapy under identical clinical conditions.Results : At study completion, there were no significant differences in HbA1c improvement between the 4-OHA group and the control group [-1.41 vs -1.56% (-15.4 vs -17.1 mmol/mol), P=0.207]. There were also no differences in hypoglycemic events (n=10 vs 14, P=0.298) and weight gain (+2.79 vs +2.30kg, P=0.21)Conclusion : The 4-OHA combination therapy has sufficient glucose lowering effects in some patients. When insulin is not feasible, 4-OHA combination therapy may be considered as an alternative therapy.

In the absence of contraindications, medication should be started when the HbA1c concentration exceeds 6.5–7.0% (48–53 mmol/mol) despite non-pharmacological intervention. Metformin is the drug of choice. If one antidiabetic drug does not lower the HbA1c value to below 7.0 % (53 mmol/mol), add another and possibly a third antidiabetic drug unless there are contraindications. Insulin therapy must be initiated at the latest when HbA1c remains over 7.0% despite medication with oral antidiabetic drugs or with injectable GLP-1 analogues. A GLP-1 analogue may also be combined with basic insulin treatment.

Book excerpt: The prevalence of diabetes is on the increase in the UK and worldwide, partly due to changes in lifestyle which predispose individuals to overweight and obesity. It is estimated that about 90% of the currently diagnosed adults have type 2 diabetes, and based on the World Health Organisation (WHO) report, about 422 million adults were living with diabetes in 2014 compared with 108 million in 1980; this condition caused about 1.5 million deaths in 2012. In the United States of America, it is estimated that about 30.3 million adults are living with diabetes, with a further 1.5 million new diabetes cases diagnosed every year, representing an increasing prevalence of this condition. Diabetes represents a major public health challenge, despite advances in technology and the pharmaceutical industry. These problems may be in the form of acute or long-term complications. Therefore, in order to attenuate the problems of diabetes, management strategies usually include lifestyle changes such as increased physical activity and dietary interventions. Studies which evaluate the role of nutrition in the management of type 2 diabetes often involve human and animal models as these approaches enable us to have a broader and more in-depth understanding of the condition. In some cases, diabetes may co-exist with other conditions, such as stroke, and these may present unique challenges with regard to nutritional interventions. This Special Issue aims to evaluate the risk factors associated with type 2 diabetes and the role of the diet in the management of people with this condition. This evidence is drawn from both human and animal studies.
