Book excerpt: Excerpt from Theoretical and Practical Treatise on Astigmatism The addition of one more medical book - even though it be a small one - to the scores that are annually issued from the press, carries with it a demand, on the part of the reading public, for its raison d'etre. The cause of the existence of this little work has its foundation in my own needs as developed in my studies of refractive anomalies, and in my conception of the needs of others as manifested to me during the last eight years as a teacher of general and special students and general practitioners. An examination into the statistics of eye-affections shows that the various anomalies of refraction form about one-third of the whole number of eye cases presenting themselves for treatment in private practice, and of these two-thirds, or about 20% of the whole, suffer from astigmatism in an appreciable degree. No one, then, I think, will deny to astigmatism the worth of a treatise special to itself, particularly when there is taken into consideration the great difficulty encountered in clearing away the perplexities, uncertainties and paradoxical manifestations which enshroud many cases of the anomaly, and which are the despair of the beginner in refraction studies. And, while fully aware that Time and Patience are two most important and indispensible factors in unraveling the tangled threads of evidence, it cannot be doubted that nowhere in the whole range of medical practice, is accurate knowledge, based on positive science, of such avail as in the diagnosis of astigmatism. To lead to such accurate knowledge through the paths of positive science has been the chief incentive to my labor. Histologists marvel at his early descriptions of cells; physiologists admire his attempts to relate structure to function; pharmacologists view in awe his heroic experiments on self-administered drugs; forensic scientists acknowledge his role in the use of fingerprints for identification; and Czech patriots salute his awakening of pride in their nation. Yet all these achievements followed his initial enquiries into vision. It is this psychological dimension that fostered this collaboration. As the title suggests, the present volume is bifocal. In the narrow sense it refers to Purkinje's studies of vision, but in its broader view it concerns Purkinje's anticipation of neuroscience. Purkinje provided evidence to support both its cellular and its conceptual base. At the cellular level his acute vision is immortalized within our bodies. At the conceptual level, he sought to relate subjective phenomena to their objective underpinnings--to link psychology to physiology. Vision provides a bond that unites psychology and physiology, and it is this bond that was strengthened by Purkinje's enquiries. The authors have tried to provide a context in which Purkinje's descriptions of visual phenomena can be placed. In some cases this exposes clear precursors of research for which Purkinje has been credited. In others, there was nothing to suggest the phenomena that he exposed. The book translates Purkinje's initial masterpiece on subjective vision and places it in the context of emerging views of neuroscience.
